Benign prostatic hyperplasia, previously called benign prostatic hypertrophy is the term to describe enlargement of the prostate gland.
The enzyme that converts testosterone to DHT is concentrated in the prostate, scrotal skin, testicles and even the scalp (hence the connection with male hair loss). DHT levels increase within prostate cells as a result of increased activity of this enzyme, as well as a greater uptake of testosterone and a lower rate of breakdown and excretion of DHT.
This leads to prostate enlargement, which results in contriction of the urethra, which carries urine to the outside, and the development of BPH symptoms including weak urine streams, dribbling, progressive frequency, urgency and intermittency of urination.
Men with benign prostatic hyperplasia often get up 3-5 times per night to urinate. In 2-3% of benign prostatic hyperplasia (bph), urinary incontinence results from instability of the detrusor muscle, the outer urinary bladder muscle.
BPH Treatment
Surgery is the most recommended treatment for BPH. There are 3 different procedures, but "transurethral resection" is performed on 95%. Complications include incontinence and retrograde ejaculation. The drug used to treat BPH is Proscar which inhibits the breakdown of testosterone. Proscar's side effects are impotence and decreased libido. Annual sales however top 1 billion dollars.

About Benign Prostatic Hyperplasia What is benign prostatic hyperplasia (BPH)? Benign prostatic hyperplasia (BPH) is one of the most common diseases affecting the prostate gland. BPH occurs when there is an increase in the number of the smaller glands and stroma of the prostate gland, which causes the prostate to become enlarged. BPH is a type of tumour that is benign or non-cancerous. It is the most common benign tumour in men, and it becomes much more common as men get older. One study has shown that BPH is present in 20% of men aged 41-50 years, 50% of men aged 51-60 years, and over 90% of men older than 80 years , although the proportion of men with symptoms was much less. BPH is not a life-threatening condition, but if left untreated for a prolonged period of time, it has the potential to cause problems with the function of the bladder, and less commonly, the kidneys.

DEFINITION:
Benign prostatic hyperplasia (BPH) is a very common condition in which the prostate gland enlarges and causes bothersome urinary symptoms in middle-aged and older men. While such symptoms as increased frequency of urination, decreased force of urinary stream, urgency, nighttime urination, and loss of control of urination (incontinence) may begin in some men as early as 40 years of age, most men do not begin to experience symptoms until their 50s and 60s. By the time a man reaches the age of 80, it is highly likely that he will have at least some degree of bother from prostatic enlargement. TREATMENTS For those patients with moderate symptoms that are bothersome despite medical therapy and who would like to avoid surgery, there are several new minimally-invasive options available. These treatments include TUNA (transurethral needle ablation), microwave thermotherapy, and laser ablation, with several other treatments undergoing clinical trials at the present time.

What is benign prostatic hyperplasia (BPH)? prostate gland that occurs in almost all men as they age. The enlargement is usually harmless, but it often results in problems urinating (known as prostatism or lower urinary tract symptoms [LUTS]). About half of all men over 50 experience some symptoms. What causes BPH? It is not clear that BPH is a disease. It is probably a normal part of the aging process in men, caused by changes in hormone balance and cell-growth factors. Why some men have more severe LUTS because of BPH than others is unknown. How will BPH affect me? For patients with symptoms that come and go, conservative monitoring may be useful. Up to one-third of patients suffering from BPH will see some short-term improvement in symptoms. In 1994, AHCPR released a clinical practice guideline to identify the most effective methods of diagnosing BPH. A multidisciplinary, 13-member private-sector panel based the guideline on syntheses of structured reviews of the literature on BPH. Since the prostate gland surrounds the urethra, the tube urine passes through, it can prevent the bladder from emptying completely. Half of all men have BPH by the time they reach age 60. The main symptom of BPH is mild to severe trouble urinating. BPH may also cause waking at night to urinate, dribbling or leaking of urine, or a weak urine stream. BPH isn’t related to prostate cancer, but symptoms can be similar. Treatment includes herbal remedies, prescription medications, and surgery. If your symptoms are mild, you may not need any treatment.

Depending on how severe your symptoms are, your doctor may recommend surgery or medication. Surgery offers the most immediate and long-lasting effect. Five years after surgery, 90 percent of men continue to report relief from symptoms. Surgery does carry the risks of infection and nerve damage. About two percent of men who undergo surgery experience nerve damage and impotence. Antiandrogens and alpha-blockers are two groups of prescription drugs that can reduce the symptoms of BPH.
