What is A-1ATD?
Alpha-1 antitrypsin is a protein that is made in the liver. The liver releases this protein into the bloodstream. Alpha-1 antitrypsin protects the lungs so they can work normally. Without enough alpha-1 antitrypsin, the lungs can be damaged, and this damage may make breathing difficult. In addition, liver damage (hepatitis, cirrhosis) can occur in both children and adults. Alpha-1 antitrypsin deficiency is an inherited (passed down from parents) disorder that causes low levels of, or no alpha-1 antitrypsin in the blood. How do normal lungs work?
Air usually enters the nose and mouth and goes down the air tube (trachea) to two main air passages (bronchi). These passages allow air to go into the right and left lung.

Alpha1-Antitrypsin Deficiency of Inherited Emphysema
Chronic obstructive pulmonary disease or COPD for short, is a lung disease that affects millions of people each year. COPD is a general term used to describe diseases such as emphysema and chronic bronchitis. In emphysema there is damage to the walls of the air sacs (alveoli) in the lungs.
What is Inherited Emphysema?
Most cases of emphysema are caused by smoking or other environmental factors. However, in a small number of cases of emphysema, there is a hereditary basis for the disease. The earliest symptom of emphysema is shortness of breath during activity. Later the shortness of breath occurs with rest also. The inherited form of emphysema is called alpha1 antitrypsin deficiency or "alpha - one" for short. People with this disease don't have enough of a major protein in the blood. This protein is alpha1 antitrypsin. Although there are many different genetic variations of this protein, only some will cause lung disease. It is more often seen in people of northern European descent, especially Scandinavians.

The Cleveland Clinic Foundation There are many inherited metabolic diseases that may have a pathologic impact on the liver. In many cases the liver component of these diseases is only an epiphenomenon of a more generalized systemic disorder. Examples of such epiphenomena are glycogen and lipid storage diseases in which hepatomegaly is a manifestation of the underlying metabolic defect although the liver is not necessarily the major target organ. However, there are three genetically determined diseases in which the liver may be the principal target organ, with manifestations of acute, subacute, or chronic disease that may become evident in early or later life. Normally, alpha 1-antitrypsin is produced in the liver and exists in levels of 1.5-3.5 gram litre . When the levels are reduced (40-60%, in the PiSS, PiMZ and PiSZ phenotypes ), most people will only suffer symptoms if they smoke , as the levels are still sufficient to counteract "normal" elastase activity in inflammation . Only in the PiZZ phenotype, when the levels are less than 15%, emphysema develops at a young age, and 50% will develop liver cirrhosis due to the accumulated protein , which is not secreted properly.
