Anal Fissure An anal fissure is a small tear or cut in the skin around the anus which can cause pain and/or bleeding. Due to the location of fissures, they become aggravated during and after bowel movements. Symptoms of an anal fissure include pain during defecation and red blood in the stool. Constipation or diarrhea can cause a tear in the anal lining resulting in a fissure. A chronic fissure may be due to poor bowel habits, tight or spastic anal sphincter muscles, scarring or an underlying medical problem. Historically chronic fissures were treated with surgical disruption or division of the internal anal sphincter muscle. New medications include topically applied Nitroglycerin and Nifedipine, both of which may be used to decrease anal muscle spasm. Botox A, which needs to be injected, also decreases anal muscle spasm and may reduce the need to treat fissures surgically. Over 90% of anal fissures heal without surgical intervention. Your physician may suggest a high fiber diet and/or stool softeners to avoid constipation so that passage of soft stools may occur. Sitz baths may also be recommended.

Anal Fissure WHAT IS AN ANAL FISSURE?
An anal fissure is a tear in the lining of the anal opening. Almost always the tear results from a combination of two factors:
  • irregular bowel patterns (either hard or liquid stools) and increased anal sphincter tone (usually associated with stress). When hard stools pass through a tight anal opening, a laceration may occur and that is referred to as a fissure. Fissures can also be caused by anal trauma, laxative abuse, childbirth trauma, or laceration by a foreign object. Fissures are not cancerous.
  • WHAT ARE THE SYMPTOMS OF AN ANAL FISSURE?
Common symptoms include rectal pain that may be worse with bowel movements, rectal burning, bleeding (especially blood on the tissue paper), itching and/or a tearing sensation. HOW IS AN ANAL FISSURE DIAGNOSED?
Fissures can often be identified through anal inspection and digital rectal examination. The anal examination includes the use of an anoscope to directly visualize the fissure. HOW IS A FISSURE USUALLY MANAGED OR TREATED?

    ANAL FISSURE BOTOX
    Updated October 2001
    Introduction
An anal fissure, or split in the skin of the distal anal canal, is a common problem that causes substantial morbidity in people who are otherwise healthy. The incidence of anal fissure is similar in men and women. The majority of fissures occur in the posterior midline of the anal canal and are usually the result of tearing of the skin from hard stool or other physical irritants. Multiple fissures or lateral fissures may have other causes, such as Crohn's disease, ulcerative colitis, tuberculosis, infection with human immunodeficiency virus (HIV) or syphilis. Spasm of the anal sphincter has been noted in association with anal fissure, and for many years treatment has focused on alleviating spasm of the sphincter. Since 1951, the most common treatment for chronic anal fissure in the United States and Europe has been lateral internal sphincterotomy. This involves cutting the internal sphincter on the right or left mid-lateral area to break the circular anatomy of the muscle permanently. Although the technique is simple and effective, the fundamental drawback of this surgery is its potential to cause minor but sometimes permanent alterations in the control of gas, mucus and, occasionally, stool. This can sometimes become a major problem, particularly in elderly patients or those with diarrhea, irritable bowel syndrome, diabetes or recurrent fissure after previous surgery.

ANAL FISSURE WHAT IS AN ANAL FISSURE? An anal fissure is a small tear in the lining of the anus. It can cause pain, bleeding and/or itching.
WHAT CAUSES AN ANAL FISSURE? It is usually caused by a hard and dry bowel movement which tears the anal lining. Other causes include diarrhoea and inflammation of the anorectal area. HOW CAN A FISSURE BE TREATED? At least 50 percent of fissures heal by themselves without need for an operation. Application of special medicated cream, use of stool softeners, avoidance of constipation, and the use of sitz baths (soaking the anal area in plain warm water for 20 minutes, several times a day) help to relieve the symptoms and allow healing to occur. WHAT IF A FISSURE DOES NOT HEAL? A fissure that does not heal should be reexamined to determine if an underlying problem that prevents healing exists. This could be due to scarring or muscle spasm of the internal anal sphincter muscle. Those which continue to cause pain and/or bleeding can be corrected by surgery. WHAT HAPPENS AT SURGERY?

    Definition
An anal fissure is a painful cut in the lining of the anus. The anus is the opening through which stool passes from the body. Tears generally occur just inside the opening. Anal Fissure and Anal Fistula www.nucleusinc.com
    Causes
A large, dry, or hard stool can tear the lining of the anus. Frequent diarrhea or irritation of the anus and rectum may also lead to a tear.
    Risk Factors
A risk factor is something that increases your chance of getting a disease or condition.
    • Constipation Straining to pass a bowel movement Prior anal surgery, which can cause scarring and decrease the tissue's elasticity Chronic diarrhea
    Symptoms
Symptoms of an anal fissure are similar to symptoms of other more serious conditions. Therefore, you should see a doctor if you have these symptoms. Symptoms include:
    • Pain during and after a bowel movement Burning sensation during a bowel movement Bleeding with bowel movements Bright red blood either on the toilet tissue or in the bowl Itching of the anus Small amount of mucus on the stool
    Diagnosis
The doctor will ask about your symptoms and medical history, and perform a physical exam. The anal area will also be examined, which involves the following:

Nelson R ABSTRACT Order full review View and/or submit comments What's new in this issue Search abstracts ... Browse by Review Group A substantive amendment to this systematic review was last made on 03 July 2003. Cochrane reviews are regularly checked and updated if necessary. Background: Because of the disability associated with surgery for anal fissure and the risk of incontinence, medical alternatives for surgery have been sought. Most recently, pharmacologic methods that relax the anal smooth muscle, to accomplish reversibly what occurs in surgery, have been used to obtain fissure healing. Objectives: To assess the efficacy and morbidity of various medical therapies for anal fissure. Search strategy: Search terms include "anal fissure randomized". Selection criteria: Studies in which participants were randomized to a non-surgical therapy for anal fissure. Comparison groups may include an operative procedure, an alternate medical therapy or placebo. Chronic fissure, acute fissure and fissure in children are included in the review. Atypical fissures associated with inflammatory bowel disease or cancer or anal infection are excluded. Data collection and analysis: Data were abstracted from published reports and meeting abstracts, assessing method of randomization, blinding, "intention to treat" and drop-outs, therapies, supportive measures (applied to both groups), dosing and frequency and cross-overs. Dichotomous outcome measures included Non-healing of the fissure (a combination of persistence and recurrence), and Adverse events (including incontinence, headache, infection, anaphylaxis). Continuous outcome measures included measures of pain relief and anorectal manometry.

Anal Fissure, Abscess and Fistula The intestinal tract (or bowel) ends with the rectum. The last part of the rectum is a section about 1 1/2 inches long, known as the anal canal. It ends with the anus the opening to the outside of the body. There are several common problems, including hemorrhoids, that can occur in the area from the rectum to the anus. While almost everyone has heard of hemorrhoids, the other conditions are not so well known. Anal Fissure
Most people have experienced a tear or fissure at the corner of the mouth that can occur in cold weather or when yawning. Similarly, an anal fissure is a small tear in the lining of the anus, frequently caused by constipation. A hard, dry bowel movement results in a break in the tissue. However, fissures can also occur with severe bouts of diarrhea or inflammation. This results in the anus becoming dry and irritated, causing it to tear. Injury to the anal area during childbirth and abuse of laxatives may be other causes. A fissure can be quite painful during and immediately following bowel movements. This is because the anus and anal canal are ringed with muscles to control the passage of stool and to keep the anus tightly closed at other times. When those muscles expand, it stretches the fissure open. There may also be bleeding or itching with an anal fissure.

    WHAT YOU SHOULD KNOW
An anal fissure is a tear in the lining of the anus. The problem can happen to anyone, at any age, including infancy. Most fissures heal within 4 to 6 weeks on their own.
    Causes
The fissures are usually caused by very large or hard bowel movements.
    Signs/Symptoms
The chief symptoms are pain and bleeding during bowel movements. The pain may last for hours afterwards, then go away until the next bowel movement. Because of this pain, children may try to ""hold it in,'' rather than go to the bathroom. You may see bloody streaks on toilet paper, underwear, or diapers. There may be itching around the anus.
    Care
There are a number of steps you take at home to help the fissure heal. You may need surgery if it fails to heal.
    Risks
Without treatment, you could end up with permanent scarring that may cause pain and bleeding whenever you move your bowels.
