Do you ever feel sleepy or "zone out" during the day? Do you find it hard to wake up on Monday mornings? If so, you are familiar with the powerful need for sleep. However, you may not realize that sleep is as essential for your well-being as food and water.
Sleep: A Dynamic Activity
Until the 1950s, most people thought of sleep as a passive, dormant part of our daily lives. We now know that our brains are very active during sleep. Moreover, sleep affects our daily functioning and our physical and mental health in many ways that we are just beginning to understand. Nerve-signaling chemicals called neurotransmitters control whether we are asleep or awake by acting on different groups of nerve cells, or neurons, in the brain. Neurons in the brainstem, which connects the brain with the spinal cord, produce neurotransmitters such as serotonin and norepinephrine that keep some parts of the brain active while we are awake. Other neurons at the base of the brain begin signaling when we fall asleep. These neurons appear to "switch off" the signals that keep us awake. Research also suggests that a chemical called adenosine builds up in our blood while we are awake and causes drowsiness. This chemical gradually breaks down while we sleep.

For mild cases of obstructive sleep apnea, treatment often consists of using methods to avoid sleeping on one’s back. For people with significant nasal congestion, a decongestant therapy may be prescribed. Patients with obstructive and central apnea should avoid central nervous system depressants such as alcoholic beverages, sedatives and narcotics. Weight loss and diet control are encouraged for overweight patients. Many serious cases of obstructive sleep apnea can be relieved by a treatment called nasal continuous positive airway pressure (nasal CPAP). Nasal CPAP uses a mask-like device and pump that work together to keep the airway open with air pressure during each inspiration. Surgery may benefit some patients by eliminating or reducing the narrowing of the airway due to anatomical defects.

Sleep apnea is a sleeping disorder in which a person stops breathing repeatedly during their sleep, sometimes hundreds of times during the night, and often for a minute or longer. A sufferer of sleep apnea has frequent episodes (up to 400-500 per night) in which he or she stops breathing. Sleep apnea warning signs include excessive snoring, choking, or gasping during sleep. There are three different types of sleep apnea; obstructive sleep apnea, central sleep apnea, and mixed (a combination of the two). The Greek word "apnea" literally means "without breath." With each apnea event, the sleeper's brain briefly arouses in order to resume breathing, causing sleep to be extremely fragmented and of poor quality. Sleep apnea is a serious, potentially life-threatening condition that can cause many health problems if left untreated.

The "I" in the title is me. The "I" in the quote is Ishmael, the narrator of Melville's Moby Dick . We both experienced what is known as "sleep paralysis." My first episode occurred many years ago, probably before I went to medical school. Since then I've had the experience three or four more times, a frequency of about once a decade. Each time is terrifying. I suddenly am awake and aware of the bed and my situation, but realize I cannot move. It is not a dream; I desperately want to move but no muscles respond. I can breathe, but on at least one occasion my head was under the blanket, and I felt a dread of suffocation.

Jane Marschand is 38 years old, a heavy woman, 5'6" tall and about 250 lbs. She is referred by her psychiatrist for "sleep disorder." After my nurse takes her vital signs I go in, introduced myself and began the interview. "What seems to be the problem?" I ask. "I sleep eat." "Oh?" I raise my eyebrows slightly, indicating, 'that's interesting,' and hastily skim through her thick office chart. She has been seeing a psychiatrist on and off for years. Current diagnosis is "BiPolar Disorder, Depressed phase." Psychiatric medications Lamictal 100 mg bid, 100 mg Seroquel hs are evidently effective, as she is working and raising a family. I see somewhere on the pages that she is married and has two teen age sons. "What do you mean? You eat while you sleep?"

What is sleep apnea?
Sleep apnea (sleep-disordered breathing) is a serious and common sleep disorder affecting about 12 million Americans, according to the National Institutes of Health (NIH). Its name comes from a Greek word, apnea , meaning "without breath." People with sleep apnea stop breathing briefly many times during the night. The breathing pauses last at least 10 seconds, and there may be 20 to 30 or more pauses per hour. The main symptoms of sleep apnea are persistent loud snoring at night and daytime sleepiness. Another symptom is frequent long pauses in breathing during sleep, followed by choking and gasping for breath. People with sleep apnea don't get enough restful sleep, and their daytime performance is often seriously affected. Sleep apnea may also lead to high blood pressure, heart disease, heart attack, and stroke. However, it can be diagnosed and treated.

Alternate Names : Sleep Apnea - Central Definition Central sleep apnea involves cessation of breathing during sleep, caused by problems with brain mechanisms that control breathing. (Note: This is not the more common obstructive sleep apnea caused by obesity or other problems, which results in loud snoring.) Central sleep apnea is rare in people who are not seriously ill. It occurs in patients with a variety of severe and life-threatening lower brain stem lesions. The brainstem controls breathing. As a result, any disease or injury affecting this area may result in problems with normal breathing during sleep or when awake. Conditions that can cause sentral sleep apnea include bulbar poliomyelitis
