    Bipolar disorder is often not recognized by the patient, relatives, friends, or even physicians. An early sign of manic-depressive illness may be hypomaniaa state in which the person shows a high level of energy, excessive moodiness or irritability, and impulsive or reckless behavior. Hypomania may feel good to the person who experiences it. Thus, even when family and friends learn to recognize the mood swings, the individual often will deny that anything is wrong. In its early stages, bipolar disorder may masquerade as a problem other than mental illness. For example, it may first appear as alcohol or drug abuse, or poor school or work performance. If left untreated, bipolar disorder tends to worsen, and the person experiences episodes of full-fledged mania and clinical depression. One of the usual differential diagnoses for bipolar disorder is that the symptoms (listed below) are not better accounted for by Schizoaffective Disorder and is not superimposed on Schizophrenia Schizophreniform Disorder Delusional Disorder , or Psychotic Disorder Not Otherwise Specified And as with nearly all mental disorder diagnoses, the symptoms of manic depression must cause clinically significant distress or impairment in social, occupational, or other important areas of functioning.

Introduction
I highly recommend the Expert Consensus Guidelines on Bipolar Disorder (2000) . Gary Sachs chaired the committee that surveyed recommendations of noted experts in the field, and the result is truly an expert consensus document on the current state of the art. The main difference between that document and this one is that the former focuses on treatments that have solid research documenting their efficacy, whereas I also include here treatment approaches that have smaller studies, case reports, or anecdotal evidence to support their use. My feeling is that there is value, too, in having access to knowledge of more speculative treatments, since some patients will not respond to a "classic" approach. I also provide more practical, hands-on information (eg about dosing, drug interactions, etc) than is offered in the more general guidelines. You are encouraged to read through the whole document or to browse sections of immediate interest. Any underlined colored text is a hyperlink to another section of this document, to another related document, or to a page on the web.

Truehope estimates that Bipolar disorder affects about 2.2 million Americans over the age of 18, it costs the government millions of dollars each year, and puts an enormous strain on the medical system. Much like diabetes, or heart problems, bipolar disorder must be treated, and managed for many years, and according to Truehope there is no quick cure. As stated earlier those suffering with bipolar disorder have many problems in their personal relationships. Truehope suggests that they can quite often alienate their friends and loved ones because of their mood swings, and their occasional break with reality. Truehope says that those with bipolar disorder often cannot hold down a job, or be trusted with any sort of responsibility, because their behaviour is too erratic, and their moods can change so rapidly.

What is bipolar disorder?
Bipolar disorder is a mental illness that causes extreme mood swings. This condition is also called manic-depressive illness. It may be caused by a chemical imbalance in the brain. Bipolar disorder sometimes runs in families. If you have a parent who has bipolar disorder, you have a greater chance of having it. Both men and women can have bipolar disorder. What are some of the signs of bipolar disorder?
At times, a person who has bipolar disorder may feel very happy, full of energy and able to do anything. The person might not even want to rest when he or she feels this way. This feeling is called mania (say: "may-nee-ah"). At other times, a person who has bipolar disorder may feel very sad and depressed. The person may not want to do anything when he or she feels this way. This is called depression. People with bipolar disorder can quickly go from mania to depression and back again. Other signs of mania may include the following:
  • Feeling very irritable or angry
  • Thinking and talking so fast that other people can't follow your thoughts
  • Not sleeping at all
  • Feeling very powerful and important
  • Having trouble concentrating

    In the normal course of life, everyone has times when they feel somewhat less than happy and optimistic. Although many people attribute their feelings of sadness or lethargy to the onset of depression, it's important to understand that medical professionals have specific indicators that they use to make a diagnosis of clinical depression. When you know more about what's wrong you can make it right.
    Let's start with the word "clinical." By definition, a condition is clinical if it is directly observed. That is, the professional does not count on a patient's self-report but is more likely to check for symptoms of depression that can be observed and documented. Clinical depression is a phrase used to describe a condition serious enough to require clinical (professional) and possibly pharmacological intervention. Clinical depression, according to medical sources, lasts more than two weeks and is usually not precipitated by anything external.
    For example, if you've just lost your job and have no money in your bank account, you may feel stressed, anxious, panicked, and unable to function. However, most of your friends and family would reasonably conclude that it's quite normal to feel that way. Although your reaction might match the symptoms of depression, a traumatic event precipitated them and your state of anxiety would not be diagnosed as clinical depression. Clearly, a new and better job would probably end the negative feelings in an instant.

    The Laboratory of Developmental Neuroscience is dedicated to studying the molecular genetics of pediatric onset neuropsychiatric disorders. We use neurochemical and molecular genetic research tools to develop new knowledge that may lead to improvement in clinical pharmacology. We began as an HPLC lab studying the relationship between hyperserotonemia and autistic disorder. We are now working with collaborators on molecular genetic studies of autism, attention-deficit/hyperactivity disorder, childhood onset obsessive-compulsive disorder, stuttering, adolescent depression, and pediatric and early onset bipolar mood disorder.
