    Adrenoleukodystrophy is a rare genetic disease characterized by a loss of myelin surrounding nerve cells in the brain and progressive adrenal gland dysfunction. Description Adrenoleukodystrophy (ALD) is a member of a group of diseases, leukodystrophies, that cause damage to the myelin sheath of nerve cells. Approximately one in 100,000 people is affected by ALD. There are three basic forms of ALD: childhood, adult-onset, and neonatal. The childhood form of the disease is the classical form and is the most severe. Childhood ALD is progressive and usually leads to total disability or death . It affects only boys because the genetic defect is sex-linked (carried on the X chromosome). Onset usually occurs between ages four and ten and can include many different symptoms, not all of which appear together. The most common symptoms are behavioral problems and poor memory. Other symptoms frequently seen are loss of vision, seizures, poorly articulated speech, difficulty swallowing, deafness, problems with gait and coordination, fatigue , increased skin pigmentation, and progressive dementia The adult-onset form of the disease, also called adrenomyeloneuropathy, is milder, progresses slowly, is usually associated with a normal life span, and usually appears between ages 21-35.     What Is Adrenoleukodystrophy (ALD)?

    ALD is a genetic disorder that affects the adrenal gland and the white matter of the nervous system. How does it work?
    In this disease, saturated very long chain fatty acids accumulate in the brain and the cerebral cortex because the enzyme responsible for breaking them down does not work properly. The very long chain fatty acids (VLCFA) cause a demyelination of nerves in the brain and/or the spinal cord. When the nerve fibers lose their myelin sheath, the nerve short-circuits. This leads to a degeneration of the adrenal gland, which is controlled by the nervous system. The adrenal gland secretes hormones that are essential for life so this disease can cause serious side effects and death. Who does it affect?
    Adrenoleukodystrophy is an X-linked disease so it is carried by the mother and affects only males. About one in every 20,000 males are victims to this disease. Symptoms
    Symptoms typically begin to emerge between the ages of 3 and 16 years.

    Adrenoleukodystrophy is one of the genetic neurodegenerative disorders . These disorders are grouped into four categories: sphingolipidoses, neuronal ceroid lipofuscinoses, adrenoleukodystrophy (like Lorenzo, whose story is portrayed in the movie “Lorenzo’s Oil”), and sialidosis. These may be distinguished from one another based on head CT, head MRI, nerve conduction velocities, visual evoked potentials, auditory evoked potentials, electroretinography, and to a lesser extent, EEG's.

    Causes, incidence, and risk factors: Adrenoleukodystrophy is transmitted as an X-linked trait (the neonatal form is by autosomal recessive transmission). Its incidence is estimated at 1 out of 20,000 to 1 in 50,000 and affects all races. The metabolic defect is the accumulation of long chain fatty acids in the nervous system, adrenal gland, and testes , where the accumulated material disrupts normal activity. There are several (seven recognized) different forms of the disease. The neonatal form appears shortly after birth and includes seizures and delayed neurological development with death occurring in infancy or young childhood. The childhood cerebral form (around 4-8 years) appears in mid-childhood, and the other forms appear during

Other characters Adrenoleukodystrophy, (also called X-linked adrenoleukodystrophy (XALD)), a peroxisomal disorder of the lipidosis group characterized by accumulation of very long chain fatty acids (VLCFA) in the white matter, peripheral nerves, adrenal cortex and testis. Clinically the disease has a wide phenotype variability, with childhood, adolescent, adult and adrenomyeloneuropathy forms. Symptoms consist of adrenal insufficiency and neurological deterioration, with gait disturbances, loss of vision, spastic diplegia, epileptic seizures and progressive dementia. The MR pattern is quite characteristic ( Fig.1 ); in T2-weighted images white matter hyperintensity is seen involving the occipital area and splenium of the corpus callosum. A rim of enhancement can be seen following gadolinium injection. The lesions are symmetrical and demyelination progresses anteriorly. Late in the disease the brain stem and ultimately the cerebellum may be involved.

It is a peroxisomal disorder, resulting in hyperpigmentation, adrenocortical insufficiency, elevated ACTH, and accumulation of very long chain fatty acids. Clinically it is characterized by impaired mental intellect, behavioural problems, cortical blindness and deafness, ataxia, spasticity and motor deficits. Onset is usually before 10 years of age, with rapid progression to death over 23 years. Pathologically, adrenoleukodystrophy is characterized by specific diagnostic inclusion bodies in cerebral macrophages Schwann cells, adrenocortical cells and Leydig cells of the testis. The inclusion bodies are seen at electron microscopy, and consist of dense long thin leaflets enclosing an electron-lucent space. Cross-sectional imaging of the adrenals is usually unremarkable.     What is Adrenoleukodystrophy?
    ALD is the acronym for a rare genetic disease called adrenoleukodysrophy . Due to its unique pattern of inheritance, and the fact that any individual may undergo random gene mutations, this horrible disease can make its devastating and often lethal appearance without any warning. ALD does not recognize racial, social, economic, or religious boundaries. It does, however, manifest preferentially in males. It is commonly known, especially to Americans, as the Lorenzo's Oil disease, named after an Oscar nominated 1993 film, starring Susan Sarandon and Nick Nolte, which was based on one family's experience. ALD affects boys and young men. In the childhood presentation, which is observed in 35% of ALD-positive individuals, it is usually diagnosed between the ages of four and ten years old. They generally have a history of learning disabilities and/or a behavior issue such as attention deficit disorder (ADD) or attention deficit hyperactivity disorder (ADHD) while in fact, insulating material in the boys' brains, the white matter (also known as myelin) is progressively being destroyed. The disease worsens over several years, and usually in less than two years from ALD diagnosis, the child will lose all cognitive, mental, and physical functions, and deteriorate into a vegetative state leading to death.
