Fibrocystic Breasts
  • Fibrocystic breasts is a condition that affects nearly 50% of American women. The symptoms are lumpy, painful breasts. Women experience these symptoms to varying degrees and each women will have variable symptoms during her lifetime.
There has been concern about whether any of the fibrocystic changes are premalignant or increase your risk of breast cancer. There are some specific changes which do indicate an increased risk for cancer development, but most of the changes are not associated with a significant risk. Pathologists also use the term fibrocystic changes to describe several microscopic findings found on breast biopsy specimens. These include:
  • Cysts Sclerosing Adenosis Hyperplasia

  • CYSTS
  • Cysts are spherical fluid filled masses. They may not cause any symptoms or they may present as painful lumps. Some cysts can be detected on physical examinations, whereas other cysts may only be seen on mammography or ultrasound. Approximately 7% of women will develop a cyst at some time during her life. Most commonly it is between the ages of 35-50.

These range from normal changes in your body to abnormal breast disease. Breast lumps may be either benign (noncancerous) or malignant (cancerous). See also: Causes, incidence, and risk factors Return to top Some lumps are age dependent. Both male and female newborn infants have lumps of enlarged breast tissue beneath the nipple, which have been stimulated by maternal hormones. These disappear within a few months of birth. Beginning as early as age 8, girls may develop tender lumps beneath one or both nipples (frequently only one). These lumps are breast buds and are one of the earlier signs of the beginning of puberty.

Printed April 1993 If you discover a lump in one breast, either during breast self- examination or by chance, examine the other breast. If both breasts feel the same, the lumpiness is probably normal. You should, however, mention it to your doctor at your next visit. But if the lump is something new or unusual and does not go away after your menstrual period, it is time to call your doctor. The same is true if you discover a discharge from the nipple or skin changes such as dimpling or puckering . If you do not have a doctor of your own, your local medical society may be able to help you find a doctor in your area. You should not let fear delay you. It's natural to be concerned if you find a lump in your breast. But it's important to remember that four-fifths of all biopsied breast lumps are benign, which means no cancer is present. The sooner any problem is diagnosed, the sooner you can take care of it.
    Clinical Evaluation
No matter how your breast lump was discovered, the doctor will want to begin with your "history": What symptoms do you have and how long have you had them? What is your age, menstrual status, general health? Are you pregnant? Are you taking any medications? How many children do you have? Do you have any relatives with benign breast conditions or breast cancer? Have you previously been diagnosed with benign breast changes?

Cancer Reference Information All About Cancer Other Information Sources Glossary ... Treatment Decision Tools Benign Breast Conditions The breast has two main types of tissues glandular tissues and supporting (stromal) tissues . The glandular part of the breast includes the lobules and ducts. In women who are breast feeding, the cells of the lobules produce milk, which is then carried inside the ducts (milk passages) to the nipple. The support tissue of the breast includes fatty tissue and fibrous connective tissue (ligaments that support the breast). Any of these areas of the breast can undergo changes that cause you to have symptoms. The two main groups of breast changes are benign (not cancer) breast conditions and breast cancers. This document reviews the types of benign breast conditions. The most common benign breast conditions are fibrocystic change, benign breast tumors and breast inflammation

You should speak to your physician or make an appointment to be seen if you have questions or concerns about this information or your medical condition. What is benign breast disease? Most breast problems, including cysts, especially in younger women, are benign (not cancer). The most common breast problem is a benign condition called fibrocystic changes. Fibrocystic changes (which may include large cysts) nodular fibroglandular tissue, and fibroadenomas (solid marble-like lumps) are all examples of benign breast disease. These lumps are not caused by cancer. However, examination of any discrete lump by your primary care physician is important to help you determine if further evaluation is needed. What are the symptoms? Symptoms of fibrocystic change include pain and tenderness, usually in both breasts. However, some women with fibrocystic changes experience no pain but notice lumps or nodules within the breast. In some women, one or both breasts may develop lumps and become most tender 7 - 14 days before the start of each menstrual period. Breast pain occurs most often in the upper, outer part of the breast.

    Benign breast tumours
Most lumps discovered in the breast are benign. Even for lumps where a biopsy is considered appropriate, studies suggest that over 80 percent are not cancerous. Although this is a reassuring thought, it is vital to report any changes in your breast to your doctor as soon as possible, so these changes can be checked. Several tests are used to distinguish these conditions from each other and from breast cancer. Often a clinical breast exam and ultrasound are sufficient for diagnosis. For other conditions, a biopsy may be appropriate to ensure that cancer or hyperplasia is not present. There are several common types of benign breast disease.
    Fibrocystic Changes
As they age, some women feel multiple lumps in their breasts. Sometimes called benign breast disease, the connective tissue in these women's breasts becomes ropy or fibrous. This is more obvious with age, as the milk-producing glands begin to be displaced by softer, fatty tissue. It may also be more apparent during pregnancy. Lumps are usually felt around the nipple, the areola and in the upper, outer part of the breast. Unless a woman is taking hormone replacement therapy, this lumpiness usually dissipates after menopause. C ysts and fibroademonas are both examples of fibrocystic changes: Cysts
Cysts are fluid-filled sacs like blisters. They can be easily identified by ultrasound because they are fluid filled. They occur most often in women 35-50 and may swell in the weeks before a woman's menstrual period. If they are painful and require treatment, they can usually be drained with a

Although any lump formed by body cells may be referred to technically as a tumor. Not all tumors are malignant (cancerous). Most breast lumps – 80% of those biopsied – are benign (non-cancerous). Following are examples of the most common benign breast conditions which produce lumps.
Fibrocystic changes: This is not a disease, but rather a benign (not cancer) condition affecting 50 to 60 percent of all women. Fibrous breast tissue, mammary glands, and ducts overreact to the normal hormones produced during ovulation, resulting in the development of fibrous lumps and/or numerous, small multiple cysts, (lumpy, fluid-filled sacs, or "pockets"). Fibrocystic changes are an exaggerated response of breast tissue to changes of ovarian hormones.
Fibrocystic changes are the most common non-cancerous breast condition. They are most common in women between the ages of 20 and 50. They are unusual after menopause unless a woman is taking hormones.
The size and tenderness of Fibrocystic lumps usually increase before menstruation, decreasing after the period ends. This condition, also known as cystic mastitis, generally disappears after menopause. Medical opinion is still divided over whether Fibrocystic disease increases the risk of breast cancer.

Adapted from the National Institutes of Health by HealthGate Editorial Staff A woman practicing BSE can encounter a broad variety of benign breast conditions. These include normal changes that occur during the menstrual cycle as well as several types of benign lumps. What they have in common is that they are not cancer. Even among breast lumps that warrant a biopsy, some 80 percent prove to be benign. Each breast has 15 to 20 sections, called lobes, each with many smaller lobules. The lobules end in dozens of tiny bulbs that can produce milk. The lobes, lobules, and bulbs are all linked by thin tubes called ducts. These ducts lead to the nipple in the center of a dark area of skin called the areola. Fat fills the spaces between lobules and ducts. There are no muscles in the breasts, but muscles lie under each breast and cover the ribs. These normal features can sometimes make the breasts feel lumpy, especially in women who are thin or who have small breasts.

Most breast lumps are benign, as in fibroadenoma, a condition that affects mostly women under age 30. Fibrocystic breast disease is present in over 60% of all women. The cysts in FBD change in size with the menstrual cycle, whereas a lump from fibroadenoma does not. While most breast lumps are benign it is important to identify those that are not. If a lump is new, persistent, growing, hard, immobile and/or causing skin deformities, it should be evaluated by a health care professional.

What Are the Most Common Types of Benign Lumps?
The most common benign lumps are fibrocystic changes, breast cysts, and fibroadenomas. Most women have a certain degree of fibrocystic changes. These are often described as benign tiny fluid filled sacs that may feel like lumps. They may be hard or rubbery and often fluctuate with the menstrual cycle. A woman can also have a single breast lump that may be large or small. Again this is a fluid filled sac that may fluctuate with the menstrual cycle. A fibroadenomas is another benign lump and is the most common tumor found in the female breast. These most often occur in women who are in their reproductive years. Can Men Have Breast Lumps?
Yes, Men can have tender breast enlargement, often with a lump beneath the nipple. Sometimes this is in one breast, often in both. This benign finding is called gynecomastia. Gynecomastia can also occur after certain types of medications are prescribed.
