Summary :
The World Health organization has named this decade the “Bone and Joint Decade”, in anticipation of new methods of prevention and treatment of musculoskeletal disorders.
The past decade has provided us with much new information on calcium homeostasis. Identification of the PTh receptor and its mutations have now been linked to a number of disorders for which there were no previous explanations, for example Jansen Type Metaphyseal Chondrodysplasia. New discoveries about the calcium sensing receptor mutations have led to the development of calcimimetic drugs, which potentiate the activity of calcium and will be available soon. Various genetic studies have emerged in the past decade giving insights into the origin of diseases such as hypophosphatemic rickets and others. An in-depth update on osteoporosis and other bone diseases is also given.

Summary :
In this highly interesting talk, Dr Karaplis shares his expertise on various topics in metabolic bone disease. Molecular and cellular background leads into a descriptive analysis of disease states and their diagnosis and treatment. Diseases covered include osteomalacia, hypercalcemia, hyperparathyroidism, vitamin D deficiency, osteoporosis, rickets, parathyroid hormone-related peptide (PTHrP) signalling, pseudohypoparathyroidism and hypocalciuric hypercalcemia.
Learning objectives :
The participant will gain background knowledge on the molecular mechanisms involved in the normal and disease states of bone metabolism; as well as the diagnosis and treatment of these diseases. Conditions discussed include osteomalacia, hypercalcemia, hyperparathyroidism, vitamin D deficiency, osteoporosis, rickets, pseudohypoparathyroidism and hypocalciuric hypercalcemia.

Vitamin D Therapy on Bone Disorders
Clinical Study Title:
Early phase metabolic bone disorders after gastrectomy: influence of active vitamin D treatment. Plain English Summary: This study examined the use of vitamin D therapy on bone disorders that are common after a gastrectomy. A total of 63 participants were given either vitamin D therapy or not given any additional therapy in the early stages post surgery. The vitamin D supplementation helped to increase calcium and phosphate levels, whiled helping to modify the parathyroid hormone. This suggest that the early stages after a gastrectomy can be pivotal in the development of bone disorders, and that vitamin D therapy can help to prevent bone disorders. Authors

PAEDIATRIC BONE DISORDERS
Background info:
Dwarfism can be PROPORTIONATE - manifesting a symmetric decrease in both truncal and limb length. Dwarfism can be DISPROPORTIONATE - manifesting either a short limb disorder or a short trunk disorder. Short - limbed dwarfism can affect either the proximal, the middle or the distal region of a limb. FORMS OF DYSPLASIA include: Achondroplasia Spondyloepiphyseal Dysplasia Chondrodysplasia Punctalta Kneist Syndrome - AD Metaphyseal chondrodysplasia Multiple Epiphyseal Dysplasia Dysplasia Epiphysiallis Haemimeliea [known as Trevors Disease] Progressive Diaphyseal Dysplasia - AD Mucopolysaccharidosis Diastrophic Dysplasia - AR Cleidocranial Dysplasia - AD Benign Bone Growth Dysplasia
A) Achondroplasia Achondroplasia is the most common form of disproportionate dwarfism. It is an autosomal dominant (AD) condition with an 80% spontaneous mutation. It is caused by abnormal endochondral bone formation [in the proliferative zone] and can be associated with late-in-life childbirth. It is a quantitative [not a qualitative] cartilage defect. The afflicted child will have dwarfed limbs and a normal trunk.

Kraemer, MD (Professors of Medicine, Stanford University School of Medicine, Stanford, California) Evaluation Version Palm OS PocketPC About PocketMedicine Software PocketMedicine was founded in August of 2000 by Lewis Reines, a medical publisher with long experience in editorial and executive positions in the medical publishing business. It was his view that the use of handheld devices by healthcare professionals was an area of great potential for growth, and that the most effective medical content for use on those devices was going to be that written expressly for the handheld platform. That is, original content, written and edited by recognized authorities, for use at the point of care. All of PocketMedicine's publications are new and original; they are not books which have been repurposed for use on handheld devices. Our publications provide the right answer...the expert's answer...when and where it is needed most.

The Relationship between Weight and Bone Disorders Author Felson (1996) Conclusions
*Literature review shows knee osteoarthritis to be four times more likely for individuals with a BMI of 30 or higher as compared with individuals with BMIs lower than 25.
Concern *All cross-sectional studies reviewed did not demonstrate the same positive relationship between obesity and osteoarthritis. Author Wardlaw (1996) Conclusions
Concern
*Higher BMI is a potential risk factor for one bone disorder (osteoarthritis), but appears to be protective against another (osteoporosis).
*The current evidence does not support citation of increased health risks to the skeletal system as justification for defining increased BMI as a risk to health. References Next Back to: The Evidence Related to Weight and Health
