To your dismay, your daughter has started to complain more and more about the appearance of her eyelids. She grudgingly compares them to those of her classmates. You frequently catch her standing before a mirror, scrutinizing their appearance. When you try to discuss your concerns, she becomes defensive. To make matters worse, you've observed her reading materials about cosmetic surgery. How do you know if your daughter is simply experiencing a typical stage in adolescence or if she has a more complex problem? Teens seem to worry incessantly about their weight and appearance, but some may become obsessed with a specific flaw or perceived defect. Along with eating disorders , body dysmorphic disorder (BDD) has become a growing concern for young adults. The severity of this disorder varies. Some are able to function and cope with daily life, whereas others experience paralyzing symptoms of depression anxiety , and avoidance of social situations. "These adolescents have a very distorted view of how they look, and it does not match how other youth see them," says Katharine Phillips, MD, director of the Body Image Program at Butler Hospital in Providence, Rhode Island. Real Life Stories For 17-year-old James, the simple act of looking in a mirror is torture. Ever since a friend made a casual comment about his appearance, James has been obsessed with the size and shape of his nose. "It's just not right... it just doesn't fit my face" he will state. James is unable to be reassured that there is nothing objectively wrong with his nose and will spend at least an hour per day checking himself in various reflections. He has begun to consult with cosmetic surgeons about the possibility of rhinoplasty. "I wish I could convince my parents to take me to a plastic surgeon" said 15-year-old Kristin. She is preoccupied with her "large" jaw, "small" breasts and "uneven" skin and will ask her mother whether she looks okay at least a dozen times per day. She has begun to use heavy makeup and has also started wearing long sleeves and pants at all times in order to cover her skin. Kristin's appearance concerns are so time-consuming and distressing that she has ceased to spend time with her friends and has dropped her extracurricular activities. Both James and Kristin have Body Dysmorphic Disorder.

A Scottish hospital has launched an inquiry after a surgeon agreed to remove healthy limbs from patients suffering from a psychological disorder. BBC News Online looks at the condition. What is Body Dysmorphic Disorder? People with body dysmorphic disorder (BDD) worry about their appearance, believing, for example, that their skin is scarred, that they are balding or their nose is too big. They refuse to believe reassurance from others that their appearance is not abnormal. The condition's severity varies - some people can manage it, others have their lives ruined by the disorder. There have been cases of suicide linked to BDD. Dr Ian Steven, a psychologist in Edinburgh, said: "People become fixated or concerned with particular areas of their body and have difficulty accepting that there is no illness present. "Most people resolve problems by going to see a doctor or get issues addressed by expert specialists, getting it clarified that there is nothing wrong with the part of the body they are concerned about."

It is often assumed to be women who suffer from extreme dissatisfaction with their body image. But experts say men are just as likely to be unhappy with the way they look. In this week's British Medical Journal, experts estimate triple the number of men suffer from body dysmorphic disorder - a severe form of body image dissatisfaction - now, compared to 25 years ago. But they say the condition is under-recognised and under-diagnosed. Up to 1% of the population are estimated to suffer from the disorder in the UK - though more could more who don't admit their concerns.

W hether by means of television, newspapers or magazines, we are constantly presented with images of perfect faces and bodies. While maturing in such an environment, the impressionable adolescent develops a mental image of how the ideal man or woman should appear. Persons who become preoccupied with perfection often view themselves as imperfect and may develop a distorted picture of their own body and face. This perception can lead to unhealthy behaviors such as eating disorders and body dysmorphic disorder (BDD). Often, the primary care physician has the first opportunity to intervene with these patients. Definitions and Etiology Patients with BDD are excessively preoccupied with a minor or an imagined defect of a specific body part, resulting in impaired social, academic or occupational functioning. Body image is defined as a mental picture of the size, shape and form of our body. It also describes our feelings about these physical characteristics. Body image is divided into the following two components: how we perceive the appearance of our body and our attitude toward our body. A significantly distorted perception of the body may lead to self-destructive behaviors aimed at improving the appearance of the body.

The Body Dysmorphic Disorder (BDD) Clinic and Research Unit at MGH/Harvard Medical School was founded in 1998 by Sabine Wilhelm, Ph.D. It is one of the few clinics in the United States specializing in Body Dysmorphic Disorder (BDD). The BDD Clinic consists of a treatment program and a research unit. Patients are treated with cognitive behavioral treatment and/or medication. The research unit currently focuses on studies regarding information processing, perception and interpretation, neuropsychology, prevalence, and treatment of BDD. Besides active clinical and research services, the clinic serves an educational purpose with research fellows and graduate students currently involved in clinical and research activities. WHAT IS BDD? Individuals with Body Dysmorphic Disorder (BDD) are very concerned about their physical appearance, most commonly about their skin, hair, nose, or ears. Some, for example, report about having red scars, long noses, or small eyes, even if others reassure them that they look fine or that the flaw is not noticeable. Individuals with BDD frequently think about their appearance, at least for an hour per day. They often have problems controlling negative thoughts about their appearance.

Body Dysmorphic Disorder (BDD) Description
Individuals with Body Dysmorphic Disorder (BDD) are beset by extreme concerns about their appearance. The disorder usually begins in adolescence and is characterized by a time-consuming and potentially disabling preoccupation with imagined or slight defects in one's appearance or excessive concern about a slight physical anomaly. To meet the criteria for this diagnosis, the preoccupation must cause significant distress or impair school, personal or social functioning. Although virtually any body part can become the source of preoccupation, BDD most commonly involves the eyes, ears, nose, skin, chin, jaw or other facial features. The most frequent co-occurring psychiatric disorders with BDD are obsessive-compulsive disorder, social anxiety disorder and depression. Suicidal thinking and gestures are, unfortunately, not uncommon consequences of BDD. Causes: Although the precise cause of BDD is unknown, most likely there are multiple sources including biological, psychological and sociocultural factors.
