May mimic urolithiasis including hematuria and vomiting. Exam may show a pulsatile mass (sensitivity between 44% to 97%) but exam is not adequate to rule out a AAA if you have a clinical suspicion of AAA. May also have a tender and possibly distended abdomen, and signs of shock depending on how much hemorrhage has occurred. Distal pulses are often maintained and are an unreliable sign! Immediate imaging is indicated. Treatment as per thoracic dissection above. Stents are being used for unruptured aneurysms but surprisingly there is no difference in perioperative mortality or complications. Additionally, the grafts tend to migrate over time (1 to 3 years out).
