Tumors Cranial Aneurysm August, 1997 An aneurysm is an abnormal increase in the diameter (dilation) of a blood vessel. These occur in all age groups but the incidence increases steadily for patients over 25 years old. The most devastating consequences of intracranial aneurysms, which often lead to severe disability or death, result from their rupture and bleeding into the space around the brain. A major rupture is often preceded by a "warning leak" which manifests itself as a new or uncharacteristic headache. An aneurysm may also be detected prior to rupture due to pressure on surrounding nerves. The outcome for patients treated before a catastrophic hemorrhage is much better than those treated after, so the need for adequate evaluation of patients suspected of harboring an intracranial aneurysm is of paramount importance. Unruptured intracranial aneurysms can be detected by non-invasive measures, including

Stent-Graft Repair of Abdominal Aortic Aneurysms
An aortic aneurysm is a weak area in the aorta, the main blood vessel that carries blood from the heart to the rest of the body. As blood flows through the aorta, the weak area bulges like a balloon and can burst if the balloon gets too big. A small aneurysm may require no immediate treatment other than "watchful waiting" - checking the aneurysm regularly to be certain it does not grow. If an aneurysm reaches a certain size, however, there is a danger that it will burst and bleed uncontrollably (hemorrhage). In these cases treatment is necessary. The following information was prepared by the Society of Interventional Radiology (SIR) to provide general information about abdominal aortic aneurysm - the most common type of aortic aneurysm. This site reviews the signs and symptoms, how it is diagnosed, and when it may require treatment. Treatment of an abdominal aortic aneurysm may require surgery. For some patients, however, a new, non-surgical treatment called "stent-graft repair" can be performed by an

  • Characterized by colicky or constant, severe abdominal or back pain which may radiate to groin, hips or lower extremities. May mimic urolithiasis including hematuria and vomiting.
  • Exam may show a pulsatile mass (sensitivity between 44% to 97%) but exam is not adequate to rule out a AAA if you have a clinical suspicion of AAA. May also have a tender and possibly distended abdomen, and signs of shock depending on how much hemorrhage has occurred. Distal pulses are often maintained and are an unreliable sign!
  • Immediate imaging is indicated. Treatment as per thoracic dissection above. Stents are being used for unruptured aneurysms but surprisingly there is no difference in perioperative mortality or complications. Additionally, the grafts tend to migrate over time (1 to 3 years out).
  • Appropriate follow-up of the patient with an asymptomatic AAA found on routine exam:

Some common locations for aneurysms include: Causes, incidence, and risk factors Return to top Aneurysms are either congenital (present before birth) or acquired. It is not clear exactly what causes aneurysms. Defects in some of the components of the artery wall may be responsible. In certain cases (abdominal aortic aneurysms), high blood pressure is thought to be a contributing factor. Atherosclerotic disease (cholesterol buildup in arteries) may contribute to the formation of certain types of aneurysms. Pregnancy is often associated with the formation and rupture of aneurysms of the splenic artery (an artery leading to the spleen).
