Sexual Disorders
What are sexual disorders?

Sexual dysfunctions are disorders related to a particular phase of the sexual response cycle. For example, sexual dysfunctions include sexual desire disorders, sexual arousal disorders, orgasm disorders, and sexual pain disorders. If a person has difficulty with some phase of the sexual response cycle or a person experiences pain with sexual intercourse, he/she may have a sexual dysfunction.
Examples of sexual dysfunctions include:
  • Hypoactive Sexual Desire Disorder. This disorder may be present when a person has decreased sexual fantasies and a decreased or absent desire for sexual activity. In order to be considered a sexual disorder the decreased desire must cause a problem for the individual. In this situation the person usually does not initiate sexual activity and may be slow to respond to his/her partner's sexual advances. This disorder can be present in adolescents and can persist throughout a person's life. Many times, however, the lowered sexual desire occurs during adulthood, often times following a period of stress.

The DSM IV describes Hypoactive Sexual Desire Disorder as:
    • Persistent or recurrent deficient (or absent) sexual fantasies and desire for sexual activity. The judgment of deficiency or absence is made by the clinician, taking into account factors that affect sexual functioning, such as age, sex, and the context of the person's life."

    Taken from DIAGNOSTIC AND STATISTICAL MANUAL OF MENTAL DISORDERS (Third Edition) published by the American Psychiatric Association, 1987. Pgs 279-281 Sexual Disorders. SEXUAL DISORDERS
    The Sexual Disorders are divided into two groups. The Paraphilias are characterized by arousal in response to sexual objects or situations that are not part of normative arousalactivity patterns and that in varying degrees may interfere with the capacity for reciprocal, affectionate sexual activity. The Sexual Dysfunctions are characterized by inhibitions in sexual desire or the psychophysiologic changes that characterize the sexual response cycle. Finally, there is a residual class, Other Sexual Disorders, for disorders in sexual functioning that are not classifiable in any of the specific categories.
    PARAPHILIAS
    The essential feature of disorders in this subclass is recurrent intense sexual urges and sexually arousing fantasies generally involving either (1) nonhuman objects, (2) the suffering or humiliation of oneself or one's partner (not merely simulated), or (3) children or other nonconsenting persons. The diagnosis is made only if the person has acted on these urges, or is markedly distressed by them. In other classifications these disorders are referred to as Sexual Deviations. The term Paraphilia is preferable because it correctly emphasizes that the deviation (para) lies in that to which the person is attracted (philia).

Swaab references Although the majority of sexual dysfunction probably has a physical basis it is fitting to mention some sexual disorders here because dysfunctions, whether mainly due to physical or psychological causes, can result in distress. For example, the individual with a sexual disorder may suffer related anxiety and sexual frustration which in turn leads to insomnia, and that insomnia may be the presenting complaint to the GP. The individual's close relationships may suffer and tension may build up in the family as a whole. According to DSM-IV (the American Psychiatric Association's classification system) there are a dozen or so sexual disorders.     Mental Health Disorders Sexual Disorders (excluding paraphilias) In this section we consider the kinds of problems that can arise in what most societies and most individuals consider as "normal" heterosexual relationships. In the section on paraphilias (literally, abnormal attractions) we consider diagnoses of sexual problems that involve socially disapproved forms of sexual behavior, and in the section on gender identity disorders we cover problems that arise when people are not satisfied with their biologically determined sexual identity. The importance of sex is so great that it is hardly necessary to say, in the case of each possible diagnosis, that the symptoms must "cause clinically significant distress or impairment in social, occupational, or other important areas of functioning." This DSM-IV requirement is almost guaranteed to apply to any substantial and persistent difficulty in an area so critical to human self-images and relationships.

    Sexual impotence, also defined as erectile dysfunction, is a very common problem which affects most men at least once during their lifetime. The incidence of erectile dysfunction varies with age, and increases in the proportion of affected men, from 7 to 8 %, from 20 to 39 years of age; to 55-60 %, in men older than 70 years; according to a study carried out in the USA. The largest increase is seen in men in the range of 60-69 years. Is is estimated that at least 10 million men suffer from chronic impotence in the USA, and that at least 20 million more men have less severe forms of erectile dysfunction associated to age, chronic health problems or psychological problems. Although in the past sexual impotence was regarded as having almost exclusively psychic causes, we now know that from 85 to 90 % of all cases of erectile dysfunction have organic causes, such as peripheral vascular diseases, diabetes, imbalances in the sexual hormones, the effect of medications, etc. This has encouraged medical science to discover new forms of treatment, which are available and work well for a large number of patients. These treatments range from oral tablets and vasoactive drugs which can be injected into the penis, to vacuum pumps and implantable penis prostheses. Efectiveness of therapies may range from 40 to 90 %, depending on the proper choice of method.
