Bipolar Disorder
Symptoms of Bipolar Disorder Types of Bipolar Disorder Treatments Bipolar Disorder in Children ... Support Groups Bipolar disorder (also known as manic depression) is a treatable illness marked by extreme changes in mood, thought, energy and behavior. It is not a character flaw or a sign of personal weakness. Bipolar disorder is also known as manic depression because a person’s mood can alternate between the "poles" mania (highs) and depression (lows). This change in mood or "mood swing" can last for hours, days weeks or months. Bipolar disorder affects more than two million adult Americans. It usually begins in late adolescence (often appearing as depression during teen years) although it can start in early childhood or later in life. An equal number of men and women develop this illness (men tend to begin with a manic episode, women with a depressive episode) and it is found among all ages, races, ethnic groups and social classes. The illness tends to run in families and appears to have a genetic link. Like depression and other serious illnesses, bipolar disorder can also negatively affect spouses and partners, family members, friends and coworkers.

What is bipolar disorder?
Bipolar disorder (formerly known as manic-depressive disorder) is a condition in which a person typically experiences "mood swings" from periods of extremely elevated moods ( mania ) to extremely low moods ( depression ). Bipolar disorder usually first appears in young adulthood, but children and adolescents can also suffer from this disorder. Bipolar disorder is a treatable condition. People with bipolar disorder are at risk of death - either by dangerous behavior in the manic state or by suicide in the depressive state. In a life-threatening emergency, call 911. When the situation has stabilized, seek ongoing treatment.

Research findings, clinical experience, and family accounts provide substantial evidence that bipolar disorder , also called manic-depressive illness, can occur in children and adolescents Bipolar disorder is difficult to recognize and diagnose in youth, however, because it does not fit precisely the symptom criteria established for adults, and because its symptoms can resemble or co-occur with those of other common childhood-onset mental disorders. In addition, symptoms of bipolar disorder may be initially mistaken for normal emotions and behaviors of children and adolescents. But unlike normal mood changes, bipolar disorder significantly impairs functioning in school, with peers, and at home with family. Better understanding of the diagnosis and treatment of bipolar disorder in youth is urgently needed. In pursuit of this goal, the National Institute of Mental Health (NIMH) is conducting and supporting research on child and adolescent bipolar disorder
A CAUTIONARY NOTE

Effective treatment depends on appropriate diagnosis of bipolar disorder in children and adolescents . There is some evidence that using antidepressant medication to treat depression in a person who has bipolar disorder may induce manic symptoms if it is taken without a mood stabilizer. In addition, using stimulant medications to treat attention deficit hyperactivity disorder (ADHD) or ADHD-like symptoms in a child with bipolar disorder may worsen manic symptoms. While it can be hard to determine which young patients will become manic, there is a greater likelihood among children and adolescents who have a family history of bipolar disorder. If manic symptoms develop or markedly worsen during antidepressant or stimulant use, a physician should be consulted immediately, and diagnosis and treatment for bipolar disorder should be considered.

Bipolar Disorder "It seems as though my mind has slowed down and burned out to the point of being virtually useless. I [am] haunt[ed] with the total, the desperate hopelessness of it all." "Ideas are fast...all shyness disappears...people, things become intensely interesting...unbelievable feelings of ease, power, well-being...you can do anything." "The fast ideas become too fast and there are far too many...overwhelming confusion replaces clarity...everything is now against the grain...you are irritable, angry, uncontrollable, and trapped."
What is Bipolar Disorder?
These quotations describe bipolar disorder (BD), an illness characterized by extreme changes in mood, behavior and energy levels. Also called manic-depressive illness, BD affects approximately 1.2% of the population . Men and women are equally likely to develop BD. The disorder does tend to run in families and the rate of BD in identical twins is higher than that in fraternal (non-identical twins). Nevertheless, the specific genetic defect has not yet been identified and it is likely that both genetic and environmental factors contribute to BD. The first bipolar episode often occurs in adolescence or early adulthood and may be triggered by a traumatic event such as the death of a family member. In between episodes, many people are symptom-free, while others are never free of symptoms. Once properly diagnosed and treated, people with BD can lead productive lives.

Frequently Asked Questions About
Early-Onset Bipolar Disorder What Is early-onset bipolar disorder and why are we suddenly hearing so much about it? Early-onset bipolar disorder is manic-depression that appears earlyvery earlyin life. For many years it was assumed that children could not suffer the mood swings of mania or depression, but researchers are now reporting that bipolar disorder (or early temperamental features of it) can occur in very young children, and that it is much more common that previously thought. Is bipolar disorder in children the same thing as bipolar disorder in adults? Adults seem to experience abnormally intense moods for weeks or months at a time, but children appear to experience such rapid shifts of mood that they commonly cycle many times within the day. This cycling pattern is called ultra-ultra rapid or ultradian cycling and it is most often associated with low arousal states in the mornings (these children find it almost impossible to get up in the morning) followed by afternoons and evenings of increased energy. It is not uncommon for the first episode of early-onset disorder to be a depressive one. But as clinical investigators have followed the course of the disorder in children, they have reported a significant rate of transition from depression into bipolar mood states.

Understanding Depression
Bipolar disorder with onset during childhood or early adolescence appears to be as common in prevalence as adult onset bipolar disorder and may be a different (possibly more severe) variant of the disorder. It is difficult to recognize and diagnose, because the symptoms and behaviors associated with the disorder manifest themselves differently than adult onset bipolar disorder, and often appear similar to (or can co-occur with) other more common childhood disorders such as attention deficit hyperactivity disorder (ADHD), disruptive behavior disorders, conduct disorders, or even normal "mood swings" of early and middle adolescence. Some controversy surrounds the diagnosis and continued research is needed to improve our understanding and treatment of this mood disorder in children and adolescents. Unlike adult bipolar disorder where mania manifests as elevated moods or euphoria, children are more prone to heightened irritability, destructive outbursts, or highly disruptive and/or aggressive behaviors during manic states. Children may express depressed states through behaviors such physical complaints, irritability, crying spells, significant difficulty with interpersonal relationships, and extreme sensitivity to negative reactions from others. In early adolescents the appearance of the disorder is often characterized by a continuous, rapid-cycling, irritable, and mixed symptom state.

Early Signs of Bipolar Disorder in Children and Adolescents
Incidence of bipolar disorder (BD) is known to be about 1% in adults and adolescents, but is unknown in prepubertal children. While studies suggest that psychiatric symptoms of BD are rare before the age of 14, up to one-third of adults surveyed displayed significant psychiatric symptoms-most commonly depressed mood-before that age (Lish et al., 1994). Younger children and adolescents may have phenomenological differences from older adolescents and adults with BD; this may account for the difficulty in recognizing BD in the younger group. Adolescent mania may present with more mixed states and psychosis than adult mania (Ballenger et al., 1982), and children's manic episodes may be briefer and present with sustained conduct and impulse-control problems. Early disruptive behavior may also predict BD or another mood disorder (Carlson, 1995). While childhood-onset BD is commonly associated with or preceded by conduct disorder (CD), attention-deficit/hyperactivity disorder (ADHD) and/or oppositional defiant disorder (ODD), the question remains whether these are truly comorbid conditions, or whether they are prodromal or concurrent representations of bipolar disorder itself (Chang et al., 2000).

What is bipolar disorder?
Bipolar disorder is characterized by extreme mood swings between mania (elevated mood or irritability) and depression . These mood swings are beyond the normal ups and downs that most people experience—they greatly affect the lives of those who experience them. Doctors used to think that bipolar disorder symptoms did not appear until the teen or early adult years. They now believe even a young child may show symptoms of bipolar disorder, although the symptoms in children are often different from those of adults. Early diagnosis and treatment can lead to more effective management of bipolar disorder and may reduce the risk of long-term problems that can result when bipolar disorder remains undiagnosed or untreated.

Children and Bipolar Disorder
What is Bipolar Disorder? Bipolar disorder (also referred to as manic-depressive illness) usually starts in adult life, before the age of 35. Although rare in young children, it can appear both in children and teenagers. This illness can affect anyone. If one or both parents have bipolar disorder , the chances are greater that their children will develop the disorder. Recognizing The Warning Signs Of Bipolar Disorder Bipolar disorder may begin with manic or depressive symptoms. The manic symptoms include:
  • Severe changes in mood, when compared to peers, either unusually happy or silly, or highly irritable.
    Unrealistic highs in self-esteem.
Great energy increase; can go with little or no sleep for days without tiring.
Increased talking - talks too much, too fast; changes topics too quickly; cannot be interrupted.
Distractibility - attention moves constantly from one thing to the next.
High risk behavior - such as, jumping off a roof and believing no harm will occur to them.
