En 1921 volvió a dedicarse por completo a la medicina, y en 1927 estableció la técnica diagnóstica de la angiografía cerebral para la detección de tumores. También realizó las primeras arteriografías e introdujo la leucotomía prefrontal en el tratamiento de algunas enfermedades mentales. En 1949 le fue otorgado el Premio Nobel de Medicina.
