    Bipolar disorder, or manic-depressive disorder, is a mood disorder in which people experience alternating episodes of mania and major depression. Mania is characterized by elation, irritability, excitability, racing thought and speech, and hyperactivity. Major depression is characterized by sadness, withdrawal, despair, and suicidal thoughts. In the early 1900s, the German psychiatrist Emil Kraeplin was the first to formally describe bipolar disorder. He used the term "manic depressive" to explain how mania and depression both affect the patient. His work in the early 20th century led to advancements in classifying, treating, and predicting the course of mental illness, which ushered in the formal discipline of psychiatry. Bipolar disorder has two distinct classifications:
    • Bipolar I : history of major depression and at least one episode of mania
    • Bipolar II : history of major depression and much less severe episodes of mania (hypomania)

    WHAT IS BIPOLAR DISORDER?
    Bipolar disorder, which is also known as manic-depressive illness and will be called by both names throughout this publicationis a mental illness involving episodes of serious mania and depression. The person's mood usually swings from overly "high" and irritable to sad and hopeless and then back again, with periods of normal mood in between. Bipolar disorder typically begins in adolescence or early adulthood and continues throughout life. It is often not recognized as an illness, and people who have it may suffer needlessly for years or even decades. Effective treatments are available that greatly alleviate the suffering caused by bipolar disorder and can usually prevent its devastating complications.     What is Manic Depression?
    Manic depression involves wide mood alterations, with periods of both depression and mania. A person experiencing depression or mania may have intense mood swings and consequent changes in thinking and behaviour. The term that is professionally used for this illness is Bipolar Affective Disorder. Bipolar means sharing two poles (high and low) and Affective Disorder means a disorder having to do with mood.
    What Causes Manic Depression and Who Are The People At Risk?
    The cause is not known. Any person can develop manic depression, however, studies indicate that highly creative, sensitive people, people tending to be perfectionists and high achievers, have a higher incidence of bipolar affective disorder. Although biological factors seem to play a major role in producing the illness, a person's personality make up and or stresses in the environment may also play a part in bringing on an acute episode.
    How Many People Are Affected and What Are The Symptoms?
    It is estimated that 1% of the population will have a manic depressive illness. Environmental factors such as death, separation and divorce may trigger the disorder. The illness manifests itself with the individual experiencing episodes of mania or elation followed by low mood or depression. The number of manic and depressive episodes varies greatly from person to person and most individuals experience "normal" periods between their manic and depressive episodes.

    Bipolar disorder, also known as manic-depressive illness, is a brain disorder that causes unusual shifts in a person's mood, energy, and ability to function. Different from the normal ups and downs that everyone goes through, the symptoms of bipolar disorder are severe. They can result in damaged relationships, poor job or school performance, and even suicide. But there is good news: bipolar disorder can be treated, and people with this illness can lead full and productive lives. More than 2 million American adults, or about 1 percent of the population age 18 and older in any given year, have bipolar disorder. Bipolar disorder typically develops in late adolescence or early adulthood. However, some people have their first symptoms during childhood, and some develop them late in life. It is often not recognized as an illness, and people may suffer for years before it is properly diagnosed and treated. A person with bipolar disorder can go from feeling very, very high (called mania ) to feeling very, very low (depression).  With proper treatment, people can control these mood swings and lead fulfilling lives. While the rate of bipolar disorder is the same among African Americans as it is among other Americans, African Americans are less likely to receive a diagnosis and, therefore, treatment for this illness. Most African Americans with bipolar disorder are going undiagnosed and untreated.     Introduction to the Conference
    For more than a decade after the introduction of lithium pharmacotherapy, manic-depressive illness was thought of as a problem solved. Recently, however, both naturalistic studies and controlled trials have indicated that we still have much to learn about this puzzling and destructive disorder. In June 1994, the University of Pittsburgh Medical Center's Western Psychiatric Institute and Clinic sponsored the First International Conference on Bipolar Disorder , which attracted more than 400 participants. The conference provided an opportunity for researchers and clinicians to meet and share new information on the neurobiology and pathophysiology of bipolar illness, psychobiology and psychosocial vulnerability factors, and recent findings on epidemiology and clinical course. The Second International Conference on Bipolar Disorder , planned for June 1997, will continue the work begun at the first conference. the second conference will present an update on etiologic factors and treatment methods, including current information on the genetics of bipolar disorder as well as both biological and psychotherapeutic modalities for the acute and maintenance treatment of bipolar disorder. An increasing array of treatment options is becoming available for this disorder, and the roles and significance of these new therapies will be discussed. Top scientists and clinicians from around the world will be available to entertain questions from the audience following presentations and in panels. The conference will also feature " Ask the Experts" question-and answer sessions on June 19 and 20, 1997.

    If the patient experiences the manic symptoms but the symptoms are not so extreme as to significantly interfere with his/her daily life, then the episode is classifed as hypomanic. A person experiencing a hypomanic episode most often can keep his/her daily functioning and meet his/her responsibilities. Mixed episodes occur when the individual experiences a manic episode and a major depressive episode nearly every day for at least one week. An individual suffers from dysthymia when he/she experiences a depressed mood or sadness most of the time in a two-year period. For Dysthymia, the symptoms of depression are not so extreme as to warrant a diagnosis of Major Depressive Disorder.
    Bipolar I, Bipolar II, and Cyclothymic Disorders
    Now that the above terms have been defined, the distinctions between the different forms of bipolar disorder are easy. A patient who has ever suffered a manic episode or a mixed episode is classified as having Bipolar I disorder. It is very important to remember that in Bipolar I disorder, the individual may or may not have experienced a depressive episode. In most cases, however, individuals suffering from Bipolar I disorder also experience either major depression or dysthymia. If an individual has never experienced a manic or mixed episode but has experienced a hypomanic episode and also has major depressive disorder, he/she has Bipolar II disorder, in which both hypomania and depression must have been present. If the individual experiences only hypomania and dysthymia, he/she is classifed as having a Cyclothymic disorder. Many individuals who are initially diagnosed with Bipolar II or Cyclothymic disorders eventually develop Bipolar I disorders.
