Prostate growth involves hormones, occurs in different types of tissue (e.g., muscular, glandular), and affects men differently. As a result of these differences, treatment varies in each case. There is no cure for BPH and once prostate growth starts, it often continues, unless medical therapy is started. The prostate grows in two different ways. In one type of growth, cells multiply around the urethra and squeeze it, much like you can squeeze a straw. The second type of growth is middle-lobe prostate growth in which cells grow into the urethra and the bladder outlet area. This type of growth typically requires surgery.

What is benign prostatic hyperplasia?
Benign prostatic hyperplasia (BPH) is a condition that affects the prostate gland in men. The prostate is a gland found between the bladder (where urine is stored) and the urethra (the tube urine passes through). As men age, the prostate gland slowly grows bigger (or enlarges). As the prostate gets bigger, it may press on the urethra and cause the flow of urine to be slower and less forceful. "Benign" means the enlargement isn't caused by cancer or infection. "Hyperplasia" means enlargement. Return to top
What are the symptoms of BPH?
Most symptoms of BPH start gradually. One symptom is the need to get up more often at night to urinate. Another symptom is the need to empty the bladder often during the day. Other symptoms include difficulty in starting the urine flow and dribbling after urination ends. The size and strength of the urine stream may decrease. These symptoms can be caused by other things besides BPH. They may be signs of more serious diseases, such as a bladder infection or bladder cancer. Tell your doctor if you have any of these symptoms, so he or she can decide which tests to use to find the possible cause.

Benign Prostatic Hyperplasia (BPH) Commonly, men over the age of 50 will suffer from benign prostatic hyperplasia (BPH). This is the benign enlargement of the prostate gland that can make urination painful and difficult. Men with benign prostatic hypertrophy can have difficulties with the starting of a urinary stream, or problems with an intermittent stream. More than fifty percent of men over the age of 60 have benign prostatic hypertrophy and approximately twenty to twenty-five percent of those require treatment. Newer medical treatment may help the condition, but often surgery is performed to remove excess tissue inside the prostate. Using a special tool called a cystoscope, the physician is able to visualize the prostate and remove the tissue without having to make an incision. Many men confuse benign prostatic hyperplasia with prostate cancer believing it is the beginning stages of prostate cancer. However this is not true and the diseases are very different. As men grow older, from about age 30, the inner portions of the prostate gland slowly enlarge which ultimately by the age of fifty to sixty results in benign prostatic hypertrophy. Prostatic cancer tends not to effect the inner portion but the outer portion of the gland. Both diseases are hard to distinguish from each other since they share some of the same symptoms and sometimes they can both exist at the same time. Benign adenomatous hyperplasia of the periurethral prostate gland, causing variable degrees of bladder outlet obstruction.
Etiology and Pathophysiology
The etiology is unknown but may involve hormonal changes associated with aging. Multiple fibroadenomatous nodules occur in the periurethral region of the prostate gland, probably originating within the periurethral glands rather than in the true fibromuscular prostate (surgical capsule), which is displaced peripherally by progressive growth of the nodules. The hyperplasia may involve the lateral walls of the prostate (lateral lobe hyperplasia) or tissue at the inferior margin of the vesical neck (middle lobe hyperplasia). Histologically, the tissue is glandular, with varying proportions of fibrous stroma interposed. As the lumen of the prostatic urethra becomes compromised, urine outflow is progressively obstructed, accompanied by hypertrophy of the bladder detrusor, trabeculation, cellule formation, and diverticula. Incomplete bladder emptying causes stasis and predisposes to infection with secondary inflammatory changes in the bladder (chronic prostatitis, see below) and the upper urinary tract.
