Notice to Former Workers About Beryllium Disease Testing If you were an Ames Laboratory employee prior to the mid-1950s, you should be receiving a letter from the Department of Energy inviting you to have a blood test to determine if you are sensitized to beryllium. Beryllium was known to be used in Gilman Hall and in Wilhelm Hall on the Iowa State University campus as part of work conducted in support of the Manhattan Project and as post-war work to improve the purification and processing of uranium and thorium. Some people who may have been exposed to beryllium or beryllium compounds during that period may develop chronic beryllium disease, or CBD. People are exposed to beryllium through inhalation of beryllium mists, dusts or fumes. The disease affects the lungs, but can be treated. If you were a non-Ames Laboratory ISU employee working in Gilman Hall during this same time period, you also should be receiving a letter from the DOE. In addition, if you know of any former Ames Lab or ISU employees who may have been exposed to beryllium, please pass this information along to them. Beryllium is a rare element that is extracted from the earth, refined and reduced to a very fine powder. Its properties of light weight, high tensile strength and ability to slow neutrons have made it useful in many industries. Common workplace forms of beryllium are beryllium oxide powder, beryllium ceramics and beryllium copper alloy. Because of the health dangers associated with beryllium exposure, the Atomic Energy Commission adopted limits for workplace beryllium exposure. These limits are extraordinarily low. They reflect the fact that there is no known safe level of beryllium exposure; even minimal exposures to the substance may cause chronic beryllium disease (also known as CBD or berylliosis) in susceptible individuals. Family members of individuals who work with beryllium can also develop chronic beryllium disease from exposure to dust that the worker brings home on his or her clothing.

WHAT IS BERYLLIUM?
Beryllium is a brittle, steel gray metal found as a component of coal, oil, certain rock minerals, volcanic dust and soil. Elemental beryllium is used in a wide variety of applications. Exposure to beryllium most often occurs in mining, extraction, and in the processing of alloy metals containing beryllium. The metal is used in the aerospace, nuclear and manufacturing industries.
WHAT IS BERYLLIUM DISEASE?
Beryllium disease is caused by inhaling beryllium particles, dust or fumes. Symptoms include coughing, shortness of breath, fatigue, weight loss or loss of appetite, fever and sweating. Medical tests may reveal abnormal lung sounds, lung scars, decreased pulmonary function, granulomas (a nodular form of chronic inflammation) and an allergy to beryllium.
The chronic form of the disease, Chronic Beryllium Disease (“CBD”) is caused by an allergic reaction to beryllium. Depending on the severity of CBD, symptoms of pulmonary hypertension or respiratory failure may be present. CBD is incurable, usually irreversible and may result in death. The acute form of the disease, involving skin disease with poor wound healing and rash or wart-like bumps, is rarely seen and usually not nearly as serious as CBD.

The Occupational Safety and Health Administration (OSHA) has recently obtained information suggesting that OSHA's current 2 micrograms per cubic meter of air (micrograms/m ) eight-hour time-weighted average (TWA) permissible exposure limit (PEL) for beryllium in the workplace may not be adequate to prevent the occurrence of chronic beryllium disease (CBD), a disabling and often fatal lung disease, among exposed workers.
OSHA is publishing this Hazard Information Bulletin to alert employees working with beryllium about the hazards associated with their work. It describes engineering controls, work practices, and personal protective equipment recommended for controlling exposures to beryllium through inhalation and skin contact. It also suggests health surveillance methods to identify workers who may have become sensitized to beryllium, or who may have CBD.

ABSTRACT: Chronic beryllium disease is an increasingly common granulomatous disorder often mistaken for sarcoidosis. Patients may present with a variety of respiratory and systemic symptoms, such as nonproductive cough, fatigue, dyspnea, weight loss, and fever. The earliest clinical signs are scattered bibasilar crackles and wheezing. Ulceration or subcutaneous tender nodules may be seen if beryllium penetrates the skin. There is no characteristic pattern of pulmonary function abnormalities, and the radiologic presentation is similar to that of sarcoidosis. A high degree of clinical suspicion must be maintained when evaluating patients with a history of direct or indirect exposure to beryllium or metal alloys. Diagnosis relies on evidence of lung inflammation and beryllium sensitization. Corticosteroid treatment is usually given to symptomatic patients who demonstrate a decline in lung function. U.S. military personnel have been exposed to the highly toxic metal beryllium at dozens of Army, Air Force, Navy and Marine Corps facilities, with some levels exceeding legal safety limits, a Tribune investigation has found.
Despite the serious risks, the Department of Defense has ignored federal health guidelines by failing to provide simple blood tests to determine if workers have been harmed.
The Defense Department's inaction is in sharp contrast to steps taken by the Department of Energy, which has tested thousands of its weapons workers and discovered that hundreds of people have been harmed by beryllium, a lightweight metal whose dust can cause an often fatal lung disease.
The screening is highly recommended by federal health agencies, including the Occupational Safety and Health Administration and the National Institute for Occupational Safety and Health, as well as by independent scientists and physicians. Early detection is important because it allows treatments that can attempt to limit lung damage.
Beryllium disease has been found in virtually every industry in which the metal has been used. Experts said that if the Defense Department were to provide the blood tests to its servicemen and women and civilian employees, many illnesses would be found.

Chronic Beryllium Disease
Beryllium, a chemical used in many industries, is a toxic substance that can be harmful, causing serious injury and even death. Beryllium dust and fumes are classified as toxic air pollutants by the Environmental Protection Agency (EPA). It is estimated that 2-6% of workers exposed to these contaminants eventually develop beryllium disease, scarring of the lung that leads to lung cancer.
