    What is an adjustment disorder?
An adjustment disorder is defined as an emotional or behavioral reaction to an identifiable stressful event or change in a person's life that is considered maladaptive or somehow not an expected healthy response to the event or change. The reaction must occur within three months of the identified stressful event or change happening. The identifiable stressful event or change in the life of a child or adolescent may be a family move, parental divorce or separation, the loss of a pet, birth of a brother or sister, to name a few.
    What causes adjustment disorders?
Adjustment disorders are a reaction to stress. There is not a single direct cause between the stressful event and the reaction. Children and adolescents vary in their temperament, past experiences, vulnerability, and coping skills. Their developmental stage and the capacity of their support system to meet their specific needs related to the stress are factors that may contribute to their reaction to a particular stress. Stressors also vary in duration, intensity, and effect. No evidence is available to suggest a specific biological factor that causes adjustment disorders.

    Adjustment disorders
Adjustment disorders are maladpative, or unhealthy, responses to stressful or psychologically distressing life events, such as the end of a romantic relationship or being terminated from a job. The American Psychiatric Association has identified and categorized several varieties of adjustment disorders, depending on accompanying symptoms and their duration. These subtypes include adjustment disorder with depressed mood, with anxiety, with anxiety and depressed mood, and with disturbances of conduct. The disorders can additionally be classified as acute or chronic. It is thought that adjustment disorders are fairly common; recent figures estimate that 5 to 20 percent of persons seeking outpatient psychological treatment suffer from one of these disorders. Psychiatrists rigidly define the time frames in which these disorders can occur to differentiate them from other types of responses to stressful events, such as post-traumatic stress disorder and acute stress disorder. Adjustment disorders must occur within three months of the stressful event and can, by definition, last no longer than six months. Symptoms of these various adjustment disorders include a decrease in performance at work or school, and withdrawal from social relationships. These disorders can lead to suicide or suicidal thinking and can complicate the course of other diseases when, for instance, a sufferer loses interest in taking medication as prescribed or adhering to difficult diets or exercise regimens.

    Adjustment Disorders
Adjustment disorders are maladpative responses to stressful or psychologically distressing life events, such as being placed in day care for the first time, being injured in an accident, or experiencing a natural disaster. The diagnosis of an adjustment disorder requires that the inappropriate emotional or behavioral symptoms develop within three months of the stressful event or occurrence. The American Psychological Association (APA) has identified and catagorized several varieties of adjustment disorders, depending on accompanying symptoms and their duration. These subtypes and their related symptoms are as follows: 1.) adjustment disorder with depressed mood, marked by tearfulness and feelings of intense discouragement; 2.) adjustment disorder with

Dr. Adams consults to occupational medicine, surgeons, nurse case managers, insurers and employers regarding the psychological impact of work-related injury and the role of psychological factors in short- and long-term disability. ADJUSTMENT DISORDERS
Adjustment Disorders arise in response to one or more stressors that occur within three months of the onset of the patient's symptoms. The symptoms can be emotional and/or behavioral. If the symptoms last longer than six months due to an enduring stressor (one that continues to occur or whose consequences continue to occur), they are referred to as chronic. If the symptoms last less than six months, the disorder is referred to as acute. There are adjustment disorders with anxiety, with depressed mood and adjustment disorders with a mixture of anxiety and depressed mood. With some adjustment disorders, there is a disturbance of conduct in which the patient becomes rebellious or reckless, and, obviously, some adjustment disorders are characterized by changes in both emotions and conduct.

This appeared in the May/June 2002 issue of Hearing Loss: The Journal of Self Help for Hard of Hearing People. What’s On Your Mind? By Michael A. Harvey
Our psychologist explains the diagnosis of Adjustment Disorder. Dear Dr. Harvey: I am a 49-year-old woman and have been hard of hearing all my life. A couple of months ago, I got laid off from my job where I had worked for over twenty years. It was particularly hard, since my company had been so good about accommodating my hearing loss. I was having a tough time of it, withdrew from everyone and began drinking more. When I had thoughts of crashing into a tree and ending it all, it scared me, so I saw a therapist. He diagnosed me as having an “Adjustment Disorder.” What exactly does that mean? Is this because I’m hard of hearing? Am I maladjusted? And, do hard of hearing people have that diagnosis more than people with normal hearing? BP Cleveland, Ohio Dear BP: Given the seemingly constant barrage of layoffs these days, it is easy to forget how emotionally devastating they can be. Bravo to you for going to psychotherapy! However, I’m concerned that you may be overburdened by the stigma and oversimplification of psychiatric labels. Your technical questions warrant technical answers. “Adjustment Disorder” is one of many diagnoses which are listed in a book published by the American Psychiatric Association titles

    Overview
Studies examining the prevalence of mental disorders in cancer [ ] show that most cancer patients do not meet the diagnostic criteria for any specific mental disorder; however, many patients do experience a variety of difficult emotional responses. [ ] In order to effectively match patient needs and treatment interventions, health care professionals must be able to distinguish the periodic difficulties that characterize normal adjustment from more serious mental disorders. Psychosocial distress exists on a continuum (Figure 1) ranging from normal adjustment issues through the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) Adjustment Disorders; [ ] to a level close to, but below, the threshold (i.e., meets some diagnostic criteria but not all) of diagnosable mental disorders; to syndromes that meet the full diagnostic criteria for a mental disorder (e.g., major depressive disorder). This summary focuses primarily on the less severe end of this continuum: the normal adjustment issues, psychosocial distress, [ ] and the adjustment disorders. (Refer to the PDQ summaries on
