People who have problems with their temporomandibular joint (TMJ) may go to as many as seven medical specialists to rule out other problems before the correct diagnosis of a TMJ disorder is finally made - often serendipitously. Because TMJ problems create symptoms common to other conditions, they have come to be known as "the Great Impostor." The American Dental Association estimates that 10 to 14 million Americans have TMJ disorders; 80 percent of these are women between 24 and 50 years of age. What are TMJ Disorders? A joint is where two bones connect. You have two TMJs, one in front of each ear where the jawbone (mandible) connects to the skull (see illustration). The mandible has rounded ends that fit into a socket and slide on a disc as your mouth opens. TMJ disorders occur when this mechanism is somehow faulty, most commonly when the teeth, the chewing muscles and the TMJ do not correctly work together. It could also result from disease in the joint itself. Symptoms of TMJ Disorders The most common symptom of TMJ disorder is a dull, aching pain around the ears. The jaw muscles may be tender to the touch and spasm in the chewing muscles may radiate to the muscles of the neck and shoulders. TMJ disorder often causes referred pain - pain experienced in one area caused by a problem in another. For example, TMJ sufferers may complain of frequent earaches, sinus pain and headaches. Other symptoms may include ringing in the ears (tinnitus), minor hearing loss, dizziness and even nausea. Clicking, grating or popping sounds when the mouth opens or closes are common in TMJ disorders, but jaw noise in the absence of other symptoms generally does not require treatment of the TMJ. stands for T emporo M andibular J oint, which is located in front of the ears and attaches the lower jaw (mandible) to the temporal bones in your skull. The TMJ serves as a hinge for the jaw and moves together every time you use your mouth.

open Any disruption or disorder to these joints could lead to a medical and dental problem known as TMJ dysfunction.
There are a wide variety of signs and symptoms indicative of TMJ dysfunction. Typically, a TMJ dysfunction diagnosis is usually made after the patient complains of tenderness in the jaw muscles, multiple headaches, and/or dull, aching pain in the face, but symptoms may also include pain in the sinuses, ears, eyes, teeth, neck muscles, and shoulders. Clicking and grating in the jaw joints, inability to open or close the mouth freely, and difficulty in chewing and swallowing are also often present. All of these pain and dysfunction symptoms can be temporary, chronic (continuing for longer than six months), or intractable (never-ending). TMJ dysfunction can have a variety of causes
and is believed to result when the chewing muscles and jaw joints
do not work together correctly.

In TMJ syndrome, also called TMJ dysfunction, you have painful symptoms involving this joint. These symptoms may include:
  • tenderness over or near the joint;
  • a dull ache in front of your ear;
  • a clicking sound or grating feeling when you open and close your mouth;
  • headaches;
  • ringing ears;
  • aching shoulders;
  • difficulty swallowing, and
  • locking of the jaw joint, making it difficult to open or close your mouth.
TMJ may be caused by grinding your teeth, clenching your jaw or trauma to the side of your face. It may be caused by arthritis, other forms of inflammation, a tumor, or a misalignment or malfunction of the joint. Your dentist or health care provider may make a diagnosis based on symptoms and an exam. X-rays or a magnetic resonance imaging scan also may be done. The pain of TMJ sometimes goes away on its own. When it does not, treatment may include resting your jaw, biofeedback, relaxation therapy, heat therapy, local anesthetics, pain relievers and anti-inflammatory medications. Stress reduction techniques and behavior modification to reduce tension may also be helpful in relieving TMJ. If you grind your teeth at night, you may need to wear a device such as a bite guard when you sleep. Orthodontic splints, which are plastic bite plates to promote better jaw alignment, may provide relief from jaw locking, pain and noise. You may need surgery to repair or remove the disc that cushions and separates the bones of the joint, portions of the bones themselves or both.
