Blood disorder (or blood disease) is any irregularity in the structure or functioning of blood parts, such as cells, platelets and a fluid called plasma that can affect the body's ability to fight infections or be a sign of organ damage. Types of blood disorders affecting the heart include anemia (e.g. sickle cell anemia or iron deficiency anemia), antiphospholipid syndrome and thrombotic thrombocytopenic purpura (TTP). Anemia is a lack of red blood cells and/or the iron-rich molecule in red blood cells (hemoglobin) that carries oxygen throughout the body and gives blood its red color. There are over 30 different types of anemia, including:
  • Iron deficiency anemia. An abnormally low level of iron in the blood.
  • Sickle cell anemia. A painful, inherited condition in which the red blood cells are shaped more like half-moons or sickles than round discs.
  • Aplastic anemia. Fat cells replace some of a patient's bone marrow.
Common symptoms of anemia include chronic tiredness, weakness, paleness and shortness of breath. These symptoms result from a lack of oxygen-rich blood to the brain and other areas of the body. In some cases, patients may also show heart-related signs or symptoms of anemia, such as irregular heart rhythm (arrhythmia), heart murmur or chest pain. Antiphospholipid syndrome (APS) is a type of autoimmune disorder in which antibodies attack the body's own phospholipids (a type of fat that contains phosphorous). Without treatment it may lead to blood clots and other peripheral vascular disease.

Hematologists, specialists in disorders of the blood, are often helpful in the medical management of patients with systemic lupus erythematosus (SLE). Indeed, it is not unusual for the initial signs of lupus to appear in the blood. In some patients with lupus, a blood disorder is the primary symptom of the disease. The major hematological symptoms of SLE are: anemia, thrombocytopenia (low platelet count), clotting disturbances, and low white blood cell counts. Each of these problems is discussed further.
ANEMIA
The most common hematological abnormality in SLE is anemia, a reduction in the number of red blood cells. Anemia is recognized by any of the following tests: the hematocrit, the hemoglobin concentration in blood, or the red blood cell count. About half of the patients with active lupus are anemic. The severity of the anemia is usually proportional to the activity level of a person's lupus. Fatigue, suffered by many patients with active lupus, can be attributed in part, to anemia. Anemia is not a specific condition, but has many causes. In a person with lupus, anemia may be caused by chronic inflammation, prolonged uremia, iron deficiency, or hemolytic anemia.
