Benign prostatic hyperplasia (BPH) is the abnormal growth of benign prostate cells. In BPH, the prostate grows larger and presses against the urethra and bladder , interfering with the normal flow of urine More than half of the men in the United States between the ages of 60 and 70 and as many as 90 percent between the ages of 70 and 90 have symptoms of BPH. What is BPH? Benign prostatic hyperplasia (BPH) is a condition that often begins at about 40-45 years, due to hormonal changes that result in prostatic enlargment . The word “benign” means that the enlargement is not caused by cancer or infection. The word “hyperplasia” means enlargement. What is the prostate gland ? It is a walnut sized gland that is part of the male reproductive system. Where is the prostate gland located ? It is located below the bladder (the organ where urine is stored) and surrounds the upper portion of the urethra (the tube that carries urine out of the body) What is the function of the prostate gland ? It produces part of the semen in which sperm travel. What are the reasons for this enlargement ? The reasons are not entirely clear but ageing and hormonal changes appear to play an important role.

This enlargement constricts the urethra so the flow of urine is reduced, making it increasingly difficult to empty the bladder.
    The prostate gland
    The prostate is a gland about the size of a walnut that is only present in men. It is located just below the bladder and surrounds the urethra, the tube through which urine flows from the bladder and out through the penis. Please see the diagram, overleaf. One of the main functions of the prostate is to produce an important liquefying component of semen, which allows the sperm to move freely. The gland is divided into three zones, peripheral, transitional and central. With BPH, it is the central part where overgrowth of cells takes place.

Fortunately, such complications are rare. Transurethral resection of the prostate (TURP) has traditionally been the "gold standard" of treatment for men with BPH. TURP represented 94% of all surgical procedures for BPH in 1996. Currently, however, TURP represents only one of a variety of treatment options available to men with BPH and/or LUTS. Alternative treatment options have emerged, including medical therapy with hydroureteronephrosis , ureteric tortuosity, bladder trabeculation, diverticula, elevation of the bladder base and an increased post-void residual urinary volume. Lower urinary tract changes such as trabeculation or diverticulum formation are best documented by cystoscopy. Ultrasound is preferred over intravenous urography (IVU) when determining postvoid residual volume. Determination of prostate size using IVU is inaccurate, while digital rectal examination, transrectal ultrasound and magnetic resonance imaging (MRI) provide more accurate estimates of prostatic volume. Traditionally, digital rectal examination (DRE) has been used to estimate prostate size prior to treatment. However, only the posterior surface of the prostate is accessible for examination. Due to the shape of the prostate as well as its location within the pelvis, an examiner is unable to ascertain the anterior  posterior extent of the gland by palpation. Thus, DRE consistently underestimates the size of the prostate when compared to local imaging modalities such as transrectal ultrasound (TRUS) and MRI.

In this Special Report, we interview Hopkins BPH expert, Dr. H. Logan Holtgrewe, who explains the differences between the terms BPH and LUTS, or lower urinary tract symptoms. This distinction is not merely semantic, but influences the selection of appropriate treatment.
