Appendicitis is an inflammation of the appendix, a finger-like glandular piece of tissue that connects to the beginning of the large intestine, usually at the lower right side of the abdomen. In older children, the classic symptoms of appendicitis are abdominal pain, fever, and vomiting. Abdominal pain usually begins in the center of the abdomen, around the area of the navel ("belly button"). Later, the pain may move downward and to the right - to an area called McBurney's point, roughly corresponding to the location of the appendix in the lower right portion of the abdomen. After his abdominal pain begins, a child with appendicitis usually develops a slight fever, loses his appetite, feels nauseous, and may vomit. The fact that abdominal pain begins before nausea and vomiting, rather than after, is one clue to suspect appendicitis rather than an intestinal infection. Other symptoms that may be seen in older children with appendicitis include: diarrhea (usually small stools with mucus); urinary tract symptoms (urinating very frequently and/or an uncomfortably strong urge to urinate); constipation; and, sometimes, respiratory symptoms. In children younger than age 2, the most common symptoms are vomiting and a distended abdomen (abdomen that is bloated or swollen). These children may also have abdominal pain, but either they are too young to be able to tell adults about it, or adults miss the symptom entirely. Because appendicitis is rare in infants, and their symptoms are not "classic," the diagnosis of appendicitis is often delayed in these children. Often, in children between ages 5 and 9, appendicitis is diagnosed as either gastroenteritis or a respiratory infection - much more common illnesses in this age group.

Causes, incidence, and risk factors Return to top Appendicitis is one of the most common causes of emergency abdominal surgery in children. Approximately 4 appendectomies per 1,000 children are done annually in the United States. Appendicitis is more common in males than in females, and incidence peaks in the late teens and early 20s. The condition is uncommon among children younger than 2, but it can occur. Appendicitis generally follows obstruction of the appendix by feces (fecalith), a foreign body, or rarely, a tumor . Typically, the first symptom is crampy or "colicky" pain around the navel (periumbilical). There is usually a marked reduction in or total absence of appetite, often associated with nausea, and occasionally, vomiting and low grade fever.

It can easily become inflamed, usually after a piece of stool or food is trapped inside. Once inflamed, the appendix will swell, become infected and cause pain. Most parents are familiar with the symptoms of appendicitis, especially the severe pain in the right lower side of a child's abdomen, but many parents still worry about appendicitis whenever their kids have belly pain, even if they don't have classic symptoms.   • Overview . Appendicitis is a common cause of abdominal pain. However, the presentation is not always classic and a high index of suspicion is necessary. Affects any age group but is rare in infants, most common in adolescence and young adult years. Generally occurs from obstruction of the appendiceal lumen by lymphoid hyperplasia or a fecalith.
  • Clinical Presentation.
  • History . Classic history is that of periumbilical or epigastric pain that migrates to right lower quadrant. Pain may be felt in flank (retrocecal appendix, pregnancy), testicle (retroileal appendix), or bladder. Anorexia, nausea, and vomiting may occur after the onset of pain. Anorexia is less likely to be present in children. Presentation is more likely to be atypical in very young, very old, and pregnant patients. Maintain a high index of suspicion in any patient with abdominal pain. Physical exam. Fever is only found in 15%.

    What is appendicitis? Appendicitis results from inflammation of the appendix. (The appendix is an outpouching from the right side of the large intestine.) This often occurs because the appendix has become obstructed, or blocked. However, no one really knows why this blockage happens in the appendices of some children and not in othersso it probably cannot be prevented. Appendicitis is the most common cause of emergency abdominal surgery in children in the United States. It's somewhat more common in boys than in girls, and more common in teenagers than in younger children. However, appendicitis is seen in children of all ageseven (very rarely!) newborn babies. Signs and symptoms of appendicitis The most common presentation of appendicitis usually begins with vague pain in the middle of the abdomen. The child then begins to vomit, and loses his or her appetite. The pain then moves to the lower right side of the abdomen. The child may develop a low-grade fever, and not want to walk. The child may notice that the pain gets worse during the bumpy parts of a car ride to the doctor. When the doctor asks the child to cough, or presses the lower right side of the child's abdomen, the pain will also get worse.

    The appendix is a closed-ended, narrow tube that attaches to the cecum (the first part of the colon ) like a worm. (The anatomical name for the appendix, vermiform appendix, means worm-like appendage.) The inner lining of the appendix produces a small amount of mucus that flows through the appendix and into the cecum. The wall of the appendix contains lymphatic tissue that is part of the immune system for making antibodies. Like the rest of the colon, the wall of the appendix also contains a layer of muscle. What is appendicitis?
    Appendicitis is inflammation of the appendix. It is thought that appendicitis begins when the opening from the appendix into the cecum becomes blocked. The blockage may be due to a build-up of thick mucus within the appendix or to stool that enters the appendix from the cecum. The mucus or stool hardens, becomes rock-like, and blocks the opening. This rock is called a fecalith (literally, a rock of stool). At other times, the

    What is appendicitis?
    The appendix (A) is a small worm-like pouch attached to the large bowel (B) right after the small intestine (C). The function of the appendix is uncertain and there seems to be no long-term problems in living without it. Appendicitis is an inflammation of the appendix, a small worm-like pouch attached to the large bowel. It can happen at any age but most cases are between 8 and 25 years of age. It is rarely seen in children aged under two years. For young people, appendicitis is probably the most common cause of stomach pain requiring emergency surgery.

    It is possible for matter to become lodged in the appendix, leading to bacterial infection , which can cause appendicitis. If the condition is untreated, the appendix can turn gangrenous and can eventually burst, leading to peritonitis septicemia and eventually death edit
    Signs, symptoms and examination findings
    The pain of appendicitis usually starts centrally before localising to the right iliac fossa (the lower right side of the abdomen). There is usually associated anorexia (loss of appetite). Fever is usually present. Nausea, vomiting and diarrhea may or may not occur. There is typically pain and tenderness in the right iliac fossa. Rebound tenderness may be present suggesting that there is some element of peritoneal irritation. If the abdomen is guarded, there should be a strong suspicion of peritonitis requiring urgent surgical intervention. edit

    Diagnosis is based on history and physical examination backed by blood tests and other diagnostic procedures.
