    What is acromegaly?
    The pituitary gland is a small, pea-size gland that lies at the base of the brain. One of the functions of the pituitary gland is to produce growth hormone (GH). Normal levels of GH are necessary for the body to grow and function properly. Acromegaly is a disease caused by overproduction of GH. This overproduction is usually caused by a benign (noncancerous) tumor of the pituitary gland. High levels of GH lead to excess production of another hormone called insulin-like growth factor I (IGF-I), which is produced in the liver. When IGF-I levels rise abnormally, numerous health complications can result. Back to top How is acromegaly diagnosed?
    There are several ways to diagnose acromegaly:
    • Clinical signs and symptoms during a physical exam.
    An elevated IGF-I level is the most accurate method of confirming a diagnosis of acromegaly. IGF-I represents overall GH production and activity. Because IGF-I fluctuates less than GH levels, it is a better indicator of disease condition. High IGF-I levels, above the normal range as specified by the test, often indicate acromegaly.

    Definition: Acromegaly is a chronic metabolic disorder caused by the presence of too much growth hormone. It results in gradual enlargement of body tissues including the bones of the face, jaw, hands, feet, and skull.

    Causes, incidence, and risk factors: Acromegaly occurs in about 6 of every 100,000 adults. It is caused by abnormal production of growth hormone after normal growth of the skeleton and other organs is complete. Excessive production of growth hormone in children causes gigantism rather than acromegaly. The cause of the increased hormone secretion is usually a benign tumor of the pituitary gland. The pituitary gland, which is located just below the brain, controls the production and release of several different hormones including growth hormone. There are no known risk factors for acromegaly other than a prior history of a pituitary tumor.

    Acromegaly is the Greek word for "extremities" and "enlargement." When the pituitary gland produces excess growth hormones, this results in excessive growth called acromegaly. The excessive growth occurs first in the hands and feet, as soft tissue begins to swell. Acromegaly affects mostly middle-aged adults. Untreated, the disease can lead to severe illness and death.
    What are the symptoms of acromegaly?
    Symptoms of acromegaly vary depending on how long the patient has had the disease. The following are the most common symptoms of acromegaly. However, each individual may experience symptoms differently. Symptoms may include:

    Growth Hormone Replacement Acromegaly is a disease of growth hormone (GH) hypersecretion. Usually, the source is the pituitary tumor. These are always benign (non-cancerous) but often large and invasive. GH itself does not promote growth. Instead, it induces production of yet another hormone, IGF-I or somatomedin C (SmC) in virtually all organs and tissues. High IGF-I in turn promotes somatic growth. Clinically, acromegaly is associated with increased amount of soft tissues (large puffy hands, rough facial features), bone overgrowth (protruding lower jaw, frontal bossing) tall stature (if the disease began before puberty). Other symptoms include headache, sweating, snoring, sleep apnea, carpal tunnel syndrome and joint aches. The development of the disease is insidious, and at the time of diagnosis the patient usually recalls the existence of symptoms for 5-10 years. Even family rarely notices the gradual development of the disease. Often, the diagnosis is made by a stranger or by a new physician during his/her first meeting with the patient.

    Acromegaly is an extremely rare condition caused by excessive and inappropriate production of the growth hormone (GH) in adults. When pathological GH excess occurs before epiphyseal fusion in the long bone, the condition is known as gigantism. Only three to four new cases of acromegaly per million are diagnosed each year and there are only 40-60 sufferers per million. The mean age of onset is 32, with diagnosis typically several years later, at age 39-42. An overwhelming majority of cases of acromegaly (95 per cent) are due to benign tumours of the anterior pituitary gland, also referred to as somatotroph adenomas. In 30 per cent of cases, GH excess is associated with hyperprolactinaemia, which is due either to prolactin secretion from the pituitary gland, or to compression of the pituitary stalk by a tumour, which impairs the delivery of the inhibitory neurotransmitter dopamine to the normal prolactin-producing cells.

    Causes and Risk Factors The pituitary gland controls critical body functions, including growth and body metabolism, thyroid function, the body's response to stress and reproduction. In over 90 percent of cases, acromegaly can be traced to a benign tumor of the pituitary gland, called an adenoma. Acromegaly can also be caused by tumors of the adrenal glands, lungs or pancreas. These tumors can lead to an excess of growth hormone, either because they produce it or they produce the hormone that stimulates the pituitary gland into making it. Diagnosis A magnetic resonance imaging (MRI) scan is the most sensitive test for acromegaly and can quickly diagnose even small tumors (microadenomas). Sometimes
