Adjustment Disorder
The development of emotional or behavioral symptoms in response to an identifiable stressor occurring within 3 months of the onset of the stressor.
Symptoms:
Depressed mood, anxious, regularly acting out of the ordinary. May be experiencing sleep and sexual difficulties. The symptoms would be significant if: the noted distress is in excess of what would normally be expected from exposure to the stressor; or there's significant impairment in social or job functioning; and the stress-related disturbance is not the result of another clinical disorder or a result of Mental Retardation or Personality Disorder and the person is not experiencing bereavement. SASOP ADJUSTMENT DISORDER Presenting complaints Patients feel overwhelmed or unable to cope. There may be stress-related physical symptoms such as insomnia, headache, abdominal pain, chest pain, palpitations. Diagnostic features Acute reaction to recent stressful or traumatic event. Extreme distress resulting from a recent event, or preoccupation with the event. Symptoms may be primarily somatic. Other symptoms may include: low or sad mood, anxiety, worry, feeling unable to cope. Acute reaction usually lasts from a few days to several weeks. Differential diagnosis If dissociative symptoms (sudden onset of unusual or dramatic somatic symptoms) are present, see Dissociative (conversion) disorder - F45. Acute symptoms may persist or evolve over time. If significant symptoms persist longer than one month, consider an alternative diagnosis: if significant symptoms of depression persist, see Depression - F32 if significant symptoms of anxiety persist, see Genralized anxiety - F41.1 if stress-related somatic symptoms persist, see Unexplained somatic complaints -F45.

    A Diagnostic Dilemma Frequently overlooked in evaluating children for a behavior problem is a common condition termed Adjustment Disorder. According to the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV) the essential feature is the development of clinically significant emotional or behavioral symptoms by an individual in response to an identifiable psychosocial stressor. The manual published by the American Psychiatric Association states that symptoms must develop within three months of the onset of the stressor causing the symptoms and must resolve within six months after the termination of the stressor. The manual also instructs that some symptoms may be prolonged if the stressor is chronic or has protracted consequences. For example, continuous academic underachievement in a non-empathetic school or home environment. An Adjustment Disorder is divided into subtypes that best characterize the predominant symptoms present in an individual with the disorder. Subtypes are listed as follows: Adjustment Disorder with Depressed Mood
    Adjustment Disorder with Anxiety and Depressed Mood
    Adjustment Disorder with Conduct Problems
    Adjustment Disorder with Emotional and Conduct Problems
    Adjustment Disorder of an Unspecified Type
    Symptoms are usually associated with reduction in productivity, physical complaints and social maladaptation. The duration of an Adjustment Disorder is designated as acute if the symptoms are present for less than 6 months and chronic if for more than 6 months.

    Adjustment disorder with depressed mood is a mild, reactive, depression which last only a few months. The disorder effects an equal number of males and females. The symptoms of adjustment disorder with depressed mood are the same as those experienced with other clinical depressions....sadness, emptiness, loss of interest and pleasure, irritability and anger, changes in appetite, sleep problems, restlessness, slow movement and thinking, fatigue, worthlessness and guilt, poor concentration, thoughts about death and suicide. A differences between adjustment disorder with depressed mood and other depressive disorders is that the adjustment disorder symptoms begin following a specific stressor. The disorder occurs in response to some specific stressful situation or circumstance. The stressor which causes the problem may be almost anything. Examples include marital conflict, divorce, financial problems, loss of a job, conflicts in a close friendship, having to move, having a close friend or family member diagnosed with a serious disease, being a victim of crime, experiencing a natural disaster...The list could go on and on. Although almost anyone would be "stressed out" by events such as these, the person experiencing an adjustment disorder with depressed mood has more symptoms of depression than others who have experienced the same stressor. They overreact.

    Disease - Adjustment disorder
    Definition: Adjustment disorder is an abnormal and excessive reaction to a life stressor, such as starting school, getting divorced, or grief. Causes And Risk: Symptoms of adjustment disorder typically begin within three months of the identifiable stressor and usually do not last longer than six months. Adults often develop adjustment disorder related to marital or financial problems. In adolescents, common stressors include school problems, family conflict, or sexuality issues. Other stressors for people of any age include the death of a loved one, general life changes, or unexpected catastrophes. Prevention: There is no known way to prevent this disorder. Strong support from friends and family can help. Symptoms: Signs And Tests: The following criteria need to be met to establish a diagnosis:
    • The person must have a psychological evaluation The symptoms clearly follow a psychosocial stressor The symptoms are more severe than would be expected There do not appear to be other underlying disorders
    Treatment: The primary goal of treatment is to relieve symptoms and help the person return to the level of functioning the person had before the stressful event.
