When I was born the doctors told my parents that I would never be able to walk or do things for myself. We proved them wrong! Sister had a lot to do with that, I think she was tired of carrying me! Our Daughter's Arthrogryposis
By Barb Burke Our family has come in contact with so many beautiful children and families struggling with Arthrogryposis. They have been such an inspiration and encouragement to our family! My husband and I did some ground work prior to accepting the referral of our daughter from the adoption agency. We attended a full-day seminar on Arthrogryposis at our local Children's Hospital headed by Dr. Judith Hall, the leading expert on the subject. She has researched this condition for the past 20 years and revealed cutting-edge information on the subject. I also contacted a few parents who were raising children with arthrogryposis and found them most helpful. Initial Contact We first heard of our daughter residing in a babies home in Seoul, Korea when she was 4 months old. At that time, she had been diagnosed as being born with a type of Arthrogryposis known as Amyoplasia, affecting only her upper limbs. Within 10 days of life, her biological parents placed her in the care of an agency for adoption because they were given little hope for her future, and felt unable to afford the needed care. We first met Soo (Excellent) Hee (Brilliant) through a picture at age four months. She was born with shoulders internally rotated, elbows fixed in extension,and flexion contracture of her wrists. She received range of motion by a therapist visiting her 3x's weekly in the orphanage. The US doctors believe this saved her small muscles from atrophying.

Okay, probably never... but I have, hundreds of times! That's because I do in fact have arthrogryposis multiplex congenita , commonly shortened to arthrogryposis or, even more commonly, AMC. The easiest way to understand the technical definition of AMC is to look at each word individually. Arthrogryposis (are-throw-gripe-oh-sis) means simply, "joint contracture". A contracture is any tightening or deformity of a joint. The most well known type of contracture is a club foot... BUT , one club foot (or even two) does not equal AMC. The next word, multiplex , rather obviously means "multiple". In AMC, contractures occur in at least two different joints, for example a foot and an elbow, but not just two feet. So, what if your grandfather had a stroke and now he has a flexed elbow and a tight shoulder... does he have AMC? Nope, the final word, congenita , means the contractures were present at birth.

What causes arthrogryposis? There are many possible causes for arthrogryposis multiplex congenita (AMC), but no specific known cause. Generally, problems with joint growth and development, decreased fetal movement, and spinal development during the first 3 months of pregnancy may lead to the development of AMC. AMC is not an inherited disorder, so a person with AMC will not pass it on to his children. Distal arthrogryposis, which is rarer than AMC, is inherited, and can be passed from parent to child.
Is there a cure for arthrogryposis multiplex congenita (AMC)? Currently, there is not a cure for arthrogryposis, but there are several treatments that can help children who have arthrogryposis live fuller lives. Splinting and bracing can help improve range of motion. Surgical intervention can improve joint position. Parents often ask about muscle transplants; however, these are not currently a valid treatment option. Muscle transfers may be used to provide power to a flexible joint, such as creating elbow flexion by the transfer of triceps to create "biceps" in one arm. Each individual must be evaluated for total body function before these transfers are considered.

    AMC is a nonprogressive congenital neuromuscular syndrome characterized by severe joint contractures, muscle weakness, and fibrosis.
    Epidemiology
    AMC occurs in 1 out of every 3000 live births. Amyoplasia, characterized by fatty and fibrous tissue replacement of the limb muscles, is the most common form (43%).
    Pathophysiology
    The principle cause of AMC is believed to be decreased fetal movements (akinesia) caused by maternal or fetal abnormalities. It is associated with neurogenic and myopathic disorders. It is believed that the neuropathic form of AMC involves a deterioration in the anterior horn cell leading to muscle weakness and fibrosis.
    Etiology
    Several factors are implicated in AMC including hyperthermia of the fetus, prenatal virus, fetal vascular compromise, septum of the uterus, decreased amniotic fluid, muscle and/or connective tissue developmental abnormalities.
    Clinical Manifestations
    The primary joints involved (in order of decreasing prevalence) include the foot, hip, wrist, knee, elbow, and shoulder. AMC is typically symmetrical and

    "Arthrogryposis" (Arthrogryposis Multiplex Congenita) is a term describing the presence of multiple joint contractures at birth. A contracture is a limitation in the range of motion of a joint. Arthrogryposis translated from the Greek literally means "curved or hooked joints." In some cases, few joints maybe affected and the range of motion may be nearly normal. In the "classic" case of Arthrogryposis, hands, wrists, elbows, shoulders, hips, feet, and knees are affected. In the most severe cases, nearly every body joint may be involved, including the jaw and back. Frequently, the joint contractures are accompanied by muscle weakness which further limits movement. Arthrogryposis is relatively rare, occurring in perhaps one in 10,000 births in Ireland.
    WHAT CAUSES IT?
    A characteristic feature of the condition is dimples near the affected joints and these suggest contact between the skin and bone during the early stages of pregnancy. As many joint spaces have formed by the seventh week and folds appear in the skin by the eleventh week, this gives an indication as to when the disabilty is caused.

    Symptoms
    In typical Arthrogryposis Multiplex Congenita (AMC), the range of motion of the joints of all limbs is limited or fixed. The shoulders are bent inward and internally rotated, the elbows are extended, and the wrists and fingers are bent. The hips may be dislocated and are usually slightly bent, the knees are extended, and the heel is bent inward from the midline of the leg while the foot is bent inward at the ankle (typical clubfoot).
