Inland Northwest Health Services (INHS) is a regional collaboration to improve patient care through innovative technology solutions and shared services. INHS has electronically linked 30 facilities, with a total of more than 2,500 beds, across eastern Washington and northern Idaho. These facilities share a common information system featuring a unified Master Patient Index. Clinics outside the network receive data via standard electronic messages. INHS has built on this foundation to provide other services, including a large telehealth system, electronic transmission of clinical images and laboratory test results, and wireless availability of patient records for clinicians in hospitals.     Executive summary (excerpts) Australian Health Ministers demonstrated their commitment to the safety and quality agenda by establishing the Australian Council for Safety and Quality in Health Care in January 2000. The Council's role is to lead national efforts to promote systemic improvements in the safety and quality of health care in Australia, with a particular focus on minimising the likelihood and effects of error. There is now a groundswell of support for and involvement in improving patient safety, at all levels, Australia-wide. The commitment and action of all jurisdictions have been vital to this, as has the widespread involvement of consumers, health care professionals and managers, academics, safety experts and other key players.

    Policy The National Eye Institute (NEI) is establishing the following system for the appropriate oversight and monitoring of the conduct of clinical trials to ensure the safety of participants and the validity and integrity of the data from all NEI-supported or -conducted clinical trials ( http://grants.nih.gov/grants/guide/notice-files/not98-084.html Introduction A detailed data and safety monitoring plan for all clinical trials must be submitted as part of the Research Plan portion of the application. This plan should address how the investigator will implement the data and safety monitoring activities. The monitoring plan must include a general description of the mechanisms for reporting adverse events to the Institutional Review Board(s) (IRB), the Food and Drug Administration (FDA), and the National Institutes of Health (NIH). Oversight of the monitoring activity will be the joint responsibility of the investigator and the NEI program director.     Historically, the U.S. and the countries of the Former Soviet Union have had independent research programs on many of the topics that are vital to the safety of nuclear reactors. Each country has developed unique capabilities and strengths over the years. It has been recognized that we can build on this combination of strengths by having an open exchange of information and by engaging in collaborative research. This approach will apply the best talents to the problems that must be addressed and it will maximize the benefits that can be derived from the available resources. Working together, the International Nuclear Safety Centers have initiated activities and collaborative research projects that address internationally-identified nuclear safety issues. The thrust of these projects is to use the resources and talents of the scientists associated with the centers to do research that will produce benefits for all participating countries.
