Child and Adolescent Bipolar Disorder
Can children and adolescents get bipolar disorder? Bipolar disorder can occur in children and adolescents and has been investigated by federally funded teams in children as young as age 6. How common is it in children and adolescents? Although once thought rare, caseloads of patients examined for federally funded studies have shown that approximately 7% of children seen at psychiatric facilities fit bipolar disorder using research standards. What are the symptoms of bipolar disorder in children and adolescents? One of the biggest challenges has been to differentiate children with mania from those with attention deficit hyperactivity disorder. Both groups of children present with irritability, hyperactivity and distractibility. So these symptoms are not useful for the diagnosis of mania because they also occur in ADHD. But, elated mood, grandiose behaviors, flight of ideas, decreased need for sleep and hypersexuality occur primarily in mania and are uncommon in ADHD. Below is a brief description of how to recognize these mania-specific symptoms in children. Elated children may laugh hysterically and act infectiously happy without any reason at home, school or in church. If someone who did not know them saw their behaviors, they would think the child was on his/her way to Disneyland. Parents and teachers often see this as "Jim Carey-like" behaviors.

Browse all health information Child and Adolescent Bipolar Disorder: An Update from the National Institute of Mental Health Research findings, clinical experience, and family accounts provide substantial evidence that bipolar disorder, also called manic-depressive illness, can occur in children and adolescents. Bipolar disorder is difficult to recognize and diagnose in youth, however, because it does not fit precisely the symptom criteria established for adults, and because its symptoms can resemble or co-occur with those of other common childhood-onset mental disorders. In addition, symptoms of bipolar disorder may be initially mistaken for normal emotions and behaviors of children and adolescents. But unlike normal mood changes, bipolar disorder significantly impairs functioning in school, with peers, and at home with family. Better understanding of the diagnosis and treatment of bipolar disorder in youth is urgently needed. In pursuit of this goal, the National Institute of Mental Health (NIMH) is conducting and supporting research on child and adolescent bipolar disorder.

Bipolar Disorder
by William T. Goldman, MD
Introduction: Why Bipolar Disorder is a problem.
Bipolar Disorder in children and adolescents has long been an overlooked health problem, and the subject of much professional debate. While it is fairly well known to the general public that "manic-depressive illness" is fairly common, affecting up to 1.0 to 1.5 % of the adult population at some point in life, it is not commonly recognized that bipolar illness in childhood and adolescence is also a major health problem: adolescents run a similar rate of risk as do adults, and a significant number more also report notable and disturbing subclinical levels of mood problems similar in quality to bipolar disorder. Related Articles Attention Deficit Disorder School Problems Depression Internet Links About Pediatrics ADHD Mental Health Information for Children Although considered rare, the exact rate of bipolar disorder in children remains unknown, although epidemiological studies are underway to determine this. Bipolar disorder in children and adolescents is arguably more significant of an issue than it is in adults. For example, if it is assumed that 1% of children and adolescents at any given time suffer from bipolar disorder, this naturally occurs during critical phases in child development, and not only can show itself with behaviors and feelings not typically viewed as part of a "depression" or "mania" but can interfere with the normal developmental processes of childhood. Add to this the diagnostic difficulties in distinguishing between disorders that have similar characteristics in children (such as delinquency, anxiety disorders, psychosis, ADHD, and learning disorders) and there is a significant number of children and adolescents both correctly diagnosed and misdiagnosed or undiagnosed.

It can occur at any time of life from early childhood onward. www.bpkids.org ), as many as one million or more children under 18 in the U.S. may have bipolar disorder (mostly undiagnosed and untreated). Mania (the activated state) may include insomnia, hyperactivity, daredevil acts, elation and grandiose thinking. Racing thoughts, separation anxiety and intense temper tantrums (also called "rages" or "affective storms") can occur during depression or mania. Sometimes symptoms of both states occur together in mixed states (depressed mood with high energy) or in quick succession within a single day (called rapid cycling).

Hour One: Bipolar Disorder in Children / Nobel Laureate Robert Horvitz
Bipolar disorder once known as manic depression seems to be on the rise in children and teens. Unfortunately, doctors aren't sure what's responsible for the increase. In this hour of Science Friday, guest host Paul Raeburn and guests talk about bipolar disorder in children, why it's difficult to diagnose, and what is available to help treat the condition. (Note: Listeners interested in the NIMH bipolar study can call 866-MAP-NIMH toll-free to reach the general recruitment line for the mood and anxiety program at the NIMH, or 301-496-8381 to reach the child and adolescent bipolar disorder research team directly.) Plus, we'll talk with one of this year's winners of the Nobel Prize in Medicine, Robert Horvitz of MIT. Dr. Horvitz shares the prize with Sydney Brenner and John E. About Early-Onset
Bipolar Disorder Bipolar disorder (also known as manic-depression) is a serious but treatable medical illness. It is a disorder of the brain marked by extreme changes in mood, energy, thinking and behavior. Symptoms may be present since infancy or early childhood, or may suddenly emerge in adolescence or adulthood. Until recently, a diagnosis of the disorder was rarely made in childhood. Doctors can now recognize and treat bipolar disorder in young children. Early intervention and treatment offer the best chance for children with emerging bipolar disorder to achieve stability, gain the best possible level of wellness, and grow up to enjoy their gifts and build upon their strengths. Proper treatment can minimize the adverse effects of the illness on their lives and the lives of those who love them. Families of affected children and adolescents are almost always baffled by early-onset bipolar disorder and are desperate for information and support. In this section of the CABF web site, you will find answers to some of the most common questions asked about the disorder. How common is bipolar disorder in children?

Child and Adolescent Bipolar Disorder:
A Review of the Past 10 Years
Barbara Geller, M.D. and Joan Luby, M.D. J Am Acad Child Adoles Psychiatry 36:1168-1176, 1997 Abstract Objective: To provide a review of the epidemiology, phenomenology, natural course, comorbidity, neurobiology, and treatment of child and adolescent bipolar disorder (BP) for the past 10 years. This review is provided to prepare applicants for recertification by the American Board of Psychiatry and Neurology. Method: Literature from Medline and other searches for the past 10 years, earlier relevant articles, and the authors' experience and ongoing National Institute of Mental Health-funded project "Phenomenology and Course of Pediatric Bipolarity" were used. Results: Age-specific, developmental (child, adolescent, and adult) DSM-IV criteria manifestations; comorbidity and differential diagnoses; and episode and course features are provided. Included are age-specific examples of childhood grandiosity, hypersexuality, and delusions. Differential diagnoses (e.g. specific language disorders, sexual abuse, conduct disorder [CD], schizophrenia, substance abuse), suicidality, and BP-II are discussed. Conclusion: Available data strongly suggest that prepubertal onset BP is a nonepisodic, chronic, rapid cycling, mixed manic state that may be comorbid with attention-deficit hyperactivity disorder (ADHD) and CD or have features of ADHD and/or CD as initial manifestations. Systematic research on pediatric BP is in its infancy and will require ongoing and future studies to provide developmentally relevant diagnostic methods and treatment.

Weight Loss Judith Waite Allee , author of "Homeschooling on a Shoestring", says I'm no expert, but I've picked up a few hints from working at a Mental Health Association and from dealing with family members. I just want to point out that bi-polar disorder (also known as manic-depression) can be overlooked in children and may be misdiagnosed as ADHD , depression, and/or Oppositional Defiant Disorder (ODD) . Previously the psychiatric community believed that bi-polar had a later onset. That theory is changing, but slowly. Many (maybe most) of the adults diagnosed with bi-polar seem to go through multiple diagnoses before finding help, partly because bi-polar is difficult to diagnose, and partly because doctors seem to be leery about "labeling" a child with bi-polar. Part of the confusion is that kids with bipolar may have very different symptoms from adults. For example, they may cycle frequently in the same day.
