Everyone has brief pauses in their breathing pattern called apnea - even your child. Usually these brief stops in breathing are completely normal. Sometimes, though, apnea or other sleep-related problems can be a cause for concern. Read on to find out what it means when your child's doctor mentions apnea. Types of Apnea
The word apnea comes from the Greek word meaning "without wind." Although it's perfectly normal for everyone to experience occasional pauses in their breathing, apnea can be a problem when breathing stops for 20 seconds or longer. There are three types of apnea: obstructive, central, and mixed. Obstructive Apnea
This type of apnea is caused by an obstruction of the airway (such as enlarged tonsils and adenoids ), and it is a common type of apnea in children. As many as 1% to 3% of otherwise healthy preschool-age children have obstructive apnea. The most common symptom is snoring; others include labored breathing while sleeping, gasping for air, sleeping in unusual positions, and changes in color. Because obstructive sleep apnea may disturb sleep patterns, these children may also show continued sleepiness after awakening in the morning, and tiredness and attention problems throughout the day. Sometimes this can affect school performance. One recent study suggests that some children diagnosed with AD/HD actually have attention problems in school because of disrupted sleep patterns caused by obstructive sleep apnea.

Abbreviations
A
Alpha activity
The presence of alpha waves or alpha rhythm (i.e., electrical oscillations in the alpha frequency) in the EEG of humans. See Alpha rhythm.
Alpha intrusion (-infiltration, -insertion, -interruptions "riddled with" alpha)
A brief interposition of alpha activity during a stage of sleep.
Alpha rhythm (-activity, -frequency)
EEG oscillations with a frequency of 8-13 Hz in adults, prominent over the occipital cortex: indicative of the awake state in humans; present in most, but not all, normal individuals most consistent and predominant during relaxed wakefulness, particularly with reduction of visual input. The alpha frequency has a range in each individual: the low end is exhibited in drowsiness or sleep and the upper end with alertness. The frequency range also varies with age; it is slower in children and older age groups relative to young and middle-aged adults.
Arousal
An abrupt change from a "deep" stage of NREM sleep to a "lighter" stage, or from REMS to awake, with the possibility of awakening as the final outcome. Arousal may be accompanied by increased tonic EMG activity and heart rate, as well as body movements.
Awakening (full arousal)
The return to the polysomnographically defined awake state from any of the NREM stages or REMS: characterized by alpha and beta waves, rise in tonic EMG, voluntary REMS, and eye blinks. This definition of awakening is valid only insofar as the polysomnogram is matched by a resumption of a reasonably alert state of consciousness.

Obstructive Sleep Apnea
What is obstructive sleep apnea?
Obstructive sleep apnea syndrome (OSAS) is a debilitating sleep and breathing disorder defined as the cessation of breathing for 10 seconds or more (an apnea) at least five times per hour of sleep. During sleep, the body's muscles relax, which can cause excess tissue to collapse into the upper airway (back of the mouth, nose and throat) and block breathing. When breathing is interrupted by an obstruction in the airway, the body reacts by waking enough to start breathing again. These arousals may occur hundreds of times each night but do not fully awaken the patient, who remains unaware of the loud snoring, choking and gasping for air that are typically associated with obstructive sleep apnea. Obstructive sleep apnea sufferers never get "a good night's sleep" because repeated apneas and arousals deprive patients of REM and deep-stage sleep, leading to chronic

Most of us spend 5-8 hours sleeping every night and take it for granted that we will fall asleep easily and wake up refreshed in the morning. However, most of us will have some problem with sleep at least once in our lives and 1 in 5 have a chronic problem with sleep. We all breath at a rate of 10-20 times a minute without conscious effort. However, 1 in 5 individuals will have some problem with breathing due to smoking, occupational exposures, or conditions such as asthma (which affects 1 in 20). Obstructive Sleep Apnea is a very under-diagnosed sleeping disorder which I was diagnosed with in December 1995. Apnea is not realistically a life or death issue but it is very much about quality of life. Since my diagnosis I have been getting treatment. To find out more about the condition, which is most noticeable in those who snore, see the following: Apnea: The Sleep Well Sleep Apnea IF YOU CAN ANSWER YES TO MORE THAN ONE OF THESE you should tell your doctor to check you out for sleep apnea:
2. My neck size is 17 or over
3. I fall asleep in inappropriate places
4. I feel way below par much of the time
5. Someone who can hear me sleep has noticed that I snore very loudly, then stop breathing and then start breathing again with a gasp.

Obstructive Sleep Apnea and DS
by Dr. Len Leshin, MD, FAAP
Apnea (literally, "without breath") is the term used when someone stops breathing for very short periods of time, usually 10 to 20 seconds. It's termed "obstructive" when respiratory efforts continue, such as movements of the chest. It's termed "central" when all respiratory effort stops. There is also a mixed version. In children, sleep apnea is almost always obstructive. During the apneic episode, the child will have decreased oxygenation of the blood. Children with Down syndrome (DS) are certainly at risk for OSA. In 1991, one study showed 45% had OSA. This can be caused by several different factors present in DS: the flattened midface, narrowed nasopharyngeal area, low tone of the muscles of the upper airway and enlarged adenoids and/or tonsils.
Why is this important? Well, first, there's the obvious problem of the child not getting enough quality sleep and the behavioral effects that brings. Second, I've mentioned above that during sleep apnea, the oxygenation of the blood decreases. It has been shown that in children with DS and heart disease this low oxygenation causes an increase in the blood pressure in the lungs as the body tries to get more oxygen. This "pulmonary hypertension" can cause the right side of the heart to become enlarged and other cardiac complications can follow. The incidence of death due to OSA is unknown.
