Acute Appendicitis: Review and Update
D. MIKE HARDIN, JR., M.D.,
Appendicitis is common, with a lifetime occurrence of 7 percent. Abdominal pain and anorexia are the predominant symptoms. The most important physical examination finding is right lower quadrant tenderness to palpation. A complete blood count and urinalysis are sometimes helpful in determining the diagnosis and supporting the presence or absence of appendicitis, while appendiceal computed tomographic scans and ultrasonography can be helpful in equivocal cases. Delay in diagnosing appendicitis increases the risk of perforation and complications. Complication and mortality rates are much higher in children and the elderly. (Am Fam Physician 1999;60:2027-34.) A ppendicitis is the most common acute surgical condition of the abdomen. Approximately 7 percent of the population will have appendicitis in their lifetime, with the peak incidence occurring between the ages of 10 and 30 years. Despite technologic advances, the diagnosis of appendicitis is still based primarily on the patient's history and the physical examination. Prompt diagnosis and surgical referral may reduce the risk of perforation and prevent complications.

Appendicitis symptoms
Here are symptoms of appendicitis. Appendicitis, infection of the appendix, occurs less frequently today than it did in the past.
The appendix is a wormlike expansion of the large bowel (cecum) measuring one to three inches in length. It is located in the lower right portion of the abdomen at the beginning of the large intestine. Normally it is about as thick as a lead pencil and is pinkish gray in color. It serves no function in humans that we know of and is thought to be a residuum of our primitive past. Appendicitis: bodyOffer(27226) Appendicitis is an inflammation of the lining of the appendix spreading to its other portions. When an acute inflammation is involved, severe pain will be felt in the lower right part of the abdomen. By this time the appendix has usually become filled with pus. If not treated right away the infection can spread through the wall of the appendix and can turn into gangrene and rupture. Because of this, appendicitis is considered an emergency situation. Causes: Appendicitis may be caused either by bacterial inflammation or by a viral infection occurring in the digestive tract. A bacterial infection can happen by a rigid particle of stool blocking its passageway causing pressure upon the blood vessels in the area. The inflammation can cause infection, a blood clot, or rupture of the appendix.

The Basics
Symptoms What Is Appendicitis? Appendicitis is an inflammation of the appendix, a 3 1/2-inch-long tube of tissue that extends from the large intestine. The appendix contains specialized tissue that can produce antibodies, but no one is absolutely certain what its function is. One thing we do know: We can live without it, without apparent consequences. Appendicitis is a medical emergency that requires prompt surgery to remove the appendix. Left untreated, an inflamed appendix will eventually burst, or perforate, spilling infection into the abdominal cavity. This can lead to peritonitis, a serious infection of the abdominal cavity's lining (the peritoneum) that can be fatal unless it is treated quickly with strong antibiotics. Sometimes a pus-filled abscess forms outside the inflamed appendix. Scar tissue then "walls off" the appendix from the rest of the abdomen, preventing infection from spreading. An abscessed appendix is a less urgent situation, but unfortunately, it can't be identified without surgery. For this reason, all cases of appendicitis are treated as emergencies. In the United States, 1 in 15 people gets appendicitis. Although it can strike at any age, appendicitis is rare under age 2 and most common between ages 15 and 30.

You are in: Esophagus, Stomach, and Intestine Appendicitis Overview The appendix is a narrow tubular pouch that is attached to the intestines. When the appendix is blocked, it becomes inflamed and results in the condition known as appendicitis. If the blockage continues, the inflamed tissue becomes infected with bacteria and begins to die from a lack of blood supply, which finally results in the appendix bursting. Appendicitis is a common condition that affects 6% of the population sometime during their lifetimes. Although it most commonly occurs among those between the ages of 10-30 years, it can occur at any age. Increased vigilance is required for the very young and old because these people have a higher rate of complications.
