"TMJ Disorders" and "TMD" are generic terms which like any other joint in the anatomy, encompasses a whole spectrum of diseases, derangements of the articulating elements in the Joint, and injured or damaged tissues affecting the function of the Joint. Obviously, in order to treat a patient effectively, it is necessary to first find out EXACTLY what the specific problems are. There are two elements to diagnosis. The first is a comprehensive history and clincal examination. The second is a series of diagnostic studies. Each practitioner has his or her own favorites which they rely on, and which may be used either singly or in any combination. In general, the diagnostic studies available for TM Joint diagnosis and evaluation are:
  • MRI (Magnetic Resonance Imaging) Electromyography (studies of muscle function) Motion Performance Studies Joint Vibration Analysis (a form of ultra-sound sonography) X-rays of the Dentition
TREATMENT
The diseases which affect the TM Joints are no different, really, than the diseases which affect other joints in the human anatomy. Rheumatoid, and Osteoarthritis; Inflammation of the Joint capsule; inflammation of the Synovium; torn Ligaments; perforation, or tears in the Articular Disks; internal derangement of the Condyle Head, and or Disk, in the Joint space, and etc. There are two basic types of treatment for TM Joint disorders: surgical and non-surgical, but it is generally agreed that in most cases the non-surgical approach should be taken first in an effort to restore comfort, and improve jaw function to an acceptable level.

You may not have heard of it, but you use it hundreds of times every day. It is the Temporo-Mandibular Joint (TMJ), the joint where the mandible (the lower jaw) joins the temporal bone of the skull, immediately in front of the ear on each side of your head. A small disc of cartilage separates the bones, much like in the knee joint, so that the mandible may slide easily; each time you chew you move it. But you also move it every time you talk and each time you swallow (every three minutes or so). It is, therefore, one of the most frequently used of all joints of the body and one of the most complex. You can locate this joint by putting your finger on the triangular structure in front of your ear. Then move your finger just slightly forward and press firmly while you open your jaw all the way and shut it. The motion you feel is the TMJ. You can also feel the joint motion in your ear canal. These maneuvers can cause considerable discomfort to a patient who is having TMJ trouble, and physicians use these maneuvers with patients for diagnosis.

WHAT IS TMJ? TMJ is a disorder of the jaw joint and related structures where the ball (condyle), socket (fossa), and cushioning disk (meniscus) of the joint are damaged or misaligned (Note 1) . Over time, the meniscus becomes increasingly compressed and torn, allowing the bony structures of the ball and socket to deteriorate at an accelerating rate due to the grinding wear on the joint and the loss of cushioning effect of the meniscus. This grinding (crepitus) and concurrent pressure on the capsular ligament surrounding the joint can cause head pain, and may create difficulty in opening or closing of the mouth. The body attempts to realign the joint using the musculature in the face, jaw and neck. As these muscles become fatigued, additional muscles in the shoulders and back attempt to help this condition, then become stressed themselves. This can cause symptoms of generalized muscle pain and weakness that are diagnosed as Myofascial Pain Dysfunction Syndrome (Fibromyalgia) (Note 2) and/or Chronic Fatigue Syndrome.

Dr. Yamamoto has studied Gnathology (occlusion) for the last 27 years. As a result, all of his procedures have a special attention to achieving a comfortable and functionally correct bite. The state requirement for continuing education for dentists is 50 hours every two years. In an effort to provide the most up to date services for his patients, Dr. Yamamoto averages over 200 hours every two years. This quadruples the state requirement.
