What is an anal fissure?
An anal fissure is a small tear or break in the skin at the outer tip of the anal canal. The anus is the opening through which bowel movements pass. Anal fissures are a common disorder, particularly in persons with irregular or hard bowel movements.
How does it occur?
Tears in the anal skin occur during bowel movements when hard, dry stools are passed. They may also be associated with:
  • straining at bowel movements when you are constipated
  • stress or tension
  • waiting too long to have a bowel movement (BM)
  • constipation after surgery

Anal Fissure
What is an anal fissure?
An anal fissure is a shallow tear or crack in the skin at the opening of the anus. More than 90% of children with blood in their stools have an anal fissure. You may notice the following symptoms:
  • The blood in the stool is bright red. The blood is only a few streaks or flecks. The blood is on the surface of the bowel movement (BM) or on the toilet tissue after wiping. Your child usually passes a large or hard bowel movement just before you notice the blood. You may see a shallow tear at the opening of the anus when the buttocks are spread apart, usually in a clock position of 6 or 12 o'clock. However, a tear cannot always be seen.

Anal Fissure www.Studenthealth.co.uk This is a very common condition, and is a small tear in the lining of the anal canal, which can cause pain, bleeding and itching. It is commonly caused by constipation, but often occurs after a bout of diarrhoea or for no obvious reason. Rarely anal fissures can be associated with more serious inflammatory bowel conditions. Probably about half of fissures will heal with little difficulty. Taking Ispaghula husk, which is a bulk forming laxative may aid the healing process, particularly if your diet is low in fibre. An anaesthetic cream is sometimes prescribed if there is a lot of pain, but sensitivity to these preparations may occur. Chronic fissures (a fissure that has not healed in about 6 weeks) may require a more active intervention. Traditionally this involves surgery usually with some form of stretching of the anal sphincter. More recently, Glyceryl trinitrate ointment (GTN) has been used with reasonable success. This is an unlicensed treatment in the UK, and you will need to discuss it's use with a medical practitioner.

They affect children and adults, males and females, gay and straights alike. There seem to be several causes for these fissures. Besides a traumatic bowel movement trauma from anal penetration is often common in gay men. The trauma does not necessarily have to be caused by a penis but by any blunt invasion of the anal cavity opening which can include sex toys. If caused by sexual penetration the fissures may occur almost anywhere unlike the fissures caused by traumatic bowel movement which are most often found in the posterior midline section. Infants or young children seem also to be at risk for anal fissures, sometimes in an acute form.

Anal Fissure
An anal fissure is an ulcer in the anal canal. It can vary from a small split in the lining of the anus to a deep ulcer down to the anal sphincters. There may be a small skin tag associated with it and this is called a 'sentinel pile'. It is nothing to do with haemorrhoids.
Symptoms
Anal fissures can occur at any age, but are more common in the young and in women. Its precise cause is not known, but it may be associated with constipation or the passage of a hard large stool, which tears the anal lining. This then causes pain, which is worse on opening the bowels. Therefore the affected person avoids defaecation as long as possible, becomes constipated and eventually passes a hard large stool that re-tears the anal lining. Hence a vicious circle is set up. It may occur as a complication of large piles, anal polyps, labour (causing problems after giving birth), and in those who practice anal intercourse.
Some fissures are asymptomatic (ie cause no symptoms), but most patients experience severe pain on opening the bowels, which can last up to an hour afterwards. There may be associated bleeding, which is usually bright red and is on the stool or on the toilet paper.

    Either extreme constipation or diarrhea , usually combined with nervous tension over a prolonged period of time, may produce anal abrasions, simple slit-like fissures, or acute ulcers at the anal verge. With constipation, this condition is usually caused by the passage of a hard dry stool that tears the anal lining upon deification.     Q: Anal fissures I have an embarrassing problem, an anal fissure. What can I do about it? Paul Dr Rob Hicks responds An anal fissure is a tear in the delicate lining of the anus. They're relatively common and are most often caused by a hard, dry stool as it's being passed. Diarrhoea and any inflammation in the same area can cause the lining to tear as well. With an anal fissure, passing bowel motions can be very uncomfortable. Bright red fresh blood will often appear in the toilet bowl or on the toilet tissue when you clean yourself, and gives people a real fright. The pain then eases off until the next time you open your bowels when back it comes again. For the pain, an anaesthetic cream or gel (available from the pharmacist) works well. Taking a Sitz bath is also recommended if you have the time. This involves sitting in a bath of warm water that covers the hips and buttocks a few times a day. To protect the fissure from the outside forces of clothes rubbing against it, use a barrier cream such as zinc and castor oil to protect it. Plenty of fibre from fruit, vegetables, wholemeal bread, and breakfast cereals; dried figs and prunes; and at least eight glasses of water a day will keep the bowel motions soft so that further damage doesn't occur. Whilst you're at the pharmacist, you may wish to get a gentle stool softening liquid.

    Hydrocortisone is not normally effective for anal fissures.
    Topical nitroglycerin increases local blood flow and reduces pressure in the internal anal sphincter, which may further facilitate healing. In studies, the median time of healing was 6 weeks.
    Another form of therapy is botulinum toxin. Injection of botulinum toxin into the anal sphincter can improve healing in patients with chronic anal fissure. Time to healing in these studies were approximately 2 months. The ability of oral nifedipine to reduce resting internal anal sphincter pressure prompted its use for the treatment of patients with chronic anal fissures. Treatment was associated with an initial reduction in maximum anal resting pressure and pain scores. Time to healing in these studies were approximately 2 months. If these medical therapies do not help, then the next option would be surgical. In trials, surgery has proven to be more effective than medical therapy.

    Anal fissures can present with burning rectal pain and a small amount of bleeding. Given your age, this diagnosis is very likely. However, to allay your concerns and to exclude other cponditions that can present this way, you should have a flexible sigmoidoscopy [examination with a short telescope].
