THE PHYSICIAN AND SPORTSMEDICINE - VOL 25 - NO. 4 - APRIL 97 In Brief: Anterior cruciate ligament (ACL) injuries occur most frequently in planting and cutting sports such as basketball, soccer, and volleyball. National Collegiate Athletic Association injury data show that female athletes injure the ACL more frequently than their male counterparts do. The greater incidence of ACL injuries in women probably stems from complex, interrelated factors, possibly including hamstring-quadriceps strength imbalances, joint laxity, and the use of ankle braces. Successful treatment often includes surgery. T he number of girls and women participating in all levels of sports has risen greatly in recent years, and the way they play has changed, too. Women's sports, once dominated by a slow, defensive style, are now played with speed, precision, and power. But with these changes have come increased injuries, and female athletes have higher injury rates than men in many sports, particularly basketball, alpine skiing, volleyball, and apparatus gymnastics (1,2). THE PHYSICIAN AND SPORTSMEDICINE - VOL 25 - NO. 4 - APRIL 97 In Brief: Athletes at all levels explore ergogenic aids. Testosterone and growth hormone are still abused and difficult to detect. Single doses of albuterol or salmeterol do not seem ergogenic, but questions remain about prolonged dosing and about other beta2 agonists. Caffeine can be ergogenic for prolonged or brief exertion. Creatine supplementation is legal and in vogue among strength and power athletes. Not all studies agree, but creatine seems ergogenic for repeated brief bouts of intense exercise. Ergogenic aids pose vexing questions for athletes, physicians, and society. T he Olympic motto is Citius, Altius, Fortius swifter, higher, stronger. Maybe, at least in the strength and power sports, we should add fraudator , the Latin word for deceiver. In spite of increasingly sophisticated drug testing at the Olympics, suspicions of the use of illegal ergogenic aids are stronger than ever. Proof of cheating is often lacking, but by all appearances, the suspicions are well-founded. In a never-ending game of cat and mouse, athletes who cheat seem always one step ahead of those who try to catch them (1). Attempts to enhance athletic performance are not new. The Olympic Games date back 2,700 years, so trickery in sport likely dates back at least that long. Ancient Greek Olympians ate mushrooms to win. Aztec athletes ate the human heart. In the late 1800s, European cyclists took heroin, cocaine "speedballs," and ether-soaked sugar tablets. The winner of the 1904 Olympic marathon, Tom Hicks, took strychnine and brandy during the race. The winner of the 1920 Olympic 100-m dash, Charlie Paddock, drank sherry with raw egg before the race. In the 1960 Olympics, Danish cyclist Knut Jensen died in the road race from taking amphetamine. In the 1967 Tour de France, famed British cyclist Tommy Simpson died, also from amphetamine (1).

Poor nutrition in female athletes is often underlying symptoms such as: tiredness, performance plateaus or decreases, burn-out, and repeated injuries. Female athletes may diet more aggressively than their non-sporting counterparts (see female athletic triad). Female athletes often want to loose weight to improve their performance.
Women who eat at least 1200 calories per day should only need iron or calcium supplements as a back-up. She should be able to get all her nutrients in by eating a variety of foods along these lines. Female Athletes Are Prone To Iron Deficiency
Iron-rich foods, dietary supplements, and vitamin C (which helps absorb iron) can improve performance. Caffeine intake blocks iron uptake. Calcium
As mentioned, few girls or women can get by with less than 1200-1500 per day. Extremely low fat diets have risks, even if the overall calories intake is good. This is generally considered to be diets that are less than 10% fat calories. Consequences of these include: low energy and performance, and deficiencies of iron, zinc, magnesium, folate (important if the woman plans a pregnancy), vitamins E, B6, B12, C, and A. Inadequate calorie intake can be the first step down the road to the Female Athletic Triad or other eating disorders. Rate this article: (1=lowest, 5=highest)

FUELING UP BEFORE THE PRE-EVENT MEAL
Adequate fluid intake is the single most important recommendation for all types of exercise.
SUPPLEMENTING INTAKE DURING THE EVENT
Replacing both fluid and carbohydrate during exercise is important. What and how much to replace depends on the type, duration, and intensity of the exercise. Adequate fluid intake is the single most important recommendation for all types of exercise. The general recommendation is to drink 1/2 to 1 cup of water every 10 to 20 minutes. In a hot environment, when perspiration is especially heavy, drinking up to 2 cups of water every 15 minutes may be necessary to replace fluid losses. Adequate hydration enables the active body to regulate its temperature effectively and allows for good circulation and muscle function.
Carbohydrate replacement is necessary in events lasting longer than 90 minutes and may even be beneficial during high intensity exercise of shorter duration. This applies to both continuous events like cycling, running, and hiking, and sports with intermittent activity like soccer and weight training. Under these conditions, consuming carbohydrates during exercise increases both the time and the intensity the athlete is able to exercise before becoming exhausted. Researchers believe that carbohydrate feedings delay fatigue by providing additional fuel for the working muscle and preventing blood sugar from dropping.

1 Roger Bannister
The Mile, 1954
For many years, physiologists said that it was inconceivable for an athlete to run under four minutes for the mile. Everyone began to think that a four minute mile would be a barrier that no human would be capable of breaking. Many world class athletes allowed the mythical time to rule them.
However, a young Oxford medical student, Roger Bannister, was not deterred from his goal to be the first to break the barrier. As others backed down from the challenge, he looked forward for his chance to conquer the barrier that had gained control of the world's elite.
On May 6, 1954,Bannister stepped onto the track at Oxford where he would accomplish one of the greatest feats in history. It was a cold and windy day, but that would not stop him from reaching his goal. He toed the line along with the other runners. He crossed the finish line and stumbled to the ground. His effort to break the barrier left him drained of all his energy. His identity hinged on the time of the watch. If he succeeded he would be hailed as one of the greatest runners ever, but if he did not who would remember his name?
A calm came over the crowd as the announcer read Bannister's time. "Three Minutes......" The crowd erupted in cheers and applause that drowned out what the announcer was saying. He had run 3min 59.4sec.
