      Overview
      Studies examining the prevalence of mental disorders in cancer show that most cancer patients do not meet the diagnostic criteria for any specific mental disorder; however, many patients do experience a variety of difficult emotional responses. In order to effectively match patient needs and treatment interventions, health care professionals must be able to distinguish the periodic difficulties that characterize normal adjustment from more serious mental disorders. Psychosocial distress exists on a continuum (Figure 1) ranging from normal adjustment issues through the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) Adjustment Disorders; to a level close to, but below, the threshold (i.e., meets some diagnostic criteria but not all) of diagnosable mental disorders; to syndromes that meet the full diagnostic criteria for a mental disorder (e.g., major depressive disorder). This summary focuses primarily on the less severe end of this continuum: the normal adjustment issues, psychosocial distress, and the adjustment disorders. (Refer to the PDQ summaries on

    Adjustment disorders are the result of identifiable stress situations that cause emotional or behavioral symptoms to develop within three months of the onset of the situation. They can be a reaction to life stresses such as divorce, death, job loss, moving, health problems, etc.
    Signs and Symptoms
    When these stressful experiences cause reactions of marked distress, which appears to be in excess of what would be expected, an adjustment disorder is likely. When the stress results in a substantial impact on an individual´s ability to function in a social, occupational or educational setting, this also is a signal. Since this is usually a short-term disorder, once the stressful situation (or its consequences) has ended, the symptoms should disappear within six months.

    Disease - Adjustment disorder
    Definition: Adjustment disorder is an abnormal and excessive reaction to a life stressor, such as starting school, getting divorced, or grief. Causes And Risk: Symptoms of adjustment disorder typically begin within three months of the identifiable stressor and usually do not last longer than six months. Adults often develop adjustment disorder related to marital or financial problems. In adolescents, common stressors include school problems, family conflict, or sexuality issues. Other stressors for people of any age include the death of a loved one, general life changes, or unexpected catastrophes. Prevention: There is no known way to prevent this disorder. Strong support from friends and family can help. Symptoms: Signs And Tests: The following criteria need to be met to establish a diagnosis:
    • The person must have a psychological evaluation The symptoms clearly follow a psychosocial stressor The symptoms are more severe than would be expected There do not appear to be other underlying disorders
    Treatment: The primary goal of treatment is to relieve symptoms and help the person return to the level of functioning the person had before the stressful event.
