    What is Temporomandibular Joint Disease?

    MPD vs. Intra-articular Disease
    One of the most important concepts to understand is the basic difference between true interarticular disease and the myofascial pain complex of symptoms which the lay press tends to erroneously label as TMJ. These two entities can, and often do, coexist but they are not synonymous. In patients where both are present it is almost always helpful and desireable to treat the MPD first and then address the interarticular pathology. The most notable exception to that treatment sequence is the onset of acute interarticular disease in a previously healthy joint.
    Surgical vs. Non-Surgical Treatment
    For MPD the majority of treatment options are non-surgical however, some consider brisement used for trismus secondary to immobilization syndrome a surgical procedure. Warmth, massage, physical therapy including range of motion exercises, non-steroidal anti-inflammatories reduction in parafunctional activity and patient education form the bulk of the treatment protocols. Many practicioners believe in the construction of intraoral "splints" of a variety of types to treat parafunction. Some purport to treat interarticular disease with these devices. The ability of such devices to treat interarticular disease is not supported by the scientific and orthopedic literature. Their efficacy in the treatment of muscle spasm/myofascial pain is perhaps a little higher but often no better than what can be obtained through the utilization of over the counter rubber mouthpieces such as are provided for high school athletes in contact sports.

    Overview
    TMJ is the commonly used acronym for temporomandibular joint disorder . The pain associated with TMJ is thought to be caused by displacement of the cartilage where the lower jaw connects to the skull causing pressure and stretching of the associated sensory nerves. I developed a very bad case of TMJ several years ago, along with an overall case of fibromyalgia. My jaw was so painful that I had to eat baby food at times.

    W elcome to our web site. In posting this site, my goal is to give you information about an often misunderstood condition that affects approximately 11 million Americans... temporomandibular disorder (TMD). I hope that you will spend a few minutes to visit the several pages that are listed at the top left on this and on every other page. It may look like a lot of reading, but if you will take a few minutes, I believe that you will have a better understanding of this pain condition, and will therefore, be better equipped to make educated decisions about the many treatment options that you will encounter. Before we start, I must state that this information is just that... information. To correctly identify your specific condition, specific diagnostic testing must be completed.

    WHAT IS TMJ? Many people, even doctors, nurses, and insurance companies, use the term TMJ. But what does this abbreviation mean? The term TMJ is an abbreviation for Temporo-Mandibular Joint , or the jaw joint. In fact, there are really two TMJs, one in front of each ear. The TMJ is the joint formed by the temporal bone of the skull ( Temporo ) with the lower jaw or mandible (hence, mandibular ). These joints move each time we chew, talk or even swallow. The TMJ is actually a sliding joint and not a ball-and-socket like the shoulder. This sliding allows for pressures placed on the joint to be distributed throughout the joint and not just in one area. The TMJ is the most complex joint in the human body. Placed between these two bones is a disc, just like the one between your back bones. This disc is primarily made of cartilage and in the TMJ acts like a third bone. The disc, being attached to a muscle, actually moves with certain movements of the TMJ. The nerve to the TMJ is a branch of the trigeminal nerve and therefore, an injury to the TMJ may be confused with neuralgia of the trigeminal nerve. The two bones of the TMJ are held together by a series of ligaments, any of which can be damaged, just like any other joint. A damaged TMJ ligament usually results in a dislocation of the disc, the lower jaw, or both. Also, the bones are connected by two main muscles: the temporalis, the masseter, and a muscle just discovered by Dr. Shankland, the zygomandibular. Any or all of these muscles may be painful and produce pain in the TMJ or at the very least, abnormal movement of the lower jaw.

Although no specific data exists regarding the social impact of temporamandibular joint disorders, TMJ is estimated to account for as much as thirty billion dollars a year in lost productivity. Americans lose 550 million work days every year due to symptoms associated with TMJ, with facial pain and headache being the most common complaints. Accordingly, analgesics directed at these symptoms are among the top selling over the counter medicines in our society. Despite the lack specific data regarding the incidence of these disorders, much has been learned over the last decade regarding specific pathology and therapy. Generic terms such as TMJ Syndrome, or Myofascial Pain Dysfunction Syndrome (MPD Syndrome) are generally outdated. It is now possible to differentiate between true muscular disorders and those disorders with pathological changes of the temporomandibular joint.

    Basics - Overview
    The TMJ:
    "TMJ" stands for temporomandibular joint, or the jaw joint. The TMJs are the small joints in front of each ear that attach the lower jaw (mandible) to the skull. They allow you to perform such functions as opening and closing your mouth, chewing, speaking, swallowing, etc.
    TMJ Diseases/Disorders:
    TMJ diseases and disorders refer to a complex and poorly understood set of conditions, manifested by pain the the area of the jaw and associated muscles and limitations in the ability to make the normal movements of speech, facial expression, eating, chewing, and swallowing. Conditions that routinely affect other joints in the body, such as arthritis and trauma, also affect the TM joint.
    Number of TMJ Sufferers:
    The National Institute of Dental and Craniofacial Research of the National Institutes of Health indicates that 10.8 million people in the United States suffer from TMJ problems at any given time.

    Anatomy Modules TMJ Anatomy The temporomandibular joint, or TMJ, is the articulation between the condyle of the mandible and the squamous portion of the temporal bone. The condyle is elliptically shaped with its long axis oriented mediolaterally. The articular surface of the temporal bone is composed of the concave articular fossa and the convex articular eminence The MENISCUS is a fibrous, saddle shaped structure that separates the condyle and the temporal bone. The meniscus varies in thickness: the thinner, central intermediate zone separates thicker portions called the anterior band and the posterior band . Posteriorly, the meniscus is contiguous with the posterior attachment tissues called the bilaminar zone . The bilaminar zone is a vascular, innervated tissue that plays an important role in allowing the condyle to move foreward. The meniscus and its attachments divide the joint into superior and inferior spaces. The superior joint space is bounded above by the articular fossa and the articular eminence. The

    • I switched my mouse hand to the the hand on my side opposite my TMJ pain . This helped quite a bit. I also got a mouse with an optical scanner. There are a number of optical scanner mice available. I bought the Logitech Wheel Mouse Optical and was very happy with it. I'd tried other mice, a trackball and a glide pad, but for me the optical scanner mouse worked the best by far. It reduced much of the tension in my hand compared to the old trackball type mouse I had been using. I also cut back on my computer use while for awhile, especially when my jaw was in pain.

    You may not have heard of it, but you use it hundreds of times every day. It is the Temporo-Mandibular Joint (TMJ), the joint where the mandible (the lower jaw) joins the temporal bone of the skull, immediately in front of the ear on each side of your head. A small disc of cartilage separates the bones, much like in the knee joint, so that the mandible may slide easily; each time you chew you move it. But you also move it every time you talk and each time you swallow (every three minutes or so). It is, therefore, one of the most frequently used of all joints of the body and one of the most complex. You can locate this joint by putting your finger on the triangular structure in front of your ear. Then move your finger just slightly forward and press firmly while you open your jaw all the way and shut it. The motion you feel is the TMJ. You can also feel the joint motion in your ear canal. These maneuvers can cause considerable discomfort to a patient who is having TMJ trouble, and physicians use these maneuvers with patients for diagnosis.

    Properly positioned TMJ (Jaw Joint) has a natural space between ball (condyle) and ear canal brings relief from migraine headache pain. If the hinge point is toward the rear of the socket (fossa) three problems will occur.
    1. Critical nerves will be pinched and major blood vessels to the brain will be obstructed when squeezed between the ball of the jaw joint (condyle) and the ear canal.
    2.The cartilage for the socket (fossa) is positioned at the front of the joint only, leaving the rear hinge point with no cartilage protection.
