Gene therapy for Parkinson's disease begins

NEW YORK (AP) Researchers slipped billions of copies of a gene into the brain of a Parkinson's disease patient Monday, marking the first attempt to test gene therapy in a person with that disease. The patient, 55-year-old Nathan Klein of Port Washington, New York, said he was "feeling fine" in a telephone interview a few hours after the surgery. Klein said the disease gave him a tremor in his right arm, and made him shuffle and have to catch himself sometimes from falling. Medicines offer some relief, he said, but when he heard about the new study he decided to volunteer. With the disease, "you don't want to go out," Klein said. "It's embarrassing when you're shaking a lot and you walk like an old man." Klein is the first of a planned 12 patients for the study, said the surgeon, Dr. Michael Kaplitt of Weill Cornell Medical College. The research is focused on the procedure's safety, rather than whether or not it relieves symptoms. "Our whole goal here is to prove this is safe before we move on," Kaplitt said.

Gene therapy will probably not be inordinately complex. Although extremely high-tech in its design and manufacture, it is likely to be easy to administer in most practice settings. It will be widely used for classic genetic and common diseases alike.
Dr. Eck is the Ann B. Young Assistant Professor of Medicine and Director of Cancer Gene Therapy Programs, University of Pennsylvania Medical Center, Philadelphia. Ever since DNA was first confirmed to be genetic material in the early 1950s, researchers have been wondering whether it might be altered, as a new kind of intervention against disease. After four decades, the knowledge and tools with which to do so are in hand. Indeed, we are already at the end of a decade of initial clinical trials. Among the strategies being pursued, perhaps the most prominent and straightforward is gene therapy. The fundamental idea is to administer a functional gene, so as to give targeted cells what amounts to a new protein-manufacturing capacity, one that would have been present but is missing or defective, or sometimes one never meant to be there. In the first instance, the treatment addresses single-gene, or mendelian, disease; in the second, it becomes applicable especially to acquired illnesses, including heart disease and cancer (Figure 1), where ultimately gene therapy may have its greatest public-health impact. In all cases, success is measured by how well an added gene functions. The major problems lie in delivering the gene to the correct destination and in having it act with a useful duration, producing sufficient amounts of the desired protein.

    Many human diseases are caused by defective genes. A few common examples: Disease Genetic defect hemophilia A absence of clotting factor VIII cystic fibrosis defective chloride channel protein muscular dystrophy defective muscle protein (dystrophin) sickle-cell disease defective beta globin hemophilia B absence of clotting factor IX severe combined immunodeficiency (SCID) any one of several genes fail to make a protein essential for T and B cell function All of these diseases are caused by a defect at a single gene locus . (The inheritance is recessive so both the maternal and paternal copies of the gene must be defective.) Is there any hope of introducing functioning genes into these patients to correct their disorder? Probably. Other diseases, also have a genetic basis, but it appears that several genes must act in concert to produce the disease phenotype . The prospects of gene therapy in these cases seems far more remote.

Vector Ventures : News and Announcements from Members THE SIXTH ANNUAL MEETING of the American Society of Gene Therapy (ASGT) took place June 4–8 in Washington, D.C. UAB researchers including James Markert, M.D., Richard Whitley, M.D., John Mountz, M.D., Donald Buchsbaum, Ph.D., Kurt Zinn, D.V.M., Ph.D., and Tandra Chaudhuri, Ph.D., chaired sessions or made oral presentations of their research. As in years past, UAB had among the largest number of accepted abstracts and posters at the conference. The meeting also created an opportunity for former GTC postdocs to gather together again and catch up on each other’s work. GTC director David T. Curiel hosted an “alumni” breakfast reception, which was attended by 32 researchers from more than a dozen countries. The ASGT was established in 1996 to foster the exchange and diffusion of research and ideas relating to gene therapy, and to encourage professional and public education in all areas of gene therapy. This year some 2,000 researchers, academicians, and investigators from around the world gathered in Washington to discuss the latest in gene-therapy research.
