Adjustment Disorders
SYMPTOMS
The development of emotional or behavioral symptoms in response to an identifiable stressor(s) occurring within 3 months of the onset of the stressor(s). These symptoms or behaviors are clinically significant as evidenced by either of the following:
  • marked distress that is in excess of what would be expected from exposure to the stressor
  • significant impairment in social, occupational or educational functioning
The stress-related disturbance does not meet the criteria for another specific disorder. Once the stressor (or its consequences) has terminated, the symptoms do not persist for more than an additional 6 months Treatment

What is an adjustment disorder and how does it occur?

    There are six major adjustment disorders:
  • Adjustment disorder with depressed mood;
  • Adjustment disorder with anxiety;
  • Adjustment disorder with mixed anxiety and depressed mood;
  • Adjustment disorder with disturbance of conduct;
  • Adjustment disorder with mixed disturbance of emotions and conduct;
  • Adjustment disorder unspecified.
What are the characteristics associated with an adjustment disorder?
A person with adjustment disorder often experiences feelings of depression or anxiety or combined depression and anxiety. As a result, that person may act out behaviorally against the "rules and regulations" of family, work, or society. In some people, an adjustment disorder may manifest itself in such behaviors as skipping school, unexpected fighting, recklessness, or legal problems. Other people, however, instead of acting out, may tend to withdraw socially and isolate themselves during their adjustment problems. Still others may not experience behavioral disturbances, but will begin to suffer from physical illness. If someone is already suffering from a medical illness, that condition may worsen during the time of the adjustment disorder. People in the midst of adjustment disorders often do poorly in school or at work. Very commonly they begin to have more difficulty in their close, personal relationships.

An adjustment disorder is a debilitating reaction, usually lasting less than six months, to a stressful event or situation. It is not the same thing as post-traumatic stress disorder (PTSD), which usually occurs in reaction to a life-threatening event and can be longer lasting. Description
An adjustment disorder usually begins within three months of a stressful event, and ends within six months after the stressor stops. There are many different subtypes of adjustment disorders, including adjustment disorder with:
  • depression
  • anxiety
  • mixed anxiety and depression
  • conduct disturbances
  • mixed disturbance of emotions and conduct
  • unspecified
Adjustment disorders are very common and can affect anyone, regardless of gender, age, race, or lifestyle. By definition, an adjustment disorder is short-lived, unless a person is faced with a chronic recurring crisis (such as a child who is repeatedly abused). In such cases, the adjustment disorder may last more than six months. Causes and symptoms
An adjustment disorder occurs when a person can't cope with a stressful event and develops emotional or behavioral symptoms. The stressful event can be anything: it might be just one isolated incident, or a string of problems that wears the person down. The stress might be anything from a car accident or illness, to a divorce, or even a certain time of year (such as Christmas or summer).

Stress happens. Stress happens every day. Stress is a normal part of your life and most people can adjust to a stressful event and move on. Stress may interfere with your study, work, friendships and family. You may express heavy stress as behavioral problems, psychosomatic or physical disease symptoms, and learning disorders. Adjusting to stressful events need not involve therapeutic coaching, counseling or psychotherapy. Adjusting to stress usually involves four steps:
  • You recognize your need to avoid or reduce a stressor You search for choices or new options You search for resources that enable you to use your options You find an option that controls, removes or reduces your stressor

    Adjustment Disorder
    An adjustment disorder occurs when a person develops affective (emotional) or behavioral symptoms in response to an identifiable stressor. Stressors can be natural disasters (such as an earthquake), events or crisis (such as car accidents, or development of a medical disorder) or interpersonal problems (such as divorce, or abuse). The person displays either marked distress, or impairment in functioning (i.e. unable to work or study). Adjustment disorders, by definition, last less than 6 months (after the stressor or its consequences end). If the symptoms last more than 6 months, the person may have another disorder, such as an anxiety or mood disorder.

    About Adjustment Disorder Course/Prognosis: When it starts, How it seems to progress Adjustment disorder occurs in response to an identifiable stressor. The person's response to the stressor is either very strong and causes a great deal of distress (feeling suicidal after moving to a new town) or results in impairment in functioning in either the person's occupational/education, social or interpersonal life (i.e. missing work for a week after breaking up with a boy/girl friend). By definition the adjustment disorder ends within 6 months after the onset of the stressor (or the consequences of the stressor). For example, if a person still has severe symptoms 6 months after moving, another disorder must be considered such as a Mood, Anxiety, or Personality disorder. The exception to the 6 month criteria occurs with Chronic stressors. If, for example a child is repeatedly physically abused this would constitute a chronic stressor and the symptoms could persist as long as the abuse persists (it is also possible that the child would develop another psychiatric disorder). By definition, adjustment disorders end. People with no history of prior psychiatric symptoms, and who have stable environments and strong social support are likely to return to their premorbid (prior to the on-set of the adjustment disorder) functioning level. The majority of these people will have a very positive prognosis, and are likely to display no further psychological symptoms in the future.

    Psychotherapy
    Psychotherapy is the treatment of choice for any adjustment disorder, since the disorder is seen as usually a quite normal reaction to a specific situational event. The form and type of psychotherapy will vary upon the clinician, but as with all psychotherapy, it should occur within a supportive, non-judgmental environment that encourages the client's growth through exploration of new behaviors and ideas. This therapy often takes the form of solution-focused, to help the individual deal more effectively with the specific life problem. Often the therapist acts as a partner in therapy, helping guide the client toward finding these new coping mechanisms, or finding a better understanding of issues in their lives. Adjustment disorder, by definition, is a short-term difficulty that rarely goes beyond 6 months. Lingering feelings may occur beyond that time, but those are natural and likely not to be severe enough to require additional attention or treatment. It often helps treatment progress (and is required in many agencies) to put together a firm but realistic treatment plan, so the patient can also see the short-term nature of the therapy. Clinicians should be careful not to lapse into acting as an advice-giver to individuals who suffer from an adjustment disorder. The exact content and type of therapy used will vary widely. Treatment will often emphasize the importance of social support within the client's life, alternative activities to explore or to find meaning in, increasing a person's range and effectiveness of coping skills, learning better ways of dealing with stress, etc. If stress is an issue, therapy may also offer relaxation training and techniques and examine methods for reducing stress.

      Antianxiety Drugs
      Sometimes a few days or weeks of antianxiety medication, (e.g. lorazepam 1 mg three times a day, or clonazepam 0.5-1.5 mg at bedtime), is prescribed to control anxiety and insomnia.
      Antidepressant Or Antipsychotic Medications
      Usually antidepressant or antipsychotic medications aren't used for Adjustment Disorder. These medications should not be prescribed merely for their sedating effect, since antianxiety drugs are much better in this regard, and have far fewer side-effects. If antidepressant or antipsychotic medication is indicated, it is more likely that the diagnosis is a more serious mood or psychotic disorder, rather than just Adjustment Disorder. to top

Causes, incidence, and risk factors Symptoms of adjustment disorder typically begin within three months of the identifiable stressor and usually do not last longer than six months. Adults often develop adjustment disorder related to marital or financial problems. In adolescents, common stressors include school problems, family conflict, or sexuality issues. Other stressors for people of any age include the death of a loved one, general life changes, or unexpected catastrophes. There is no way to predict which people are likely to develop adjustment disorder, given the same stressor. Financial conditions, social support, and career and recreational opportunities can influence how well a person reacts to stress. A given person's susceptibility to stress may be influenced by such factors as social skills, intelligence, flexibility, genetic factors, and coping strategies.
