The American Society of Ophthalmic Plastic and Reconstructive Surgery Eyelid Skin Cancer Skin cancer often involves the skin of the eyelid or adjacent face. Eyelid skin cancers occur most often on the lower eyelid, but may be found anywhere on the eyelid margins, corners of the eye, eyebrow skin, or adjacent areas of the face. Usually, they appear as painless elevations or nodules. Occasionally, the eyelashes are distorted or missing. There may be ulcerations of the involved area, along with bleeding, crusting, and/or distortion of the normal skin structure. Such findings need to be evaluated and may require a biopsy to confirm the diagnosis of skin cancer. Causes of Skin Cancer Excessive exposure to sunlight is the single most important factor associated with skin cancers on the face, eyelids, and arms. Fair-skinned people develop skin cancers far more frequently than dark-skinned people. Skin cancers may also be hereditary. Types of Skin Cancer The most common types of skin cancers are basal cell carcinoma and squamous cell carcinoma. Both types enlarge locally and usually do not spread (metastasize) to distant parts of the body. However, with time, if not completely removed, either type will invade adjacent structures. It is important to know that basal and squamous cell carcinomas are relatively slow growing. Thus, when detected early and treated in a prompt and appropriate manner, there is a better chance of removing the tumor completely and minimizing the amount of tissue affected by the carcinoma. Sebaceous gland carcinoma and malignant melanoma are more serious forms of skin cancer because they may spread (metastasize) to other parts of the body. These types of skin cancer require prompt, aggressive treatment because of the threat of early spread.

Two patient information handouts on skin cancer, provided by an AAFP staff patient education writer, are presented on pages and O ne in six Americans develops skin cancer at some point. Skin cancer accounts for one third of all cancers in the United States. Most patients with skin cancer develop nonmelanoma skin cancer. This group of cancers includes basal cell carcinoma, the most common neoplasm worldwide, and squamous cell carcinoma. Fortunately, mortality associated with nonmelanoma skin cancer is unusual. However, malignant melanoma accounts for 75 percent of all deaths associated with skin cancer. Melanoma, the eighth most common malignancy in the United States, is the cancer with the most rapidly increasing incidence. One in 1,500 Americans born in 1935 were likely to develop melanoma, compared with one in 105 persons born in 1993.

Actinic keratosis
- A sun-caused, scaly growth. The actinic keratosis is a precancer that may become a squamous-cell skin cancer (carcinoma). Scientific studies say that between 1% and 20% of actinic keratoses may turn into squamous-cell skin cancer. It is usually found on sun-exposed skin. It is usually composed of a dry white scale on a pink or brown mat of tissue, usually about half the size of a dime. The actinic keratosis is a growth or tumor and is, by definition, one that stays high up in the skin and has not had a chance to go into the body.
Aminolevulinic acid (ALA) - A naturally occurring chemical in the human body that is converted to protoporphryn IX, especially in actively growing cells (usually a sign of disorder like precancer or cancer). Certain wavelengths of light cause protoporphryn to absorb light energy, killing cells in which protoporphryn is present in excess. This is the basis for photodynamic therapy, a treatment that will almost certainly be used successfully for precancers in the not-too-distant future Appendages - When used in dermatology, usually refers to parts of the epidermis and include the hair, nails and sweat glands (eccrine cooling sweat glands, apocrine arm/groin sweat/smelly glands).

WHAT’S THE PROBLEM? Australians have the highest rate of skin cancer in the world. Two out of every three Australians who live to the age of 75 can expect to develop some type of skin cancer. For many years Australians thought a good suntan was a sign of a healthy body. The reality is that a good suntan is a sign that you are being exposed to too much UV radiation. There are three main types of skin cancers in Australia. The most dangerous of these cancers is melanoma. Early detection of skin cancers is extremely important; it can stop the spread of cancer to other parts of the body. The three main types of skin cancer are: Basal cell carcinoma (BCC) - This is the most common, but least dangerous type of skin cancer. BCCs are usually found on the face and neck. BCCs begin to form as small round or flattened lumps which are either red, pale or pearly in colour and may have blood vessels (which look like tiny veins) over the surface. If untreated, they will spread and can leave bad scarring if you do not get early treatment. Squamous cell carcinoma (SCC) - This skin cancer is less common, but more dangerous than BCC. This cancer can develop on the lips, particularly the lower lip. SCCs are best described as red scaling areas that may bleed and become ulcerated. Very occasionally, SCCs may spread to the lymph nodes.
