Click here to view next page of this article AIDS Dementia Complex HIV/CNS infections: it appears that infections of the nervous system - we see this particularly in the spinal fluid but probably the same thing is true in the brain - that it’s an intrinsic part of the ecology of HIV. One of the things that is puzzling about that is that early infection for a protracted period of time, despite this infection or exposure to the nervous system, it is entirely asymptomatic, even though we might find abnormalities in the spinal fluid. Only under certain conditions late in AIDS dementia complex. There are two variants that we encompass within the definition of AIDS dementia complex, and sometimes they are separated out. One is the dementia or the brain problem, the cerebral problem, which is a combination really of cognitive Myelopathy. This syndrome is a coherent syndrome that has a hierarchy of symptoms and signs, although with some variability. Early on patients, the major problem is slowing of both cognitive and motor spheres. So patients complain of some difficulty with concentration, focusing on things and so forth, and they may also have mild motor slowing. Later on, that is those that progress - and they don’t all progress - other domains are involved cognitively. Then there’s a sub-group within this pathology of patients who have true HIV infection or HIV encephalitis. In this case marked by macrophage infiltration and you probably can’t see but a number of these macrophages are multinucleated and that’s the so-called multinucleated-cell encephalitis, which is true HIV encephalitis. These are HIV-infected macrophages. The major cell that is productively infected in the brain, which has been known for some time, is the macrophage or microglial cell. Subsequently more recently it has been shown that other cells can be infected.

    by Robin Brett Parnes, MS, MPH Dementia is a brain disorder characterized by a general loss of intellectual abilities involving impairment of memory, judgment, and abstract thinking as well as changes in personality. AIDS Dementia Complex (also known as AIDS Related Dementia and HIV Associated Dementia) is a condition in people with AIDS that results in the loss of cognitive capacity, affecting the ability to function in a social or occupational setting. Prior to effective antiretroviral therapy, AIDS Dementia Complex occurred in more than 60% of patients who developed AIDS. With the use of combination highly active antiretroviral therapy (HAART), the incidence has declined to about 10% to 15%.
    What is AIDS Dementia Complex?
    AIDS Dementia Complex (ADC) is one of the most common and clinically important nervous system complications of late stage HIV infection . It is a serious complication and left untreated can progress to a fatal outcome. There is no way to know how quickly the disease will progress, however, since the rate varies greatly from person to person. ADC is marked by severe changes in three areas:

Decreased serum vitamin B12 levels occur in up to 20% of patients with acquired immune deficiency syndrome (AIDS) and may adversely contribute to the haematologic and neurologic dysfunction which is frequently attributed to the human immunodeficiency virus. We describe a patient with AIDS who presented with an apparent advanced AIDS dementia complex. There was an associated low serum vitamin B12 resulting from malabsorption due to low gastric intrinsic factor secretion. Following treatment with vitamin B12 the symptoms resolved over a 2-month period. We believe that the AIDS dementia complex represented a reversible adverse synergistic interaction between the human immunodeficiency virus and vitamin B12 deficiency.     Aids Dementia Complex (Adc):
    (HIV-associated dementia or HAD.) A degenerative neurological condition attributed to HIV infection, characterized by a group of clinical presentations including loss of coordination, mood swings, loss of inhibitions, and widespread cognitive dysfunction. It is the most common central nervous system complication of HIV infection. Characteristically, it manifests itself after the patient develops major opportunistic infections or AIDS-related cancers. However, patients can also have this syndrome before these major systemic complications occur. The cause of ADC has not been determined exactly, but it may result from HIV infection of cells or inflammatory reactions to such infections.

    AIDS Dementia Complex AIDS dementia complex (ADC) is one of the most frequent and serious neurologic complications of HIV. It is characterized by cognitive dysfunction (trouble with concentration, memory and attention), declining motor performance (strength, dexterity, coordination) and behavioral changes. It occurs primarily in more advanced HIV infection when the CD4 cell counts are relatively low. While the progression of dysfunction is variable, it is regarded as a serious complication historically predicting death in less than one year. Diagnosis is made by neurologists who carefully rule out alternative diagnoses. This routinely requires a careful neurological examination, brain scan (MR or CT) and a lumbar puncture to evaluate the cerebrospinal fluid. No single test is available to confirm the diagnosis, but the constellation of history, laboratory findings, and examination reliably establish the diagnosis when performed by experienced clinicians. Current experimental studies available at some centers include: In all cases, it remains important to seek a maximally effective HIV treatment regimen when dementia develops. Efforts to bring viral load to the lowest possible level in serum (and probably in CSF) should be primary goal for all treating physicians. Antivirals with good CNS penetration including zidovudine, D4T, nevirapine, and abacavir may be used preferentially if they have not already failed.

    by Mark Bowers Dementia and its Symptoms The term dementia is used by neurologists to describe a clinical syndrome composed of memory loss, decreased mental concentration and the loss of other intellectual functions because of progressive disease in the brain. There may be motor signs, such as weakness, lack of coordination and unsteady gait. Behavioral changes are most likely to be noticed by friends or family members, and may include apathy, personality changes and loss of libido. ADC (AIDS Dementia Complex) is often characterized by depression, but depression alone is not enough to establish a diagnosis of ADC. Depression has a profound effect on survival and should be treated, whether it appears as part of the of symptoms called ADC or alone. The clinical course of ADC is variable. Some people experience only mild symptoms, while others progress rapidly. A scale that allows clinicians to stage ADC is provided below. The first symptoms to appear are often the most difficult to quantify: short-term memory loss and poor concentration may be attributed to stress, to prescription or recreational drug use, or to fatigue, but may also be early manifestations of ADC.     Results of a number of clinical studies show that HAART has significantly slowed the progression of HIV disease. HAART has also produced significant declines in the incidence of HIV-related opportunistic infections. Finally, HAART has significantly increased survival rates in people with AIDS. Despite the impact of HAART in these areas, there are few data available concerning the natural history of AIDS dementia complex (ADC) in the era of HAART. Although incidence of ADC has declined since HAART became widely available, this decrease could be less significant than for other AIDS-associated OI and even could lead to an increased incidence of ADC. The current study, conducted in multiple centers in Australia, evaluates the impact of HAART on survival following ADC and the prevalence of ADC at a population level through examination of routine AIDS surveillance data in Australia in the period 1993-2000.
    Australian AIDS notification data in the period 1993-2000 were examined. In order to examine the impact of HAART, two periods of AIDS diagnoses were chosen: pre-HAART (1993-1995) and HAART (1996-2000). AIDS Dementia Complex is a term used to describe cognitive, motor and behavioral neurological impairments that are unrelated to opportunistic infections. These impairments significantly impair activities of daily living, resulting in decreased independence. A AIDS - Advanced HIV infection D Dementia - Cognitive impairment worsening during late-stage HIV infection C Complex - A condition consisting of neuropsychologic disturbances in the areas of cognition, motor function, and behavior In the central nervous system, HIV penetrates the blood-brain barrier, infecting CD4 + cells in the brain (Kalichman, 1998). HIV dementia is a subcortical dementia (Gabrieli, 1995), affecting structures beneath the cerebral cortex. Because the basal ganglia are affected, HIV-associated cognitive disorders (including dementia) share many features with diseases such as Parkinson's disease and Huntington disease. People with damage or dysfunction in the basal ganglia have slower thought processes (bradyphrenia), and move more slowly (bradykinesia). They are forgetful, but may often recall information if given enough time. Diagnosis is difficult, as symptoms may vary. A thorough evaluation is necessary to determine the cause of cognitive impairment and appropriate treatment. Procedures used in the diagnostic process include: neuropsychological testing; mental status examination; neuropsychiatric interview; neurologic examination; and radiologic and laboratory testing.
