Alternate Names : Growth Hormone Excess, Somatotroph Adenoma Definition Acromegaly is a chronic metabolic disorder caused by the presence of too much growth hormone. It results in gradual enlargement of body tissues including the bones of the face, jaw, hands, feet, and skull.
Endocrine Glands Acromegaly occurs in about 6 of every 100,000 adults. It is caused by abnormal production of growth hormone after normal growth of the skeleton and other organs is complete. Excessive production of growth hormone in children causes gigantism rather than acromegaly. The cause of the increased hormone secretion is usually a benign tumor of the pituitary gland. The pituitary gland, which is located just below the brain, controls the production and release of several different hormones including growth hormone.

General Discussion Acromegaly is a rare, slowly progressive chronic disorder that affects adults. The disorder is characterized by an excess of growth hormone. Symptoms include abnormal enlargement in bones of the arms, legs, and head. The bones in the jaws and in the front of the skull are typically the most affected. Acromegaly may also cause thickening of the soft tissues of the body, particularly the heart and accelerated growth leading to tall stature. Acromegaly (from Greek akros "high" and megalos "large" - extremities enlargement) is a is a hormonal disorder that results when the pituitary gland produces excess growth hormone (hGH). Most commonly it is a benign hGH producing tumor derived from a distinct type of cells (somatotrophs) and called pituitary adenoma . It most commonly affects middle-aged adults and can result in serious illness and premature death. In children the related condition is called gigantism , because long bones can still grow. The disease because of its slow and often insidious onset, is frequently not diagnosed correctly. Features that result from high level of hGH or expanding tumor include:
  • Soft tissue swelling of the hands and feet (early sign) Brow and lower jaw protrusion (enlarging jaw and hat size) Enlarging hands (ring size) Enlarging feet (shoe size) Arthritis and carpal tunnel syndrome Teeth spacing increase Heart failure (major medical problem). Compression of the optic chiasm leading to loss of vision in the outer visual fields. diabetes mellitus hypertension

Gigantism: growth hormone is secreted by the anterior lobe of the pituitary gland. It controls the rate of growth of the individual, as well as determining the timing of sexual maturity. It certain cases, usually due to a micro-tumour, the gland continues to secrete growth hormone for much longer, and in greater quantities, than it should. The affected person will continue to grow upwards, as well as outwards, until secretion is stopped, by medical or surgical intervention. If unchecked, the person will take on the characteristic appearance of one suffering from acromegaly. Acromegaly: when normal growth is complete the ends of the long bones fuse, and the person has reached their final height. If the pituitary starts to produce growth hormone again, the person will experience some upwards growth but, due to the fusion of the long bones, many bones, such as the lower jaw, will grow outwards. This leads to a characteristic appearance of the lower jaw and lips, thickening of the fingers and feet, and enlargement of cartilage structures, such as the nose and adams apple, the latter resulting in a deeper than normal voice. The condition is not hereditary, i.e. is not passed on from parent to child via the genetic material, however, some people believe that a set of conditions, or family tendencies, can be passed on, which may make the child more likely than normal to develop the condition. In one case, a man reported that several members of his family have had several pituitary related problems: - diabetes and cryptorchidism (undescended testes), for instance.

Overcoming the problem A question of size If asked, many people say that they would like parts of their body to be bigger. It's a fact of life that in today's society, 'size matters.' However, there are times when enlargement of certain body parts is not so great, or indeed desired. Growth hormone does what it says The pituitary gland is responsible for producing many different hormones. Some of these hormones act directly on parts of the body, whilst some trigger other glands to in turn release their own hormones. One hormone produced by the pituitary gland that acts directly is called growth hormone (GH) and it doesn't take a genius to figure what is does. Growth hormone stimulates the growth of muscle, cartilage, and bone. Overproduction of growth hormone In rare circumstances, the pituitary gland may produce excessive amounts of growth hormone. Invariably this is the result of the presence of a pituitary tumour, which in the majority of cases is non-cancerous. Very occasionally these tumours appear as part of an inherited condition called multiple endocrine neoplasia, in which tumours develop in several different hormone-producing glands around the body.
