Acromegaly
is a serious adult condition that results when the body is exposed to high levels of growth hormone for a prolonged time. The progression of the disease is subtle and gradual, often resulting in misdiagnosis for years. When increased growth hormone secretion occurs in childhood, it is clinically referred to as “gigantism”. Besides the debilitating nature of the disease, acromegaly results in severe physical changes, and increased morbidity due to enlargement of the heart, liver, spleen, kidneys, and increased risk for cancerous colon polyps.
The increased growth hormone secretion is most often due to a benign adenoma of the pituitary gland . In rare cases, acromegaly is caused by tumors of the pancreas, lungs or

Links The pulsatile release of growth hormone (GH) by normal somatotroph cells is regulated by growth hormone releasing hormone (GHRH), which stimulates GH secretion, and somatostatin, which decreases secretion. At the liver, GH stimulates secretion of somatomedin C, also known as insulin-like growth factor 1 (IGF-1), which mediates many of the peripheral somatic effects of GH. IGF-1 feeds back at the level of the hypothalamus and pituitary resulting in a reduction in GH secretion. The diagnosis of acromegaly is based on three key findings: 1) clinical evidence, 2) demonstration of an elevated IGF-1 level, and 3) inability to suppress serum GH to less than 2 ng/ml following an oral glucose challenge (OGTT). In patients who do not have a significant reduction in GH levels in response to intermittent octreotide injections, more frequent dosing of octreotide may result in a greater clinical response. Octreotide may be administered continuously by a subcutaneous pump to patients with refractory acromegaly to prevent escape of GH between injections. In addition, studies using a long-acting injectable form of octreotide that is administered once a month are underway in Europe and are likely to be initiated here in the near future.

General Discussion
Acromegaly is a rare, slowly progressive chronic disorder that affects adults. The disorder is characterized by an excess of growth hormone. Symptoms include abnormal enlargement in bones of the arms, legs, and head. The bones in the jaws and in the front of the skull are typically the most affected. Acromegaly may also cause thickening of the soft tissues of the body, particularly the heart and accelerated growth leading to tall stature. In most cases, acromegaly is caused by the growth of a benign tumor (adenoma), affecting the pituitary gland.
Symptoms
The symptoms of acromegaly generally occur after puberty and slowly become more noticeable as affected individuals age. Facial features gradually become coarse because of the overgrowth of soft tissues and cartilage. Facial bones gradually become prominent, the lower jaw protrudes (prognathism), and an overbite may cause a wide separation and misalignment between the teeth (malocclusion). Affected individuals may also have an abnormally large tongue and unusually thick, full lips. People with acromegaly eventually develop a deep and husky voice due to thickening of the vocal cords and enlargement of the sinuses.
