Attention Deficit Disorder Discussion Forum. “Enthusiastic student” and “Pleasure to have in class” appeared on my son’s first mid-term report this year, replacing the “Very little effort” and “Missing assignments” comments that chronically plagued past mid-terms and report cards. My son’s seventh grade teachers gave glowing reports of a young man who handed class work and homework in on time, who paid attention and actively participated in class. I am still amazed that this is the same person who, just one year prior, struggled to maintain a “C” average in school, was tested for learning disabilities and received an Attention Deficit Disorder diagnosis ( Causes of ADHD ) after a school career marked with distress and defeat. When we talk about the use of stimulant medications in psychiatry, we generally are referring to methylphenidate (Ritalin) amphetamine (Dexedrine, Adderall) methamphetamine (Desoxyn) and pemoline (Cylert.) The first two are by far the most commonly prescribed. The stimulant medications increase the release or block the reabsorption of dopamine and norepinephrine, two brain neurotransmitters. This increases the transmission between certain neurons. Each stimulant has these effects in slightly different ways. 20 mg Starting dose for children is 5 mg twice daily, 3-4 hours apart. Add third dose about 4 hours after second. Adjust timing based on duration of action. Increase by 5-10 mg increments. Daily dosage above 60 mg not recommended. Estimated dose range .3-.6 mg/kg/dose Insomnia, decreased appetite, weight loss, headache, irritability, stomachache, and rebound agitation or exaggeration of pre-medication symptoms as it is wearing off. About 3-4 hours. Most helpful when need rapid onset and short duration. Works quickly (within 30-60 minutes). Effective in over 70% of patients.

Mario's Story
Mario is 10 years old. When he was 7, his family learned he had AD/HD. At the time, he was driving everyone crazy. At school, he couldn’t stay in his seat or keep quiet. At home, he didn’t finish his homework or his chores. He did scary things, too, like climb out of his window onto the roof and run across the street without looking. Things are much better now. Mario was tested by a trained professional to find out what he does well and what gives him trouble. His parents and teachers came up with ways to help him at school. Mario has trouble sitting still, so now he does some of his work standing up. He’s also the student who tidies up the room and washes the chalkboard. His teachers break down his lessons into several parts. Then they have him do each part one at a time. This helps Mario keep his attention on his work. At home, things have changed, too. Now his parents know why he’s so active. They are careful to praise him when he does something well. They even have a reward program to encourage good behavior. He earns “good job points” that they post on a wall chart. After earning 10 points he gets to choose something fun he’d like to do. Having a child with AD/HD is still a challenge, but things are looking better.
