Background: Now a rare disease, acrodynia (painful extremities) primarily affects young children. The symptoms of irritability, photophobia, pink discoloration of the hands and feet, and polyneuritis can be attributed to chronic exposure to mercury. Pathophysiology: The most frequent sources of mercury prior to the legislated removal of the heavy metal from these preparations were calomel-containing anthelminthics, laxatives, diaper rinses, teething powders, fungicides in paint, repeated gamma-globulin injections, termite-protected wood (mercury bichloride), watch batteries (ie, via ingestion), and mercurial antibacterial ointments. This legislation corresponded to the virtual disappearance of acrodynia. Present-day cases reveal more novel exposure, such as mercuric oxide used to treat eyelid mites. Some have suggested the disease may represent a delayed allergic or hypersensitivity reaction because not all persons exposed to mercurial compounds develop the disease. Because the metal can be stored in the body to some extent and intolerance may develop long after exposure, morbid symptoms may appear weeks or months after the drug administration (ie, exposure), with its cause escaping recognition. The deleterious effects of relatively small doses of mercury on the nervous system that are sometimes seen in the course of acrodynia add to the acrodynic reaction. In acrodynia, no reflex dilatation of the peripheral vessels occurs in response to heat. Vasoconstriction is abolished only when the nerve supply to the arterioles is interrupted.

In August 1989, a previously healthy 4-year-old boy in Michigan was diagnosed with acrodynia, a rare manifestation of childhood mercury poisoning. Symptoms and signs included leg cramps; rash; itching; excessive perspiration; rapid heartbeat; intermittent low-grade fevers; irritability; marked personality change; insomnia; headaches; hypertension; swelling; redness and peeling of the hands, feet, and nose; weakness of the pectoral and pelvic girdles; and nerve dysfunction in the lower extremities. A urine mercury level of 65 ug/L was measured on a 24-hour urine collection. Treatment with intensive chelation therapy increased his urine mercury excretion 20-fold. Examination of his mother and two siblings found urine mercury levels greater than or approximately equal to his; his father had elevated, although lower, levels. Parents and siblings were asymptomatic, although electromyographic abnormalities were detected in one sibling. The Michigan Department of Public Health (MDPH) identified inhalation of mercury-containing vapors from phenylmercuric acetate contained in latex paint as the probable route of mercury exposure for the family; 17 gallons of paint had been applied to the inside of the family's home during the first week of July. Samples of the paint contained 930-955 ppm mercury; the Environmental Protection Agency (EPA) limit for mercury as a preservative in interior paint is 300 ppm. During July, the house was air-conditioned, and the windows were not opened.

ACRODYNIA OR HOW TO FAIL TO RECOGNIZE MERCURY POISONING FOR MORE THAN 100 YEARS Mats Hanson, Ph.D Anyone who has experienced the toxic effects of amalgam fillings and has compared the symptoms with those reported in the mercury literature, is amazed that the poisoning has not been recognized at any of numerous visits to doctors during years or decades. The oral symptoms (salivation, ulcers, metallic taste, bleeding gingiva and general irritation of mouth and upper airways) directly indicate mercury poisoning and in addition there are usually numerous other typical symptoms: psychic, neurologic, gastrointestinal etc. Damage from medicines is called iatrogenic and it is certainly not an easy task to bring dentists to admit that a practice they thought was beneficial for the patients in reality caused damage. It appears to be even harder to get the medical profession to recognize that numerous patients with symptoms indicating anyone of a number of disaeses are mercury poisoned. The problem is certainly not new, especially not regarding mercury. The metal and its chemical compounds have been used widely; in numerous medicines, for disinfection, electrical apparatus, thermometers and barometers, photographic purposes, toys, cosmetic preparations, paints etc. There are several cases described where children have played with mercury and distributed the metal at home. The ignorance is still baffling. In a recent electronic magazine I found a description of experiments with lasers to produce holographic pictures. The hologram was obtained by having the laser beam reflected from a drop of mercury towards the object to be photographed and a film. "The drop of mercury can be obtained from a broken thermometer... The high voltage is directly life- threatening to touch and looking into the laser beam can give irreversible eye damage." Not a word about mercury giving off toxic vapor, especially if heated by a beam of light.
