Vitiligo
Vitiligo is a skin condition whose exact cause is unknown. In vitiligo, patches of skin lose their pigmentation when the pigment producing cells, the 'melanocytes' are attacked and destroyed. It may affect the skin, mucous membranes, eyes, inner ear or hairs leaving white patches. The usual type of vitiligo is called 'Vitiligo Vulgaris' (means: common vitiligo). Variant types include linear, segmental, trichrome and inflammatory vitiligo.
This disease affects an estimated 1% of the world's population. It affects individuals of all ethnic origins and both sexes, but is much more easily noticed on darker skin as areas that fail to tan. It is hereditary in one third of those affected. Vitiligo often starts on the hands, feet or face, and frequently pigment loss is progressive. Half the patients first notice vitiligo before 20 years of age. It often appears in an area of minor injury or sunburn.
It is believed that vitiligo is an autoimmune disorder (autoimmune means the bodies own immune system turns on itself). Certain white blood cells direct the destruction of melanocytes. People with vitiligo are also somewhat more prone to other autoimmune diseases, such as alopecia areata, autoimmune thyroid disorders, Addison's disease, pernicious anemia, and diabetes mellitus.
The diagnosis of vitiligo is usually straightforward, and no special testing is needed. While vitiligo is a cosmetic problem and does not affect the health directly, it is disfiguring and may be psychologically traumatic. The condition cannot be cured at present, but treatments are available that may be very helpful. Medical treatments target the immune system, and try to reverse the destruction. Surgical treatments are less commonly done, and transplant healthy melanocytes from other areas. Both treatments may be difficult and prolonged.

Le phénomène de Koebner est l’aggravation d’une maladie de peau lors des traumatismes. Le vitiligo atteint en moyenne 1% de la population. Grâce à une classification judicieuse, on peut porter un pronostic lors de la première consultation. On distingue :
  • Le vitiligo segmentaire dont la pathogénie et l'évolution sont particulières et qui est la meilleure indication pour la transplantation mélanocytaire
  • Le vitiligo généralisé pour lequel trois stades évolutifs sont décrits :
    • Stade 1 : taches hypochromes avec persistance de rares mélanocytes épidermiques Stade 2 : taches achromiques avec poils noirs (disparition des mélanocytes épidermiques, mais persistance des mélanocytes folliculaires) Stade 3 : taches achromiques avec poils blancs (disparition des mélanocytes ou absence de réservoir folliculaire en peau glabre)
    Chez un même patient, certaines taches auront des chances de se repigmenter (réservoirs folliculaires préservés) et d'autres ne se repigmenteront pas (zones glabres des extrémités).

Vitiligo Information.
Vitiligo is a chronic (long term) skin disorder that affects about 1 percent of the world population and is characterised by the loss of skin colour, or pigment, from the skin causing the affected skin to become lighter than the skin surrounding it. Vitiligo affects people from all races and colours of skin, male and female, all age groups society groups and currently cannot be cured.
The condition is not infectious and cannot be spread or caught by coming into contact with sufferers. The condition causes the body's melanocytes to stop producing melanin (the body's natural pigment), which in turn causes the skin to lose its natural colour. It is believed that the condition is a result of our individual genetic make up. This does not mean, however, that a person will contract Vitiligo just because a parent has the condition. The circumstances for why a child may contract Vitiligo whilst both parents show no signs of the disorder are still unknown. This issue of triggering factors is still under investigation to see if there are a series of genetic triggers required to cause the skin condition to take hold of the sufferer.
Occasionally Vitiligo may appear in sufferers who also suffer from other disorders such as alopecia areata thyroid imbalance or diabetes , or where there is a family history of these conditions. Whilst this is not always the case, a predisposition to one or more of these conditions may help a medical professional or dermatologist to diagnose Vitiligo.

VITILIGO Vitiligo is a distressing disorder and especially so in darker skinned patients. Treatment is unsatisfactory. Advice
  • High protection factor sunblocks will protect the involved areas from sun damage and hopefully will prevent the normal skin from getting darker and so enhancing the cosmetic disability.
    If the vitiligo is of recent onset, it might be worth trying Betnovate Cream Full Strength twice a day for several months but patients should be warned that atrophy of the skin may result which may be permanent.
    PUVA and UVB phototherapy is very rarely effective and we now rarely use them.
  • Referrals for vitiligo are unlikely to be worthwhile Index
