Bipolar disorder in children appears more severe than in most adults The findings were presented today at the Fourth International Conference on Bipolar Disorder. The conference, held in Pittsburgh every other year, is the only venue in the world devoted exclusively to highlighting new research into bipolar disorder. The average age of the bipolar children involved in this study was just over 10. More than half had not yet reached puberty and 43 percent were between the ages of 7 and 10 years old. Almost a quarter of the bipolar study participants were seriously suicidal. Geller believes this low use of anti-manic medications may be because gatekeeper physicians are not yet aware that children can have manic -depressive illness. Only about one-third of these children had recovered from their mania at the end of one year. Geller plans to continue following these children over time to see if their chronic, rapid cycling illness continues or if they will develop the episodic pattern with relatively well periods that is seen most commonly in adults. Her group also is conducting molecular genetic and neuroimaging studies to learn whether the genetic factors involved in the disorder are similar for children and adults and whether the brains of children with bipolar disorder differ from the brains of normal children. Attention-Deficit Hyperactivity Disorder and Bipolar Disorder in Children
How Do They Relate?
By Melissa Sharpe and Chris Petersen, M.D. Introduction Let's first review some of the features of attention deficit hyperactivity disorder and bipolar disorder. Then we can look at the some of the shared features of these disorders and what that tells us about these children. Attention Deficit Disorder Attention deficit disorder is characterized by a persistent pattern of developmentally inappropriate attention or hyperactivity, which has been present for at least six months. Onset is before seven years of age and symptoms are present in two or more situations (i.e. home, school, work). There are three major features: inattention, hyperactivity, and impulsivity. The inattentive subtype is characterized by careless mistakes, trouble paying attention, difficulty listening, organizing and finishing tasks. Children can be easily distracted or forgetful. Children with the hyperactivity-impulsivity subtype often squirm in seats, fidget, and are always on the go. They may not be able play quietly, and may talk excessively. Impulsive behavior includes blurting out answers, difficulty waiting one's turn and interrupting or intruding on others. Bipolar Disorder Bipolar disorder has not been studied as extensively in children as it has in adults. This may be because bipolar disorder most frequently presents in late adolescence or early 20s. Bipolar disorder is an affective disorder in which the patient experiences cyclic intervals of mania and depression. Mania is characterized by the presence of an abnormally elevated, expansive or irritable mood, which lasts for at least one week or requires hospitalization. Mania is also characterized by symptoms of grandiosity, decreased need for sleep, increased talkativeness, flight of ideas, and distractibility. People with mania may also have increased goal directed behavior, psychomotor agitation, and excess involvement in pleasurable activities that have a high potential for painful consequences.

What is Bipolar Disorder or Manic-Depression?
Bipolar Disorder is a serious mental illness that affects a child's or adolescent’s mood and behavior. Young people with Bipolar Disorder have dramatic changes in their moods, alternating between feeling very depressed and feeling high or manic. They may have more normal moods between these episodes. The periods of depression or mania can last for days, weeks or even months. Because of the depression and mania, Bipolar Disorder is also called Manic-Depression. At this site, we will use the term Bipolar. In other places, you may see the term Manic-Depression used.

    Bipolar Disorder and Children
    Many children and especially adolescents experience mood swings as a normal part of growing up, but when these feelings persist and begin to interfere with a child’s ability to function in daily life, bipolar disorder could be the cause.  Bipolar disorder, also known as manic-depression, is a type of mood disorder marked by extreme changes in mood, energy levels and behavior.  Symptoms can begin in early childhood but more typically emerge in adolescence or adulthood.  Until recently, young people were rarely diagnosed with this disorder.  Yet up to one-third of the 3.4 million children and adolescents with depression in the United States may actually be experiencing the early onset of bipolar disorder according to the American Academy of Child and Adolescent Psychiatry.  Doctors now recognize and treat the disorder in both children and adolescents, but it is still an under-recognized illness.
    Children with bipolar disorder usually alternate rapidly between extremely high moods (mania) and low moods (depression).  These rapid mood shifts can produce irritability with periods of wellness between episodes, or the young person may feel both extremes at the same time.  Parents who have children with the disorder often describe them as unpredictable, alternating between aggressive or silly and withdrawn.  Children with bipolar disorder are at a greater risk for anxiety disorders and Attention-Deficit Hyperactivity Disorder (ADHD).  These "co-occurring" disorders complicate diagnosis of bipolar disorder and contribute to the lack of recognition of the illness in children.

    In recent years, our understanding of Bipolar Disorder has expanded to include reasonable deviations from this themeso called "mixed" states (coexisting manic and depressive symptoms), "rapid-cyclers" (4 or more episodes of mania and/or depression per year), and numerical subtypes such as alternating major depression with hypomania ("Type II").     What is Bipolar Illness?
    Table Of Contents The bipolar disorders are mood disorders. That means that amongst other things, there is a major change in mood. In bipolar disorders, this change in mood can be down, as in depression, or the opposite, mania. That is, a person can be inappropriately up. Some types of bipolar disorder have a lot of depression and only a little mania. Others have half and half. Still others seem to be both manic and depressed at the same time. Some people with bipolar disorders only have a few cycles of depression and mania. Others have many cycles a year. When bipolar illness is present in children and adolescents, it is more severe and harder to treat than when it occurs in adults. Pediatric Bipolar illness is one of the most severe conditions in pediatrics. In the milder forms, it can be disabling. In the severe forms, it can be lethal. The prognosis cancers in pediatrics is better than many forms of bipolar illness. All bipolar disorders are a combination of mania with or without depression. So what is mania? The age for first the onset of bipolar disorder once known as manic depression has plummeted from 32 to 19. The age for the first onset of bipolar disorder once known as manic depression has plummeted from 32 to 19. Bipolar disorder, a ferocious mental illness, seems to be showing up in kids at an increasing rate. As doctors look deeper into the condition and begin to understand its underlying causes, they are coming to the unsettling conclusion that large numbers of teens, children and even babies might also be suffering from the disease.
