June 9, 1999 (from CNN.com) Web posted at: 3:10 PM EDT (1910 GMT) LONDON (Reuters) Ultrasound scans, routinely used to look at internal organs and to monitor the growth of a developing fetus, can stop cells from dividing normally, Irish scientists said Wednesday. Researchers at University College in Dublin told New Scientist magazine it is too early to tell if the changes they found in the cells of mice are harmless or what the implications of the findings could be for humans. "It has been assumed for a long time that ultrasound has no effect on cells," said Patrick Brennan, who led the research team. "We now have grounds to question that assumption." During the study, the rate of cell division in mice that were given an 8 megahertz scan lasting 15 minutes was 22 percent lower than normal, and the rate of cell death doubled. Routine hospital scans use frequencies between 3 and 10 megahertz and can last up to 60 minutes. Brennan said the sound waves of the scans could be damaging the DNA in cells, which could delay cell division and repair, or it might be switching on p53, a tumor suppressor gene that controls cell death.

Ultrasound is a technique that uses sound waves to show a picture of a baby (fetus) in the womb. Because it uses sound waves instead of radiation, ultrasound is safer than X-rays. Ultrasound has become an increasingly important prenatal care tool. It provides information that can guide a health care provider’s plans for a pregnant woman and improve the outcome of pregnancy.
How does ultrasound work? Ultrasound works by bouncing sound waves off the developing fetus. Echoes from the waves are analyzed by computer to produce a moving or still picture, called a sonogram, on a monitor. The technique is also called sonography. How is ultrasound performed? Sound waves are sent by a handheld device, called a transducer, placed on the pregnant woman’s abdomen. For a clearer picture, the abdomen is covered with a thin layer of gel, and the woman may be asked to have a full bladder during the test. Ultrasound is painless, but many women find having a full bladder uncomfortable. In early pregnancy, the uterus and fallopian tubes are closer to the vagina than to the surface of the abdomen. If ultrasound is needed very early in pregnancy, sound waves may be delivered by a probe inserted into the vagina, a technique called transvaginal ultrasound, or by placing the probe at the opening of the vagina, called translabial ultrasound. Both of these techniques can be used throughout pregnancy to allow a closer look at the cervix and lower uterus.

Special Medical Reports The following is part of " Tips From Other Journals " from the October 1, 1997 edition of American Family Physician Diagnosis of Placenta Previa by Transvaginal Ultrasound Placenta previa occurs in approximately one in 200 to 250 pregnancies and is associated with potentially serious consequences from hemorrhage, abruption of the placenta or emergency cesarean delivery. Abruption of the placenta occurs 14 times more frequently in pregnancies with placenta previa than in normal pregnancies, and cesarean delivery occurs four times more frequently. Taipale and colleagues evaluated the usefulness of the assessment of placental location by transvaginal ultrasonography during the first and early second trimesters of pregnancy as a screening tool for placenta previa. A total of 6,428 pregnant women with a singleton fetus who were screened by transvaginal ultrasonography to determine gestational age and to diagnose major fetal congenital anomalies were included in the study. Placental location and morphology were systematically recorded. Pregnancy outcomes were traced and correlated with data on placental position early in pregnancy. The placenta extended to or over the internal cervical os in 287 patients (4.5 percent), but this diminished from 5.5 percent at less than 13 weeks' gestation to 2.4 percent at 15 to 16 weeks' gestation. Overall, the placenta extended at least 15 mm over the internal os in 156 (2.4 percent) of the patients when ultrasound screening was performed early in pregnancy. In eight of these patients, placenta previa was present at delivery. Conversely, two cases of placenta previa at delivery were not identified by the screening. The risk of placenta previa at delivery was found to be 5.1 percent if the edge of the placenta extended at least 15 mm over the internal cervical os at 12 to 16 weeks' gestation.

    ULTRASOUND
    Medical Ultrasound (US) uses high frequency sound waves to create an image of living tissue. The basic technique is similar to weather radar and submarine ultrasound. A sound signal is sent out, and the reflected "echoes" are used to create the image. Ultrasound, unlike most other imaging methods, can create truly real-time "movies" of the heart beating, contraction of bowel loops (peristalsis), and can even show blood flowing. Using a special form of US called DOPPLER (just like police speed RADAR) the speed and direction of flowing blood can be measured and illustrated in color pictures. All of this without harming the patient. This doppler technique allows Radiologists to detect blocked blood vessels in the neck, and elsewhere. One of the most common US procedures is the examination of the living fetus within the mother's womb (OBSTETRIC sonography). US images (like CT and MRI) are viewed as if they were "sections" of the body.

    In consideration of prenatal screening it is important to keep in mind that there is NO BENEFIT to the baby, only the actual harm of the invasive tests such as ultrasound and amniocentesis, and the potential of harm if the results aren't what the parents are hoping for. Prenatal screening is generally used as a means of finding out if the baby is good enough or perfect enough to be allowed to live. This kind of "tentative pregnancy" has ill effects on all tested babies, even those who are deemed good enough to be allowed to live. (For more information about how a tentative pregnancy affects a fetus "in utero" and later throughout life, familiarize yourself with the work of APPPAH Becoming a parent is a very serious matter, and if you cannot live with any child who is less than perfect, well, then, maybe you want to give some thought to the commitment one makes in choosing to become a parent. If you can't accept and love a baby born with developmental problems, what will you do if your child develops problems later in life? What will you do if your child is autistic or develops juvenile diabetes or leukemia or is disabled in an accident? If your children don't disappoint you before you are born, you can generally bet that they will at some time disappoint you after they are born. It takes tremendous maturity and responsibility to accept that in choosing to become a parent, you are offering to satisfy humanity's drive to propagate the species, and that your role as a parent is to serve the child, not the other way around.
