Blepharospasm (Eye Twitching) Printable Version (PDF) 425k Almost everyone at some time in his or her life has experienced a frequent and annoying muscle twitch of the eyelid. It may have followed a late night of studying or hours of driving, stress or drinking caffeinated beverages. Usually, this annoyance disappears after a period of rest and relaxation. Occasionally, the spasms are caused by an actual change in the eye's physical makeup and the spasms do not go away, but rather become a constant annoyance. Symptoms Due to a nervous system disorder, such as Tourette's syndrome, or an irritation on the surface of the cornea or the conjunctiva (the outermost lining of the eye under the eyelid), the eye will frequently or continuously twitch (spasm). Vision is generally not affected except in the most severe cases where the eyelid(s) remain shut. Frequently, dry-eye syndrome is experienced along with blepharospasm. Occasionally, blepharospasm is drug-induced and will be resolved when the dosage is reduced. Treatment The most common forms of blepharospasm will resolve after a period of rest. Usually, no treatment is necessary. Stress and anxiety can play a large roll in chronic eye twitching. Therefore, stress management skills may be one of the first treatments prescribed.

Definition: Blepharospasm is the repetitive contraction of eyelid muscles in a rhythmic fashion. In some instances, the eyelid may repeatedly close or nearly close.
Alternative Names: Eyelid twitch or spasm
Causes, incidence, and risk factors: The most common causes of muscle twitching in the eyelid are fatigue, stress, and caffeine. Once the spasms begin, they may continue intermittently for a day to more than a week, then disappear. Most people experience this type of eyelid twitch on occasion and find it extremely annoying. It usually stops suddenly, without the person noticing it.
More severe contractions with closure of the eyelid are thought to be caused by irritation of the surface of the eye (cornea), or the membranes lining the eyelids (conjunctiva). This type of eyelid twitching is common, especially in certain seasons. Another form of blepharospasm lasts much longer, is very uncomfortable, and can close the eyelids completely.
Symptoms: A person experiences repetitive, uncontrollable twitching or spasms of the eyelid, usually the upper lid. The person may also be very sensitive to light or have blurry vision.

Botox is a purified preparation of botulinum toxin (distributed and licensed by Allergan, Inc.) Excessive amounts of this substance is known to cause food poisoning and weakness in humans. However, the amount that is injected into the muscles of the face to treat blepharospasm is 100 times smaller than the dose that causes poisoning. No patient has had poisoning from this drug after being treated for eyelids spasms. While no poisoning has ever occurred when using Botox for eyelid spasms, the drug can cause side effects around the eye due to spreading into structures around the eye or due to excessive weakness of the eyelid muscles . These effects are temporary in almost all cases, resolving within 4 to 8 weeks.
1. The upper eyelid can droop a few mm. (risk 1 in 50) 2. Difficulty closing the eye can lead to drying of the eye and cornea (risk 1 in 10): Artificial tears and ointments may be needed to keep the eye moist. 3. Intermittent tearing can occur as the tear film is not pumped efficiently into the tear sac. (1 in 50)

INFORMATION ABOUT BLEPHAROSPASM
There are several disorders that cause excessive blinking or severe spasms (marked tightening) of the eyelids. Patients with these disorders find it difficult to keep their eyes open. Both the reflex and voluntary movements of the eyelids are usually involved. Two of the most severe disorders that lead to eyelid spasms are benign essential blepharospasm (BEB) and hemifacial spasm. Patients with blepharospasm show repetitive spasms of the eyelids of both eyes (see photos of a patient). Muscles around the mouth or in the neck can also go into spasm. It is a disease that usually develops in persons over the age of 50, and it can get worse over the years. It is thought that this disease is caused by chemical imbalances in the areas of the brain that control the movement of the muscles of the face. Patients with blepharospasm are usually diagnosed with the condition by an ophthalmologist or a neurologist. They frequently present to an eye doctor because they often have symptoms of dry eyes. When the usual treatments with dry eye medications do not work to stop the blinking, the diagnosis may, in fact, be essential blepharospasm. The presence of uncontrolled facial spasms can also lead to referral to a neurologist, who must differentiate the eyelid spasms of benign essential blepharospasm from those seen with other less common conditions.

  • Blepharopspasm was first described in the 16th century by the painting by Pieter Brueghel entitled "De Gaper." During the 16th century, patients were institutionalized in insane asylums Little progress was made in understanding and treating blepharospasm until the mid-1900's. In 1907, Meige described a similar patient with dystonia which is now termed Meige syndrome. In 1956, Henderson wrote a classic article on Blepharopspasm describing the disability and treatments. can occur with specific precipitating stressors, such as bright lights, fatigue, and emotional tension; as the course continues, it occurs more frequently during the day and it may intensify so that when the spasms occur the eyelids may remain forcefully closed for short periods of time. Baylor co-hosts scientific symposium on blepharospasm HOUSTON(July 29, 2002)New treatments and research for blepharospasm will be highlighted at the 20th annual international conference and scientific symposium of the Benign Essential Blepharospasm Research Foundation, Aug. 23-25 at the Wyndham Greenspoint Hotel. Baylor College of Medicine is co-hosting the event. Blepharospasm is a condition in which the muscles in the eye spasm and the eyes involuntarily close, rendering the individual essentially blind. Blepharospasm usually begins during the most productive period in an individual's life, starting with excessive blinking or eye irritation. As the condition progresses, the spasms may intensify and interfere with the patient's ability to see while reading, watching television, driving or otherwise pursuing usual activities of daily living. The eyelids may remain forcefully closed sometimes causing not only blindness but also a great deal of discomfort and pain. "Blepharospasm is a very common disease, neurologically classified as dystonia," said Dr. Joseph Jankovic, program director of the symposium and director of the Parkinson's Disease Center and Movement Disorders Clinic at Baylor. "But it is often misdiagnosed as nervousness or some type of eye disorder, when it is actually a disorder of the brain."
