E’ un’affezione rara che colpisce il sesso maschile e si caratterizza per febbre, dolore addominale, diarrea da malassorbimento, anemia, linfoadenopatia, artralgie ed artriti, pleuriti, pericarditi ed endocardite valvolare, calo di peso e si accompagna a lesioni dermatologiche con iperpigmentazione cutanea, noduli pre-rotulei, edemi periferici. E’ specifica e patognomonica di questa malattia l’infiltrazione di tessuti interessati da parte di grossi macrofagi contenenti glicoproteine e di piccoli bacilli a forma di bastoncino. Il primo caso fu descritto da George Hoyt Whipple in un lavoro di più di 70 anni fa, il quale descrisse nell’intestino tenue e nei linfonodi mesenterici un’infiltrazione estesa della lamina propria da parte di grosse cellule schiumose (macrofagi) che distorcevano l’architettura dei villi intestinali ed avevano depositi di lipidi nella lamina propria e nei linfonodi. Anatomia patologica . Nel M.d.W. l’intestino tenue presenta una parete intestinale ispessita ed edematosa e nella struttura della mucosa, nelle sue sezioni istologiche, si nota l’infiltrazione di grossi macrofagi, appunto, il cui citoplasma è ripieno di molti grossi granuli glicoproteici che si colorano con la tecnica del PAS. La lamina propria può contenere anche accumuli di leucociti polimorfonucleati. I vasi della mucosa sono dilatati e sono ripieni di grasso e lipidi. L’epitelio assorbente della mucosa è alterato sia per la presenza dei macrofagi, sia per l’accumulo dei lipidi e la distrofia; nella lamina propria si evidenziano bacilli. Sembrerebbe che il materiale PAS positivo presente dipenda da un accumulo di residui di bacilli fagocitati.

Other characters Whipple's disease, (George Hoyt Whipple, 18781976, American pathologist), uncommon systemic disease that may affect virtually any organ system of the body and in the majority of patients involves the small intestine and its lymphatic drainage. Although the pathogenesis of the disease is not yet fully understood it is generally accepted that the disease is caused by a bacterial organism. The bacterium has not yet been cultured but studies based on polymerase chain reaction technique indicate that the Whipples bacillus belongs to a subdivision of actinomycetes and has distinct morphologic characteristics. It has been named Tropheryma whippleii Grossly the wall of the small intestine is thickened and oedematous. The villi may be markedly widened. The mesentery tends to be markedly thickened and contains numerous enlarged lymph nodes. Coalescence of mesenteric or retroperitoneal lymph nodes may mimic an abdominal mass. The typical abnormality seen on histological sections is the presence in the mucosa of large macrophages containing large glycoprotein cytoplasmatic granules that stain brilliant magenta with periodic acid Schiff (PAS) agent with virtual replacement of most cellular elements in the lamina propria by these macrophages. Mucosal and submucosal lymphatics are dilated, probably lymphatic obstruction, from enlarged lymph nodes. Electron microscopic studies have revealed the presence of numerous tiny rod-shaped Gram-positive but not acid-fast bacilli. After treatment with antibiotics the bacilli decrease or disappear, together with a decrease in the number of PAS-positive macrophages.
