Cumulative Trauma Disorders Cumulative trauma, or repetitive motion, disorders are disorders of the musculoskeletal and nervous systems which may be caused or aggravated by reptitive motions, forceful exertions, vibration, mechanical compression (hard and sharp edges), sustained or awkward postures, or by exposure to noise over extended periods of time. CTDs can affect nearly all tissues - the nerves, tendons, tendon sheaths, and muscles, with the uppoer extrmeities being the most frequently affected. These painful and sometimes crippling injuries develop gradually over periods of weeks, months, and years, and result from repeated actions such as twisting and bending the hands, arms, and wrists. A common risk factor among these disorders is the use of force combined with repetitive motion over time. The most common occupational diseases associated with CTDs are tendon disorders such as tendonitis, tenosynovitis, De Quervain's disease, trigger finger, Raynaud's syndrome, and carpal tunnel syndrome. Tendon disorders are very common and often occur at or near the joints where the tendons rub against ligaments and bones. The most frequently noted symptoms of tendon disorders are a dull aching sensation over the tendon, discomfort with specific movements, and tenderness to touch. Recovery is usually slow and the condition may easily become chronic if the cause is not eliminated.

Low back disorders are among the top reasons why patients seek out the services of a physician and can be quite persistent in the face of various methods of treatment. Diagnosis can also pose a problem as primary care physicians are often left with little help from clinical imaging studies. With the multitude of hypotheses that address everything from the etiology of damage in the lumbar spine to how the patient's own psychological and social environment may impact on their experience of pain, what is clearly needed is a reference which can intelligently review what is known and sort through some of the more promising routes which are being explored in treating the lower back. Low Back Disorders: A Medical Enigma by Dr. Calliet who has built his career upon the study and management of orthopedic conditions is aimed at serving this purpose. As the title implies, the book does not attempt to side-step the fact that many of the leading theories that have been proposed over the years often fail and in many respects we are no closer to a definitive "answer" to the low back problem than those physicians and researchers before us.
Treatment protocols that have arisen from working with patients with a variety of presentations are discussed in the last section of the book and reflect the diversity that seems to be present in etiology. Exercises which address muscular imbalances are discussed as well the ability of chiropractic manipulation and traction to alleviate pain and reduce relapses. Very importantly, this section talks about how some patients may require a psychosocial intervention, showing the true impact that non-physical variables may have in these disorders. Research suggesting that increased sympathetic nervous system activity due to psychological factors can exacerbate the experience of pain is also presented.

T he Bone and Joint Decade focuses on five disorder groups: joint diseases, osteoporosis, back disorders, severe injury and children's severe joint diseases.
Joint diseases cover dozens of disorders. The most common is osteoarthritis. It appears usually in the joints of toes, fingers and the spine, but the biggest health problems are caused by osteoarthritis that appears in the hip or knee areas. Around 250,000 Finns are more or less disabled because of osteoarthritis. It is becoming more common while population's middle age rises. About half of the over 65-year olds have osteoarthritis.
Rheumatoid arthritis is a long-term inflammatory disease of the joints. There are around 1,700 new adult cases in Finland yearly. It appears most commonly in women between the ages of 60 and 70, least commonly in young men. There are about 37,000 adults with rheumatoid arthritis in Finland.
Osteoporosis is a general condition of the bones related to aging. It is characterised by an increase in bone fragility and susceptibility to fracture risk. Most typical osteoporotic fractures are the vertebral crush in the spine, and wrist and hip fractures. An estimated 400,000 people in Finland suffer from osteoporosis. There are some 35,000 osteoporotic fractures in Finland yearly.

  • Unload the lumber spine during sitting by shifting the load to thighs and upper spine

Yet, as far as your back is concerned, that’s pretty much what you do whenwhile balancing a large bag of groceries on your hipyou twist down to a child’s car seat, reach over to unlock the harness, and pick up 27 pounds of squirming toddler. It was just such a maneuver that recently landed 42-year-old Mark H. in the office of Duke orthopedic surgeon Lloyd Hey, MD, with severe back pain. Many people put themselves through similar contortions several times every day. The fact that so many people also suffer from back pain is not a coincidence. Whether it’s caused by back disorders, disc disorders, or injuries to the back (see a list of common back pain culprits, below), back pain ranks second only to headaches as the most frequent source of pain complaints. By some estimates, back pain sufferers ring up more than $90 billion annually in health-care expenses, and account for 33 cents of every dollar spent on U.S. disability claims. At Duke, Hey and his colleagues take care of patients from across the southeast U.S. and around the world with every sort of painful back condition, from herniated disks to congenital spinal deformities. Hey’s specialty, in fact, is helping patients with serious spinal problems like scoliosis to live normal, healthy lives. But he says that most people with back pain don’t need aggressive treatment so much as a tutorial in responsible spine stewardship.

    The Thoracic Spine and Associated Disorders and Treatment
    by Joseph Kurnik,DC My previous articles have referred for the most part to the relationship of the lumbar spine and sacroiliac functioning. The middle (and especially the lower) thoracic spine also may influence SI joint functioning. There are a few situations involving the mid and lower thoracic regions which can be looked at in relation to SI joint functioning:
  • thoracic extension fixations (dysfunctions); and
  • thoracic round back disorders In the case of thoracic extension dysfunctions, which may include rotation and lateral bending dysfunctional elements, compensation for such extension restrictions will occur in sacral areas, but especially in the lower lumbar spine. Lower lumbar compensation will be in the form of increased extension. Increased extension will cause increased posterior disc and facet compression. This can lead to increased sacral counter-nutation and AS ilium fixation dysfunctions. The same principle applies to upper lumbar extension restrictions. The result of this can be low back pain, hip pain/disorders, groin strain/discomfort, and various knee complaints. Correction of the thoracic and upper lumbar extension fixations can result in partial or complete release of the AS ilium fixations.

It can range from a dull, annoying ache to absolute agony. Many cases of back pain are caused by stresses on the muscles and ligaments that support the spine. Sedentary jobs and lifestyles may create a vulnerability to this type of stress or damage. Obesity, which increases both the weight on the spine and the pressure on the discs, is another factor. Strenuous sports such as football and gymnastics can also damage the back. Is there any treatment?

    Office Ergonomics Training This site contains office ergonomics information aimed at ergonomics committee members as well as office workers in general. The information here is based on the research literature whenever possible; it may not be consistent with some of the conventional wisdom that has persisted for decades, such as "sit up straight" or "have the monitor at eye level" or "the keyboard should be at elbow height." Office ergonomics deserves to be treated separately from general (industrial) ergonomics. The principles are the same, but there are so many subtle differences and different products that it's best to specialize. This site collects and publishes some "handouts" that have been used widely in training courses for office workers and ergonomists and health care professionals who work with office workers. The handouts have received good reviews with respect to their clarity, objectivity, and scientific basis. Some have been published in slightly different form in book chapters and manuals. We are pleased to share them with you. This site was developed by Chris Grant, PhD, now retired from ergonomics. Her Michigan-based ergonomics practice, F-One Ergonomics, was limited to office-related issues. Dr. Grant and other cited authors on the site are Certified Professional Ergonomists and Certified Industrial Ergonomists who have consulted in this field for years and have been involved in the development of national and international ergonomics standards. Dr. Grant has generously transferred the site to Dennis Ankrum, who will strive to maintain the standard of excellence she has set.

    SUMMARY
    Over 40 recent articles provided evidence regarding the relationship between low-back disorder and the five physical workplace factors that were considered in this review. These included (1) heavy physical work, (2) lifting and forceful movements, (3) bending and twisting (awkward postures), (4) whole-body vibration (WBV), and (5) static work postures. Many of the studies addressed multiple work-related factors. All articles that addressed a particular workplace factor contributed to the information used to draw conclusions about that risk factor, regardless of whether results were positive or negative. The review provided evidence for a positive relationship between back disorder and heavy physical work, although risk estimates were more moderate than for lifting/forceful movements, awkward postures, and WBV. This was perhaps due to subjective and imprecise characterization of exposures. Evidence for dose-response was equivocal for this risk factor.
