What are anxiety disorders? Children and adolescents with anxiety disorders typically experience intense fear, worry, or uneasiness that can last for long periods of time and significantly affect their lives. If not treated early, anxiety disorders can lead to:
  • Repeated school absences or an inability to finish school;
  • Impaired relations with peers;
  • Low self-esteem;
  • Alcohol or other drug use;
  • Problems adjusting to work situations; and
  • Anxiety disorder in adulthood.

We have divided this chapter into two major sections: the first on vestibular disorders, and the second on auditory disorders. Each section reviews the essential aspects of the history and the office examination. Because vestibular symptoms are often accompanied by auditory complaints, neurologists who choose to deal with dizzy patients should be familiar with both vestibular and auditory testing. Therefore, we include a brief review of pertinent tests for vestibular and auditory disorders. Finally, we introduce therapeutic strategies for patients with dizziness and hearing loss.

I am an expert in MALINGERING as well (in which people do the same thing, but for external gain such as narcotics). Factitious disorders are well-recognized among psychiatrists, but they have not received the attentionor advocacy among consumers, families, and professionalsthat have greeted more common ailments such as depression. However, factitious disorders can be every bit as disabling and further public and professional education are vital. People with factitious disorders feign, exaggerate, or actually self-induce illnesses. Their aim? To assume the status of "patient," and thereby to win attention, nurturance, and lenience that they feel unable to obtain in any other way. Unlike individuals who "malinger," people with factitious disorders are not primarily seeking external gains such as disability payments or narcotic drugsthough they may receive them nonetheless.

Apnea, sleep apnea or obstructive sleep apnea are defined as " . . . the cessation of breathing for 10 or more seconds while asleep . . . " Obstructive Sleep Apnea
(showing a closed upper airway) Obstructive sleep apnea (obstructive sleep apnea) is cause by the closing of the upper airway while asleep. The uvula and soft pallet collapses on the back wall of the upper airway. Then the tongue falls backward, collapsing on the back wall of the upper airway, the uvula and soft pallet forming a tight blockage, preventing any air from entering the lungs. The effort of the diaphragm, the chest and the abdomen only cause the blockage to seal tighter. In order to breathe the person must arouse or awaken, causing tension in the tongue thereby opening the airway, allowing air to pass into the lungs.

Obstructive sleep apnea causes a drop in one's blood oxygen saturation (SaO2) and an increase in the blood's carbon dioxide (CO2). When the SaO2 drops the heart will start pumping more blood with each beat. If the SaO2 continues to drop the heart will start beating faster and faster. As the CO2 increases the brain will try to drive the person to breathe. The effort and action of the abdomen and chest will increase. Eventually that action can become severe enough to cause an arousal, clearing the upper airway blockage, allowing the person to breathe. Then you go back to sleep and it happens all over again.
