Beriberi
From Wikipedia, the free encyclopedia.
Beriberi is an ailment caused by a deficiency of vitamin thiamine ), the symptoms of which may include weight loss, emotional disturbances, impaired sensory perception, weakness, and periods of irregular heartbeat. Beriberi occurs in people whose staple diet consists mainly of polished white rice, which has little or no thiamine. Therefore the disease has been seen traditionally in people in Asian countries and in chronic alcoholics with impaired liver function. If a baby is fed the milk of a mother who suffers from a deficiency in thiamine, the child may develop beriberi. There are two forms of the disease: wet beriberi and dry beriberi. Wet beriberi affects the heart; it is sometimes fatal, as it causes a combination of heart failure and weakening of the capillary walls, which causes the peripheral tissues to become waterlogged. Dry beriberi causes wasting and partial paralysis resulting from damage to the peripheral nerves. The first stage in discovering the cause of beriberi was in the 1890s, when a Dutch doctor, Christiaan Eijkman , found that fowls fed only on polished rice developed similar symptoms to his patients who had beriberi, and that they could be cured if they were also fed some of the husks from the rice grains. Diagnosing Beriberi in Emergency Situations

This piece will be most useful to medical professionals - doctors and nurses. However, the article may also be useful for non specialists who need to communicate with medical personal about this deficiency disease. In some places medical terms have been simply explained for the non-specialist.
Recently the NGO community in West Africa learned of an outbreak of beriberi in an area with severe nutritional deprivation. The diagnosis had been based upon the presence of epidemic oedema found during a nutritional survey. The survey team included experienced, doctors. The prevalence of beriberi was estimated at 30%. I accompanied an ACF team working in the area to examine these patients. The only clinical feature that these patients had that resembled beriberi was the oedema. The patients had 'famine oedema' which is the same as adult kwashiorkor. In the absence of diagnostic laboratories, the diagnosis of nutritional deficiency diseases depends upon:
  • a high index of suspicion;

  • Beriberi is a disease caused by a deficiency of the B vitamins, particularly vitamin B (thiamine). This disease occurs mainly in the Far East, where the diet consists principally of polished rice, which does not supply sufficient thiamine. Cases of beriberi that occur in the United States are usually associated with alcoholism, hypothyroidism, infections, pregnancy, and/or stress. SYMPTOMS Symptoms of beriberi in children can include:
    • Impaired muscle growth.
    • Mental confusion.
    • Convulsions.
    • Gastrointestinal problems.
    • Nausea.
    • Vomiting.
    • Constipation and diarrhea.
In adults, the symptoms also include:
    • Mental confusion and speech difficulties.
    • Wernicke's encephalopathy. If a person with Wernicke's encephalopathy receives thiamine replacement, language problems, unusual eye movements and walking difficulties may subside, but may be replaced by Korsakoff's syndrome, which includes retrograde amnesia (memory loss), impaired ability to learn, and confabulation (making up stories to explain behavior that have little relation to reality).
    • Strange eye movements (nystagmus).
  • 29. Beriberi

    beriberi,
also called VITAMIN B DEFICIENCY, nutritional disorder caused by a deficiency of vitamin B (thiamine) and characterized by impairment of the nerves and heart. General symptoms include loss of appetite and overall lassitude, digestive irregularities, and a feeling of numbness and weakness in the limbs and extremities. (The term beriberi is derived from the Sinhalese word meaning "extreme weakness.") In the form known as dry beriberi, there is a gradual degeneration of the long nerves, first of the legs and then of the arms, with associated atrophy of muscle and loss of reflexes. In wet beriberi, a more acute form, there is edema (overabundance of fluid in the tissues) resulting largely from cardiac failure and poor circulation. In infants breast-fed by mothers who are deficient in thiamine, beriberi may lead to rapidly progressing heart failure. The cardiac symptoms, in both infants and adults, generally respond promptly and dramatically to the administration of thiamine. When neurological involvement is present, response to thiamine therapy is much more gradual; in more severe cases, the structural lesions of the nerve cells may be irreversible. Thiamine normally plays an essential role as a coenzyme in the metabolism of carbohydrates; in its absence, pyruvic acid and lactic acid (products of carbohydrate digestion) accumulate in the tissues, where they are believed to be responsible for most of the neurological and cardiac manifestations.

    Overview Symptoms Treatment Prevention Definition:
Beriberi is a vitamin deficiency disease, caused by inadequate bodily stores of thiamine , (Vitamin B1). It can damage the heart and nervous system.
Alternative Names: Thiamine deficiency; Vitamin B1 deficiency
Causes, incidence, and risk factors: There are two major manifestations of thiamine deficiency: cardiovascular disease (wet beriberi) and nervous system disease ("dry beriberi" and Wernicke-Korsakoff syndrome). Dry beriberi is somewhat of a misnomer because both types are most often caused by excessive alcohol consumption. Symptoms of dry beriberi include pain, tingling, or loss of sensation in hands and feet (peripheral neuropathy), muscle wasting with loss of function or paralysis of the lower extremities, and potentially brain damage and death. Wet beriberi is characterized by swelling (edema), increased heart rate (tachycardia), lung congestion, and enlarged heart related to congestive heart failure. Beriberi has become very rare in the United States because most foods are now vitamin-enriched, which means that a normal diet contains adequate amounts of

In Eijkman's time, Indonesia was a Dutch colony and Jakarta was called Batavia. "White rice can be poisonous!" This was the conclusion Christiaan Eijkman declared in 1896 on his return to the Netherlands, after ten years of research in Batavia, Java in the Dutch East Indies (now Jakarta, Indonesia). Christiaan Eijkman was born on August 11, 1858 in the Dutch town of Nijkerk. He was the seventh child of the headmaster at the local school. Robert Koch was Eijkman's teacher in Berlin in 1885. Twenty years later, Koch was to receive the Nobel Prize in Physiology or Medicine for his research on tuberculosis. Who Was Eijkman and Why Did He Travel to Java? Early on Christiaan Eijkman had decided he wanted to become a doctor. When the time came, his family couldn't afford to send him to medical school. Nevertheless, he found the means to get the education he needed. The Dutch colonial army had use for many doctors and they paid for his education in exchange for a few years of medical work in the Dutch East Indies. Christiaan got his medical training, and then went to the colonies to work as an army surgeon. A short time after his arrival he got malaria, and after two years, it became impossible for him to work. He was sent back home to recover.

      Eijkman's Early Career
The seventh of ten children, Christiaan Eijkman was born on 11 August 1858 in the rural village of Nijkerk, in the eastern-central part of the Netherlands. His father, a schoolmaster, ran a boarding school at the time, but soon moved the family closer to Amsterdam. Early on, Eijkman proved to be an exceptionally bright youngster who showed a desire to study medicine. However, because funds were limited, he signed up with the colonial army as a way of obtaining his medical training, negotiating an agreement with the Department of War to pay the bill for his training in exchange for his services in the army in the Dutch East Indies. After finishing his medical studies, he also obtained a doctoral degree in physiology; the title of his thesis was "Polarization in the Nervous System" (Over Polarisatie in de Zenuwen).
