Anorexia
, or the lack or loss of appetite that is combined with decreased food intake, is one of the most frequently reported problems for people with cancer. A poor appetite can result from the effects of the cancer itself or from treatments for cancer. It is the most common symptom contributing to poor nutrition in cancer patients. Some evaluators feel that as many as 80% of those with cancer experience changes in appetite. Anorexia can keep you from eating enough nutrients . It can have a severe impact on your quality of life. Sometimes these changes last a few days. For others anorexia becomes an ongoing concern. Anorexia can be one of the most challenging symptoms for patients and families.
Anorexia associated with cancer is the result of many aspects of altered body function. Researchers continue to learn what has an effect on eating and digestion. Studies try to determine how each identified function interacts within the process to maintain adequate nutritional health. A cancerous tumor can affect the appetite, the digestion of food eaten, and the energy requirements of the body. Dr. Gordon is Assistant Director, Family Practice Residency Program, Stamford Hospital/Columbia University, Stamford, Conn. If you are a practicing physician, you have patients with eating disorders. You may not be aware of the female patient who regularly binges on thousands of calories and then forces herself to vomit, or of the teenaged boy who aims to eat less than 400 calories a day while exercising rigorously. It is estimated that in the United States, five to ten million women and up to one million men have an eating disorder. These may be underestimates, especially if one takes into account the diagnosis of binge eating disorder that was introduced in the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV) of the American Psychiatric Association. Anorexia nervosa has the poorest prognosis of all the eating disorders; however, the prognosis is moderately improved if the disorder is detected and treated in its early stages. To that end, this article will highlight signs and symptoms that may be indicative of anorexia and interventions that are within the scope of the primary care physician. In addition, a brief description of some of the theories of anorexia's origins may help physicians to prevent the disorder in some patients.
Etiology
Theories about the causes of anorexia nervosa include both the psychological and biologic. One psychological theory is that food intake and weight are areas that a young woman can control in a life otherwise dictated by an overly involved family, which may include a parent who is unduely concerned with weight or appearance.

Once a vigorous, lithesome young girl, Susie lay quietly among an array of tubes and sterile machines. She was not hospitalized due to a tragic illness or from an accident. Rather, she exemplifies one of the many cases of self-inflicted starvation that sadly affects one percent of our nation's women. You may know or even be an anorexic. Why do these girls torture themselves by denying that most basic human need: food? Teenagers are supposed to have fun and eat ice cream, pizza, and jelly-belly's. Most do, but some become stuck in one of the most difficult psychological maladies to overcome. The answers are varied, but for now we will deal with the most common issue contributing to the disease: pressure. Pressures to be a "super-girl" begin at a very young age nowadays. Whether inflicted by demanding parents, the media, coaches or teachers, the result for the anorexic is a serious lack of self-confidence. There are very high expectations for these girls to achieve. They often equate achievement with love. They become people-pleasers in search of the love and acceptance they so desperately crave.

Much is written about teen anorexia. But what about the anorexic teenagers who become young women in their twenties? Many fall in love, get married and try to build a life with their husbands just like other young women. The difference is that the anorexic young woman has anorexic thinking and feeling influencing every decision and action in her life. She is often very afraid. Most people in their mid-twenties go through a kind of developmental shock as they are confronted by new and different kinds of personal challenges in their lives. The woman is only recently no longer a young girl. There are new responsibilities to get hold of and has different expectations placed on her by others and herself. Whether she accepts those expectations or not, she still has to deal with them. This is a particularly stressful and often overwhelming time for an anorexic young woman. An anorexic who for years has been doing a 'good job' at being anorexic is hiding in plain sight all the time. She's thin, but not skeletal. According to fashion dictates, she is elegantly lean in a most feminine way.

What is anorexia?
Anorexia nervosa is often considered to be a type of eating disorder in an entire spectrum of eating disorders. It is also, and more importantly, a psychological disorder. It is a condition that goes beyond out-of-control dieting. The person with anorexia initially begins dieting to lose weight. Over time, the weight loss becomes a sign of mastery and control. The drive to become thinner is actually secondary to concerns about control and/or fears relating to one's body. The individual continues the endless cycle of restrictive eating, often to a point close to starvation, in order to feel a sense of control over the body. This becomes an obsession and is similar to an addiction to any type of drug or substance. Anorexia generally affects females, but can affect males as well. And while anorexia typically begins to manifest itself during early adolescence, it is also seen in young children and adults.
